un succès sur toute la ligne
L
e FORUM canadien de chirurgie 2006 tenu à
Calgary, en Alberta, en septembre s’est avéré
un succès sur toute la ligne. Cette activité
s’est clairement établie comme « la » réunion
nationale de chirurgie, attirant des chirurgiens
universitaires et communautaires de tous les coins
du pays. Parmi le nombre record de 651
participants figuraient 368 chirurgiens, 142
résidents, 16 étudiants et 13 infirmières. Le soutien
de l’industrie a atteint un sommet sans précédent
en raison de perspectives de croissance
favorables.

The major participants – the Canadian
Association of General Surgeons, the Canadian
Society of Colon and Rectal Surgeons and the
Canadian Association of Thoracic Surgeons –
were joined by other participating societies
including, for the first time, the Canadian
Association of Pediatric Surgeons, the Alberta
Association of General Surgeons, the American
College of Surgeons, the Canadian Associations
of Surgical Chairmen, the Canadian Association
of University Surgeons, the Canadian Society of
Surgical Oncology, the Canadian Undergraduate
Surgical Education Committee, James IV
Association of Surgeons and the Trauma
Association of Canada. The fledgling
Association of Bariatric Surgeons also met under
the umbrella of the FORUM, as of course did a
number of informal special interest groups. We
acknowledge, as well, the important role of the
Royal College of Physicians and Surgeons of
Canada and its active support and participation
in the meeting, including the opening plenary
devoted to Continuing Professional
Development and Professionalism for Surgeons:
A Lifelong Commitment to Learning, the direct
funding of speakers and the underwriting of the
Gallie Lecture, this year delivered by Dr. Ori
Rotstein of Toronto and entitled, Novel
Resuscitation Strategies Aimed at Improving
he Canadian Surgery FORUM 2006 held in
Calgary in September was a success by
every measure. The event clearly has
established itself at “the” national surgical
meeting, drawing academic and community
surgeons from all parts of Canada. In all there
were a record 651 registrants, including 368
surgeons, 142 residents, 16 students and 13
nurses. Support from industry was at an all time
high with encouraging prospects for growth.

The Canadian Surgery FORUM 2007 scheduled
for the Royal York in Toronto, from September
6th to the 9th, promises to be even better, so mark
you calendar and plan to attend! Details of the
program will soon be forthcoming.
The busy schedule, including Paper and Poster
Sessions and a plethora of named and invited
lectures, debates, seminars and symposia were
balanced by equally inviting opportunities for
networking and socializing. The highlight being
the Presidents’ Dinner held at the prestigious
Petroleum Club on Saturday evening.
The Postgraduate Courses offered this year
included Endocrine Surgery, Part 3; Advances in
Minimally Invasive Gastrointestinal Surgery;
and, Preoperative Evaluation and Neoadjuvant
Treatments of Intraabdominal Malignancies and
a potpourri of colorectal problems, including
diverticular disease, lower GI bleeding, bowel
obstruction, inflammatory bowel disease,
molecular genetics of colorectal cancer,
laparoscopic colon resection and everything
from hemorrhoids to horseshoe kidneys to
presacral lesions.
Outcome in Trauma Patients. The Royal College
Medalist in Surgery was Dr. Sandro Rizoli and
we are delighted the FORUM was chosen as the
venue for the Royal College Medalist in Surgery
Lecture entitled, The Immunomodulatory Effects
of Hypertonic Saline Resuscitation in Trauma
Patients.

de la réunion, notamment la séance d’ouverture,

Parmi les cours de perfectionnement offerts

de réseautage et des activités sociales tout
aussi intéressantes. Le Dîner du président tenu
au prestigieux Petroleum Club le samedi soir
s’est avéré le point culminant de la réunion.
Le FORUM canadien de chirurgie 2007, qui se
tiendra à l’hôtel Royal York à Toronto, en Ontario,
du 6 au 9 septembre, s’annonce encore meilleur.

Chers collègues,
C’est à la fois avec honneur et avec humilité, compte tenu des
chirurgiens qui m’ont précédé dans ce poste, que j’accède à la
présidence de l’Association canadienne des chirurgiens
généraux. En collaboration avec un exécutif d’expérience, un
conseil dévoué et un secrétariat compétent qui travaille fort, je
m’engage à poursuivre les efforts déployés par le passé pour
faire progresser les objectifs de notre Association. N’hésitez
surtout pas à nous indiquer les domaines où nous réussissons
bien ainsi que ceux qui méritent des efforts supplémentaires.
Le FORUM canadien de chirurgie 2006 tenu à Calgary en
septembre, le meilleur jusqu’ici, a établi cette activité comme LA
réunion des chirurgiens généraux canadiens de tous les milieux.

Je suis très heureux de constater que le site Web de l’ACCG est
opérationnel. Le site est le fruit d’efforts concertés de la part des
cadres au cours des dernières années. Je suis d’avis que nous
avons maintenant un site attrayant, intuitif, robuste et flexible
qui est en mesure de combler les besoins de nos membres tout
en facilitant grandement le travail du secrétariat. Consultez le site
et faites-nous part de vos commentaires (www.cags-accg.ca).
Nous avons hâte au jour où le site répondra aux besoins de nos
membres étudiants et résidents pendant leur cheminement en
formation jusqu’à la pratique autonome. Un bavardoir sécurisé,
loin des regards indiscrets des directeurs de programmes (pour
vrai!), l’affichage de cas intéressants, les perspectives de stages
de perfectionnement et d’emplois, voilà quelques-une des
multiples possibilités, lesquelles ne sont limitées que par
l’imagination et l’initiative.

Je pense souvent à la perte que représente pour nous le décès
de notre cher Tom Williams. Son livre, History of CAGS – The
First 25 Years – sera publié bientôt pour être disponible dans le

accepté d’agir à titre d’archiviste de l’Association. Bill a une
mine de connaissances et d’expérience à son compte, grâce à
quelque treize années de service en qualité de membre de
l’exécutif.
Votre exécutif est sérieusement préoccupé du conflit d’opinion
acrimonieux qui perdure entre les spécialistes du Québec et le
gouvernement provincial. Il est à souhaiter que les esprits
calmes prennent le dessus et qu’une solution acceptable pour
les deux parties et responsable du point de vue fiscal soit
identifiée, une solution qui :

respecte le régime de négociations collectives;
assure l’accès par les patients aux soins de qualité en
temps opportun; et
est conforme aux principes du professionnalisme et de

médecins – sans oublier que le mot médecin veut dire
enseignant.

Your executive regards with grave concern the acrimonious
ongoing disagreement between specialists in the Province of
Quebec and the Provincial Government. Hopefully cooler heads
will prevail and a mutually acceptable, fiscally responsible
solution to this impasse will be found that:
I am delighted to announce that Bill Mackie of Edmonton has
agreed to serve as archivist to the Association. Bill brings to the
position a wealth of knowledge and experience derived of some
thirteen years service as a member of the executive.
I often reflect on our loss in the passing of our beloved Tom
Williams. Tom’s History of CAGS - The First 25 Years is
nearing publication and will be out to you in the New Year.
In a readership survey recently conducted by the Canadian
Journal of Surgery the selected Evidence Based Reviews
appearing in that publication were rated very highly. This is but
one example of CAGS commitment to working with the scientific
editors and management staff of the Journal to enhance the
profile of this publication, the official organ of the Association.

Sous l’habile gestion de Robin McLeod, le Programme des
études fondées sur des données scientifiques en chirurgie est
dorénavant en ligne, stimulant la discussion et des
commentaires intéressants. Déjà été adoptées par l’American
College of Surgeons, ces études sont de plus en plus utilisées
par les chirurgiens en exercice et les programmes de formation
américains. Il est prévu qu’elles seront élargies afin d’englober
d’autres surspécialités, dont la chirurgie colorectale constituerait
un premier choix naturel, s’avérant utile à bon nombre de nos
membres.
The Evidence Based Reviews in Surgery under the capable
stewardship of Robin McLeod are once again on line and
generating thoughtful discussion and comment. These Reviews
have already been adopted by the American College of
Surgeons and are increasingly being used by practicing
surgeons and training programmes south of the boarder. There
are plans to expand the reviews to include other subspecialty
areas. Colorectal surgery seems a natural fit in the first instance
and will resonate with much of our membership.
We are particularly anxious that the site serve the needs of our
Student and Resident Members as they move through their
training into independent practice. A secure chat room away
from the prying eyes of Programme Directors (Honest!),
postings of interesting cases, fellowship and job opportunities
are only a few of an infinite array of possibilities limited really
only by one’s imagination and initiative.
I am delighted to report that the CAGS web site is once again up
and running. This site is the product of the concerted efforts of
executives over several years. I believe we now have an
attractive, intuitive, robust but flexible site that is capable of
responding to the needs of our membership and will greatly
facilitate the work of the secretariat. Check it out and give us

The Canadian Surgery FORUM 2006 held in Calgary in
September was the best yet establishing the FORUM as THE
meeting for Canadian General Surgeons of all stripes.
See elsewhere in this Newsletter for more details. I sincerely
thank all those who had any part in the planning and execution
of this event. Already plans are well underway for next year’s
FORUM, again under the direction of Elijah Dixon, Chair of
CAGS Programme Committee. So, mark your calendar and to
avoid disappointment make plans early to attend:
I am greatly honoured and, when I survey the list of surgeons
who have preceded me in this office, profoundly humbled to
have been elected President of the Canadian Association of
General Surgeons. In collaboration with an experienced
executive, a dedicated board and a capable, hard working
secretariat I pledge to build on the good work of the past in
furthering the aims of our Association. Do let us know both
where we are doing well and where we need to expend more
effort.

During the power surge the lab and x-ray facilities were rendered
completely unserviceable, the relays controlling the back up

The second interruption occurred while the transformer feeding
the hospital was being serviced on a quiet Sunday evening. This
routine, scheduled maintenance erupted into chaos when a
power surge started an electrical fire in the basement. Only quick
action by those on site averted a major disaster. Nonetheless
lights and electrical equipment, including monitors and
computers throughout the facility were cooked filling the place
with acrid fumes and necessitating a partial evacuation.
Unlike some hospitals in less affluent countries we are
unaccustomed to such situations and have therefore become
somewhat complacent. Nowhere in the facility was there either a
readily available source of water or the necessary pitchers /
buckets to permit hand washing, bathing or the flushing of
toilets. Staff soon tired of hauling water during an emergency
that in the end lasted less than 24 hours. The accompanying boil
order has yet to be lifted. Fortunately we have had no examples
of fisticuffs erupting in the line ups for store bought bottled
water as reported recently during the recent floods in British
Columbia and to my knowledge no one has become ill from
consuming contaminated water.
e recently had two “unscheduled” interruptions to service
in our small hospital which severely disrupted the usually
smooth flow of activities. In fact nothing was flowing when,
prior to our receiving any insulating snowfall, an early cold snap
froze the water intake pipe at the reservoir behind the town. With
no functioning back up system in place things quickly became
“backed up” –in the worst way! Furthermore, because the motor
driving the central vacuum system which supplies wall suction
is water cooled we suddenly had no suction as was quickly
apparent when on early morning rounds an elderly patient was
found to have aspirated. One of our two portable suction
devices had been dispatched in response to a Medevac call the
night before. The second was retrieved — eventually — from
the Storage Room.

I’d drink to that!
We in St. Anthony were recently briefly reminded of what is a
daily reality for too many in our global village. We smug,
privileged residents of the west forget how fragile is the façade
of civilization. It’s long past time we leveled the playing field.
All these scourges if they are to be ameliorated require
education of the public and of health care professionals to be
sure but also require a modest, functioning infrastructure
including public utilities and hospitals.
According to the Canadian Network for International Surgery
13% of Africans die as a consequence of trauma. Millions of
productive life years are lost as a result of trauma related death
and disability. In Africa one woman in 13 dies of complications
of pregnancy and childbirth.
In much of the developing world carrying water for washing and
cooking is a daily, life long, back breaking chore borne mostly by
women. Water is commonly contaminated such that diarrheal
disease is a major killer especially of infants and children.
We are fortunate that such events are for us an irksome
inconvenience. Already plans are afoot to provide the hospital
with a secure alternate water supply. The damage to the facility
as a result of the power surge will probably run to many
thousands of dollars — perhaps a million but will be covered
largely by insurance. The lawyers will get rich deciding just
whose insurance.

back on one’s unsubstantiated clinical acumen - which, no
matter how substantial, leaves one decidedly insecure.

Une réalité quotidienne pour trop de personnes dans notre village global
N
ous avons récemment été victimes de deux interruptions «
non prévues » dans notre petit hôpital, lesquelles ont
sérieusement perturbé le courant normal des activités. En fait, il
n’y avait plus aucun courant, en raison d’un coup de froid
précoce qui, survenu avant l’arrivée de la neige, a gelé la
conduite d’admission d’eau au réservoir derrière la ville. Avec
aucun système supplémentaire en état de fonctionner, la
situation s’est rapidement détériorée pour en dire le moins! Qui
plus est, comme le moteur qui met en marche le système central
de mise sous vide – lequel alimente les prises murales de vide–
est refroidi à l’eau, soudainement, nous n’avions plus de
succion, ce qui est devenu apparent lors des visites matinales en
découvrant un patient âgé qui avait succombé à l’aspiration.
L’un de nos deux appareils d’aspiration portatifs était sorti en
réponse à un appel Medevac le soir précédent. Le deuxième a
éventuellement été récupéré du local d’entreposage.
Contrairement à certains hôpitaux dans les pays moins favorisés,
nous ne sommes pas habitués à de telles situations et par
conséquent, nous sommes moins vigilants. Nous ne disposions
d’aucune source d’eau accessible, ni des contenants et des
sceaux nécessaires à se laver les mains, à prendre un bain et à
vider les toilettes. Le personnel s’est rapidement fatigué de
transporter de l’eau pendant cette urgence qui, en bout de ligne,
a perduré moins de 24 heures. L’ordre de bouillir l’eau qui s’est
ensuivi est toujours en vigueur. Heureusement, il n’y a eu aucun
incident d’agressivité parmi les personnes enlignées dans les
magasins en quête d’eau embouteillée, comme ce fut le cas lors
des inondations récentes en Colombie-Britannique. À ma
connaissance, personne n’a succombé à une maladie suite à la
consommation d’eau contaminée.
La deuxième interruption est survenue par un dimanche soir
tranquille, alors qu’on procédait à l’entretien systématique et
prévu du transformateur qui alimente l’hôpital. Cette activité
s’est transformée en chaos lorsqu’une saute de puissance a
causé un incendie électrique au sous-sol. Seule l’action rapide
des gens qui se trouvaient sur les lieux a évité un désastre
majeur. Tout de même, les lumières et l’équipement électrique,
notamment les moniteurs et les ordinateurs, d’un bout à l’autre
de l’hôpital ont été grillés, et une évacuation partielle s’est
avérée nécessaire en raison de la fumée âcre.

Nous avons la chance que de tels événements ne sont pour
nous que de fâcheux inconvénients. Des plans se dessinent déjà
afin d’assurer que l’hôpital dispose d’un approvisionnement
d’eau de secours. Les dommages entraînés par la saute de
puissance se chiffreront vraisemblablement dans les milliers de
dollars, possiblement un million, bien que la majorité de cette
somme soit couverte par l’assurance. Les avocats s’enrichiront
en décidant quelle assurance devra assumer ces coûts.
Dans une bonne partie du monde en développement, le fait de
transporter de l’eau pour faire le lavage et la cuisson s’avère une
corvée quotidiennement dont les femmes, le plus souvent,
s’acquittent toute leur vie durant. Souvent, l’eau est contaminée,

Selon le Canadian Network for International Surgery, 13 % des
Africains meurent par suite de traumatisme. Des millions de vies
productives sont perdues en raison de décès et d’invalidités
causés par les traumatismes. En Afrique, une femme sur 13 meurt
de complications liées à la grossesse et à l’accouchement.
Le soulagement de ces fléaux exige non seulement l’éducation
du public et des professionnels de la santé, mais aussi une
modeste infrastructure opérationnelle, y compris des services
publics et des hôpitaux.
Ici à St. Anthony, nous avons eu un bref rappel de ce qui
constitue une réalité quotidienne pour trop de personnes dans
notre village global. En qualité de résidents privilégiés de
l’Ouest, nous oublions à quel point la façade de la civilisation
est fragile. Le temps est venu d’assurer à tous et chacun l’égalité
des chances.

The re-build of the site has focused on a
complete IT redevelopment, allowing for the
EBRS portion to be the success it has, and to
maintain an entirely new database dedicated
solely to CAGS Member services. Also of
key importance in the construction of the site
was to open it up to others in the national
and international surgical population by
posting their meetings and site links. Ideally
the CAGS site will foster knowledge transfer
by sharing clinical guidelines and policy
papers from stakeholders in the surgical
community and provide an opportunity to
ask questions and share ideas.
The site has been developed largely by a web
design company called Mediaforce, located
in Ottawa, ON with the help of Louise
Tremblay, CAGS Executive Director, and Dr.
Jim Watters, Secretary to CAGS Executive
Committee and Board. “We are quite pleased
with the site. After a lot of hard work, we feel
that the clinical resources the site can offer all
surgeons across Canada and potentially the
world will be a great asset”, said Dr. Watters.

all CAGS Members will benefit from what the

The Canadian Surgery Research Fund was founded in the early 1980s
to promote research performed by general surgeons and general surgery
residents in both clinical and basic science. The CSRF currently

communauté chirurgicale nationale et
internationale puissent afficher leurs réunions et
des liens vers leurs sites constituait un autre
objectif important de la construction du site.
Idéalement, le site de l’ACCG favorisera le
transfert des connaissances grâce au partage des
lignes directrices cliniques et des énoncés de
politique de nos partenaires de la communauté
chirurgicale, en plus de permettre aux utilisateurs
de poser des questions et de partager leurs idées.

e Fonds canadien de recherche en chirurgie (FCRC) a été créé
au début des années 1980 afin de promouvoir la recherche menée
par des chirurgiens généraux et des résidents en chirurgie générale,
dans les domaines des sciences cliniques et fondamentales. À l’heure
actuelle, le FCRC subventionne deux projets de recherche par année
ainsi que la Séance de réflexion des résidents sur la recherche.
Des demandes sont sollicitées pour les Subventions d’exploitation du
Fonds canadien de recherche en chirurgie. Elles peuvent être
