Our Mission
The Van Wert County Hospital Association serves Van Wert County, Ohio and
adjacent communities as a financially independent nonprofit community health
system. Van Wert County Hospital Association recognizes the collaborative efforts
and contributions of individuals and organizations that strive to meet the health care
needs of the communities we serve.
The Association will meet community health needs by continually endeavoring to
improve the quality of patient care we provide within our resource capabilities.
Programs and services sponsored by the Association will serve as an access point
to health care within our community. We accept responsibility for providing community
leadership.
The Association will remain abreast of national, regional and state trends,
enabling us to continually assess and meet the needs of the communities

A Message from the President & Chief Executive Officer Mark Minick
The past year was again successful for Van Wert County Hospital. We experienced success financially, strategically and
in client relations. The commitment and dedication of our staff, our doctors and our volunteers are responsible for this
success as we continue to strive to meet the healthcare needs of our community.
From a financial standpoint, we experienced an unaudited benchmark profit of
$8,983,715. As always, these resources will be returned to our patients and
community through new programs and services, new technologies and new care
models to improve care to our patients. These resources will also be used to meet
the challenges and changes coming through the transformation of the healthcare
system with the Affordable Care Act.
New services include a new digital fluoroscopy unit to improve “real time” imaging
of internal organs. Patient registration was also relocated to a more central location
to provide “line of sight recognition” from our main entrance.
A community health assessment, a collaborative effort that included a number of
providers and area leaders, was also completed. The Van Wert County Health
Collaborative identified and prioritized a number of areas to improve the

health of individuals, families and our community. The implementation
of these priorities should have a lasting effect on the health and
overall wellness of the Van Wert community.
In November, the hospital was awarded the Guardian of
Excellence Award for inpatient satisfaction from Press Ganey
& Associates, a national client satisfaction vendor. One of the
hospital’s main goals, past, present and future, is happy,

For the last 108 years, Van Wert County Hospital has been and continues to be dedicated to providing superior patientcentered care while remaining a leader in the health and wellness of our community.
Healthcare, as we know it, is truly transforming. Community health, economic challenges and healthcare reform are
powerful forces of change. We are under enormous pressure to improve quality while decreasing costs. As a local
independent community hospital, Van Wert County Hospital is uniquely equipped to handle
these changes. Decisions are made right here. Our governing board is comprised
of local volunteers who are committed to meeting the needs of our community.
Local decision-making empowers our hospital to react timely and effectively to
the challenges ahead.

As healthcare evolves, Van Wert County Hospital will continue to lead communitywide health improvement efforts. According to the Van Wert County Health
Assessment, 72.9% of our residents are overweight or obese. Many of our
residents are struggling with substance abuse. And our children are struggling
with these same issues. We must expand beyond health care to truly impact
the health of Van Wert County.
Patients are center to the care provided at Van Wert County Hospital.
Physicians and staff take great pride in providing personalized care to the
community. Recently, these efforts were recognized on a national
level with the presentation of the Guardian of Excellence award.
This achievement reflects the organization’s commitment to
delivering outstanding service and quality. On behalf of the
board, we applaud each and every member of the Van Wert
County Hospital team for their dedication providing an
excellent patient experience.

Progress is the movement toward a goal or course of action.
Progress has certainly played its part in our Red Cross organization
throughout the year. In 2013, our former executive director retired
in late January. In the interim, I picked up some additional duties
during the search for a new executive director to ensure
uninterrupted services to the hospital. With the support of Karen
Jantz, Lima Red Cross, the program ran smoothly until Sandy
Lane accepted the director’s position, in July 2013.
Carol Hennis – Volunteer Coordinator
Amidst all of this forward progress and change, I am proud to
acknowledge that we are the only Red Cross office in the area
that operates a hospital volunteer program. We are beyond grateful
to the 45 active volunteers who have collectively donated 6,045
hours of service to the hospital this year, and we are thankful to
the community for welcoming our services. We are proud to be a
part of Van Wert County.
So as I have spent this past year learning to appreciate the meaning
of progress and change, I leave you with the question to ponder:
Is it possible to make progress without making changes?
As for change, you may have seen an article published in the
newspaper regarding our Chapter’s name. We are no longer the
Van Wert Chapter of the American Red Cross. We are now the
American Red Cross of Van Wert. That means, instead of just
serving Van Wert County, as we have in the past, our Chapter will
now provide mutual aid to a 6 or 7 county area. Should a disaster
occur in our area, the surrounding offices will collaboratively work
together to help if and where it is needed.

2013 Medical Executive Committee
THE SEWING LADIES
When the hospital was first built in 1911,
the Lady Board of Managers supervised
the hospital operations. Over the years, the
role of the Lady Board of Managers has
evolved. Today, this group supports the
hospital and community by volunteering
their time and talents in both the mending
and making of new articles for the hospital.

Tuesday morning is Sewing Day. Ladies
from the city and the twelve townships
gather to assist the hospital. The workforce
can vary from 3-6 ladies. The ladies are
always eager to work on any projects they
are provided.
The Sewing Ladies volunteer over 928
hours making over 610 new and 208
mended items annually.

A message from the President of the Medical Staff
Thomas Conte, M.D.
On behalf of the hospital and entire medical staff, I would like to congratulate one of Van Wert County Hospital’s
physicians and newly termed President of the Medical Staff, Dr. Jennifer Hohman, who was
presented with two-consecutive awards, recognizing her commitment toward improving
childhood immunization rates. Dr. Hohman was one of only nine practices in the state to
receive this recognition. We applaud Dr. Hohman for her continued service to the
children of our community.
It is also with great pride that I have the opportunity to introduce two new physicians to
the Van Wert County Hospital medical team, Dr. Samar Hijazi and Dr. David Barnes.
Samar Hijazi, M.D. joins our team of physicians following service in Amman, Jordan.
Dr. Hijazi is board-certified in internal medicine. She truly is an asset to our medical
staff. Her vast experiences allow her to provide comprehensive medical care to her adult
patients, including prevention, diagnosis and treatment of diseases. Dr. Hijazi is pleased
to be a part of our community.

We are also pleased to announce the addition of David Barnes, M.D. to our medical
staff. Dr. Barnes serves as a family practitioner at Family Medicine Associates in
Rockford, Ohio. Prior to joining our medical team, Dr. Barnes volunteered
his service to Central-American refugees. He also has served as a
Christian Missionary caring for individuals of social and economic
needs. We are happy to welcome Dr. Barnes to our team.
With the arrival of Drs. Hijazi and Barnes, the number of Van
Wert Medical Services’ employed physicians has grown to 12
providers. This multi-specialty physician group offers comprehensive
services conveniently located under one roof. Indeed, 2013
was a successful year for Van Wert Medical Services with fiscal
growth, medical staff expansion and recruitment efforts.

It is certain that our team will continue to deliver the highest
quality healthcare to our patients. I look forward to another
year of caring for the Van Wert community. Thank you for

The Twig I organization operates the
Hospital Thrift Shop on Central
Avenue, in Van Wert. Our Twig I
organization is exceedingly fortunate
for the many dedicated volunteers
who graciously share their gifts and
time to organize and operate the
Hospital Thrift Shop, as well as for the
support the community continues to
show the shop year after year.
As in years past, the community of
Van Wert continued to generously
support the Twig II organization
through in-kind donations and item
purchases throughout 2013. As a
result, we are proud to announce, the
Twig II organization was able to
contribute one of the largest monetary
donations to the hospital in the history
of the organization.
The Twig III organization operates the
Hospital Gift Shop, available for patients, families, and hospital staff. The
shop is open 24 hours a day, 7 days a
week, and we are extremely grateful
for the ladies who make this service
available to our entire hospital family
and guests.
The Thrift Shop operates entirely from
generous donations by the community
and our members. Our members invest
their many, varied talents to make
items to be sold to the community at
the Thrift Shop. In turn, all of the net
proceeds generated from Thrift Shop
purchases are donated to the hospital
to support ongoing needed equipment
purchases and upgrades for improved
patient care. Together, through the
support of the Thrift Shop, the community
and Twig I and Twig II members’ work
is in action every day.
The members of Twig II share their
talents and dedicate their time for the
purpose of operating and maintaining
the Hospital Thrift Shop alongside of
the Twig I organization. There are 35
volunteers and 11 emeritus members
who make up the Twig II organization.
Each member is greatly appreciated,
as they continue to sustain the
organization and its fundraising efforts
year after year. Each lady is freely
giving of their time and talents, knowing
that the reward of their efforts plays a
pivotal role in the hospital, the
community, and the organization.
Over the past year, the Twig IV
members have worked diligently to
ensure a successful year for our
hospital fundraising projects. Our
Sweet Shoppe, open on the second
Thursday of the month, offers an array
of homemade goodies made by our
talented members. Our volunteers
also sell RADA Cutlery at the Sweet
Shoppe. We encourage the public to
stop by our monthly sale.
The Twig I organization consists of
41 active members, as well as 17
emeritus members. These faithful
ladies, alongside of one part-time paid
shop manager, work tirelessly to
maintain the Hospital Thrift Shop.

The Twig III organization would like to
thank the community of Van Wert,
surrounding counties, and staff of
Van Wert County Hospital for making
2013 a memorable year. In November,
we celebrated the twentieth year for
our annual Tree of Lights ceremony.
Without the support of such a united
community, we would not have been
able to reach this milestone. We are
proud to report, in the twenty years
our organization has been holding the
Tree of Lights ceremony, the project
has raised over $72,000.00, which is
in turn donated to the hospital, so that
the hospital can continue to invest in
the most up-to-date medical equipment
and provide the highest quality
healthcare to the members of our
community.
The members of Twig III would like to
sincerely thank the Van Wert community
and hospital staff for their continued
support of these projects.

We are also proud to share that each
year the Twig IV members host the
annual Poinsettia Sale in early
December. We are blessed by the
community and hospital support of
this event. Our sales this year made
this a very worthwhile project.
Lastly, we hosted a number of Artisan
Sales. Local and out-of-town vendors
presented a fantastic display of craft
and various other items for the staff
and community to purchase. All
proceeds from our projects and
events throughout year are given to
the hospital to support equipment
upgrades and improvements.
The Twig IV organization appreciates
all the support received during 2013.
You have helped to make each event
and project successful and helped to
make the impossible, possible. We
now look forward to what 2014 brings!

About the Twig Organization
2013 Medical Staff
The Twig Association was established more than sixty years ago.
Today, each of the four member Twig organizations continues to hold
true to its founding purpose. The members of the Twig organizations
make it their mission to serve the Van Wert County community and “to
contribute financially to the Van Wert County Hospital; to provide personal

About the Twig Organization
2013 Medical Staff
The Twig Association was established more than sixty years ago.
Today, each of the four member Twig organizations continues to hold
true to its founding purpose. The members of the Twig organizations
make it their mission to serve the Van Wert County community and “to
contribute financially to the Van Wert County Hospital; to provide personal
service gratis for the benefit of Van Wert County Hospital; and to promote
better community health standards.”

A message from the President of the Medical Staff
Thomas Conte, M.D.
On behalf of the hospital and entire medical staff, I would like to congratulate one of Van Wert County Hospital’s
physicians and newly termed President of the Medical Staff, Dr. Jennifer Hohman, who was
presented with two-consecutive awards, recognizing her commitment toward improving
childhood immunization rates. Dr. Hohman was one of only nine practices in the state to
receive this recognition. We applaud Dr. Hohman for her continued service to the
children of our community.
It is also with great pride that I have the opportunity to introduce two new physicians to
the Van Wert County Hospital medical team, Dr. Samar Hijazi and Dr. David Barnes.
Samar Hijazi, M.D. joins our team of physicians following service in Amman, Jordan.
Dr. Hijazi is board-certified in internal medicine. She truly is an asset to our medical

We are also pleased to announce the addition of David Barnes, M.D. to our medical
staff. Dr. Barnes serves as a family practitioner at Family Medicine Associates in
Rockford, Ohio. Prior to joining our medical team, Dr. Barnes volunteered
his service to Central-American refugees. He also has served as a
Christian Missionary caring for individuals of social and economic
needs. We are happy to welcome Dr. Barnes to our team.
With the arrival of Drs. Hijazi and Barnes, the number of Van
Wert Medical Services’ employed physicians has grown to 12
providers. This multi-specialty physician group offers comprehensive
services conveniently located under one roof. Indeed, 2013
was a successful year for Van Wert Medical Services with fiscal
growth, medical staff expansion and recruitment efforts.

The Twig I organization operates the
Hospital Thrift Shop on Central
Avenue, in Van Wert. Our Twig I
organization is exceedingly fortunate
for the many dedicated volunteers
who graciously share their gifts and
time to organize and operate the
Hospital Thrift Shop, as well as for the
support the community continues to
show the shop year after year.
As in years past, the community of
Van Wert continued to generously
support the Twig II organization
through in-kind donations and item
purchases throughout 2013. As a
result, we are proud to announce, the
Twig II organization was able to
contribute one of the largest monetary
donations to the hospital in the history
of the organization.
The Twig III organization operates the
Hospital Gift Shop, available for patients, families, and hospital staff. The
shop is open 24 hours a day, 7 days a
week, and we are extremely grateful
for the ladies who make this service
available to our entire hospital family
and guests.
The Thrift Shop operates entirely from
generous donations by the community
and our members. Our members invest
their many, varied talents to make
items to be sold to the community at
the Thrift Shop. In turn, all of the net
proceeds generated from Thrift Shop
purchases are donated to the hospital
to support ongoing needed equipment
purchases and upgrades for improved
patient care. Together, through the
support of the Thrift Shop, the community
and Twig I and Twig II members’ work
is in action every day.
The members of Twig II share their
talents and dedicate their time for the
purpose of operating and maintaining
the Hospital Thrift Shop alongside of
the Twig I organization. There are 35
volunteers and 11 emeritus members
who make up the Twig II organization.
Each member is greatly appreciated,
as they continue to sustain the
organization and its fundraising efforts
year after year. Each lady is freely
giving of their time and talents, knowing
that the reward of their efforts plays a
pivotal role in the hospital, the
community, and the organization.
Over the past year, the Twig IV
members have worked diligently to
ensure a successful year for our
hospital fundraising projects. Our
Sweet Shoppe, open on the second
Thursday of the month, offers an array
of homemade goodies made by our
talented members. Our volunteers
also sell RADA Cutlery at the Sweet
Shoppe. We encourage the public to
stop by our monthly sale.
The Twig I organization consists of
41 active members, as well as 17
emeritus members. These faithful
ladies, alongside of one part-time paid
shop manager, work tirelessly to
maintain the Hospital Thrift Shop.

The Twig III organization would like to
thank the community of Van Wert,
surrounding counties, and staff of
Van Wert County Hospital for making
2013 a memorable year. In November,
we celebrated the twentieth year for
our annual Tree of Lights ceremony.
Without the support of such a united
community, we would not have been
able to reach this milestone. We are
proud to report, in the twenty years
our organization has been holding the
Tree of Lights ceremony, the project
has raised over $72,000.00, which is
in turn donated to the hospital, so that
the hospital can continue to invest in
the most up-to-date medical equipment
and provide the highest quality
healthcare to the members of our
community.
The members of Twig III would like to
sincerely thank the Van Wert community
and hospital staff for their continued
support of these projects.

annual Poinsettia Sale in early

community and hospital support of
this event. Our sales this year made
this a very worthwhile project.
Lastly, we hosted a number of Artisan
Sales. Local and out-of-town vendors
presented a fantastic display of craft
and various other items for the staff
and community to purchase. All
proceeds from our projects and
events throughout year are given to
the hospital to support equipment
upgrades and improvements.

THE SEWING LADIES
When the hospital was first built in 1911,
the Lady Board of Managers supervised
the hospital operations. Over the years, the
role of the Lady Board of Managers has
evolved. Today, this group supports the
hospital and community by volunteering
their time and talents in both the mending
and making of new articles for the hospital.

Tuesday morning is Sewing Day. Ladies
from the city and the twelve townships
gather to assist the hospital. The workforce
can vary from 3-6 ladies. The ladies are
always eager to work on any projects they
are provided.
The Sewing Ladies volunteer over 928
hours making over 610 new and 208
mended items annually.

Progress is the movement toward a goal or course of action.
Progress has certainly played its part in our Red Cross organization
throughout the year. In 2013, our former executive director retired
in late January. In the interim, I picked up some additional duties
during the search for a new executive director to ensure
uninterrupted services to the hospital. With the support of Karen
Jantz, Lima Red Cross, the program ran smoothly until Sandy
Lane accepted the director’s position, in July 2013.

Amidst all of this forward progress and change, I am proud to
acknowledge that we are the only Red Cross office in the area
that operates a hospital volunteer program. We are beyond grateful
to the 45 active volunteers who have collectively donated 6,045
hours of service to the hospital this year, and we are thankful to
the community for welcoming our services. We are proud to be a
part of Van Wert County.
So as I have spent this past year learning to appreciate the meaning
of progress and change, I leave you with the question to ponder:
Is it possible to make progress without making changes?
As for change, you may have seen an article published in the
newspaper regarding our Chapter’s name. We are no longer the
Van Wert Chapter of the American Red Cross. We are now the
American Red Cross of Van Wert. That means, instead of just
serving Van Wert County, as we have in the past, our Chapter will
now provide mutual aid to a 6 or 7 county area. Should a disaster
occur in our area, the surrounding offices will collaboratively work
together to help if and where it is needed.

A Message from the Board of Trustees’ Chairman Jon Bagley
For the last 108 years, Van Wert County Hospital has been and continues to be dedicated to providing superior patientcentered care while remaining a leader in the health and wellness of our community.
Healthcare, as we know it, is truly transforming. Community health, economic challenges and healthcare reform are
powerful forces of change. We are under enormous pressure to improve quality while decreasing costs. As a local
independent community hospital, Van Wert County Hospital is uniquely equipped to handle
these changes. Decisions are made right here. Our governing board is comprised
of local volunteers who are committed to meeting the needs of our community.
Local decision-making empowers our hospital to react timely and effectively to
the challenges ahead.

As healthcare evolves, Van Wert County Hospital will continue to lead communitywide health improvement efforts. According to the Van Wert County Health
Assessment, 72.9% of our residents are overweight or obese. Many of our
residents are struggling with substance abuse. And our children are struggling
with these same issues. We must expand beyond health care to truly impact
the health of Van Wert County.
Patients are center to the care provided at Van Wert County Hospital.
Physicians and staff take great pride in providing personalized care to the
community. Recently, these efforts were recognized on a national
level with the presentation of the Guardian of Excellence award.
This achievement reflects the organization’s commitment to
delivering outstanding service and quality. On behalf of the
board, we applaud each and every member of the Van Wert
County Hospital team for their dedication providing an
excellent patient experience.

A Message from the President & Chief Executive Officer Mark Minick
The past year was again successful for Van Wert County Hospital. We experienced success financially, strategically and
in client relations. The commitment and dedication of our staff, our doctors and our volunteers are responsible for this
success as we continue to strive to meet the healthcare needs of our community.
From a financial standpoint, we experienced an unaudited benchmark profit of
$8,983,715. As always, these resources will be returned to our patients and
community through new programs and services, new technologies and new care
models to improve care to our patients. These resources will also be used to meet
the challenges and changes coming through the transformation of the healthcare
system with the Affordable Care Act.
New services include a new digital fluoroscopy unit to improve “real time” imaging
of internal organs. Patient registration was also relocated to a more central location
to provide “line of sight recognition” from our main entrance.
A community health assessment, a collaborative effort that included a number of
providers and area leaders, was also completed. The Van Wert County Health
Collaborative identified and prioritized a number of areas to improve the

health of individuals, families and our community. The implementation
of these priorities should have a lasting effect on the health and
overall wellness of the Van Wert community.
In November, the hospital was awarded the Guardian of
Excellence Award for inpatient satisfaction from Press Ganey
& Associates, a national client satisfaction vendor. One of the
hospital’s main goals, past, present and future, is happy,

the nation achieve this recognition.

Our Mission
The Van Wert County Hospital Association serves Van Wert County, Ohio and
adjacent communities as a financially independent nonprofit community health
system. Van Wert County Hospital Association recognizes the collaborative efforts
and contributions of individuals and organizations that strive to meet the health care
needs of the communities we serve.
The Association will meet community health needs by continually endeavoring to
improve the quality of patient care we provide within our resource capabilities.
Programs and services sponsored by the Association will serve as an access point
to health care within our community. We accept responsibility for providing community
leadership.
The Association will remain abreast of national, regional and state trends,
enabling us to continually assess and meet the needs of the communities

excellence in which all health care professionals strive to identify, provide and
