-It is a dangerous place
In past 18 months there have been a
number of roll off deaths/accidents
Three deaths in 2013/2014 that we know of
-All were operator error
There are a few major types of accidents
-They are preventable!!!
BENLEE trailers try to out think the user
to make him/her safe

-Crazy but true

-Ensure hoist up alarms operating

-“Most” roll off trucks that come into
BENLEE service have hoist up alarms
disconnected-literally kills people
Drivers short them out/cut wires
-Most trailers (until now) did not
have alarms
-Install hoist up flasher (“alarm”)
directed at driver’s mirror
Can be purchased at low cost

Raised hoist and went behind box, which
slid down
Cause-Box was not attached to cable
What to do
-Hoist never to be raised if box is
detached
-Never get behind a box, even if cable
attached
Cable could snap!

Other Safety Issues and Solutions!
1) Tires coming off while driving
Cause-Loose nuts
-Solution-Regular torque checks and
install Wheel Checks
Wheel Checks are simple, low cost
and effective
-Identifies loose lug nut. Spins
out of place
Standard on all BENLEE units
Standard at WM & Omni
2)Flat tires shredding causing accidents
as tire disintegrates
-Solution-Regular tire check and
install Auto Inflate system
Auto Inflate-Keeps tires inflated &
activates warning light if leak
-Also saves tires and fuel
THE ONLY OPTION
RECOMMENDED
3)Sheaves freeze up and snaps cable
-Box falls
-Solution-GREASE, GREASE,
GREASE
Ensure replacement sheaves have
grease grooves in bushings
Trailer will last and be SAFE
25+ years if Greased
4)Trailer tilt over
-Operate on uneven ground &/or
Jackknifed
-Solution-Pretty simple-Do not do it!

5)Roll off box door hits driver as opens
-Solution-Stand as clear as possible
Consider using door “tool”
wear hardhat and safety glasses

-Solution-Ensure back up alarms working

mechanical alarm

8)Material flying out of box while driving
-We get calls weekly from around
U.S., due to our name on mud flaps

Never drive with material sticking
out of box
Many States have Tarping Laws
9)Employee falls while climbing to hand tarp load
-Solution-Install Automatic or Semi
Automatic Tarp system
Climbing is dangerous
Can use on roll offs, luggers, open tops &
dumps
“Window Shade” Systems Require
some climbing, but are low cost
10)Hoist falls during maintenance
-Solution-ensure use of safety stands,

-Solution-Inspect daily.

13)Accidents due to drivers with bad driving

driving history is checked

14) Driver gets hurt climbing in and
Out of Tractor (Power Unit)-If Trailer,
Driver must go in and out of unit.
-Solution - Install ‘Dead Man’ button
eliminates second trip and/or Run
away unit
Can be Installed in most Trailers
15) Ensure accurate Pre & Post trip
reports are being done
Solution-Audit yourself-get under truck!
