By submitting this application, I am requesting membership with the Mid-Florida Officials’ Association (MFOA).
I will submit all application documentation and fees by the deadlines for each sport I choose to officiate.
I agree to read, understand and abide by the Constitution and Policies and Procedures of the MFOA. I also agree that I
am an independent contractor not employed by the MFOA.
NOTE: Per policy, no member will be guaranteed any number of games. The MFOA Board is responsible for providing
the best available officials for all games.
ROOKIES DO NOT pay membership dues but must pay booking fees.

as you do so before the deadline. (For example, an official may register and pay for membership and only football, at
