Uli MANAGER, PROCUREMENT OFFICE DATED ISSUED: December 9, 2011 REQUEST FOR QUOTES NO: FY12.1070.FSM ‐ Dielectric Testing Service INSTRUCTIONS: This REQUEST FOR QUOTES shall require an original, one PDF electronic copy, and three (3) hard copies that must be received at the ASPA Procurement Office no later than 2:00 p.m. on January 9, 2012. The envelope or box must be labeled “RFQ NO. FY12.1070.FSM ‐ Dielectric Testing Service.” Late submittals will not be opened or considered and will be determined to be non‐responsive. All Offerors shall provide sufficient written and verifiable information that responds to the requirements set forth herein and in the attached Scope of Work. 1) Pre‐Quote Questions – Any pre‐quote questions and/or clarifications shall be submitted in writing (through electronic mail or hard copy) to [email protected] no later than 4:00 p.m. American Samoa time on December 19, 2011. The ASPA shall issue addenda to address any questions and/or clarifications as necessary. NOTE TO OFFERORS: This Quote is subject to the General Terms and Conditions and the attached Special Terms and Conditions of the Request for Quotes for the Dielectric Testing Services. The undersigned offers and agrees to furnish within the time specified, the articles and services at the price stated opposite the respective terms listed on the schedule provided, unless otherwise specified by an Offeror. In consideration to the expense of the American Samoa Power Authority in opening, tabulating, and evaluating this and other quotes, and other considerations such as the schedule, the undersigned agrees that this quote shall remain firm and irrevocable within One Hundred Twenty (120) calendar days from the date opening to supply any or all of the items which prices are quoted. FY12.1070.FSM I. SOLICITATION The American Samoa Power Authority (“ASPA”) issues this RFQ in order to solicit quotes for the dielectric testing of bucket trucks for the Transmission and Distribution (“T & D”) Division. II. SCOPE OF WORK The selected Offerer is responsible for, but not limited to the specifications on Attachment B. III. PREPARATION INSTRUCTIONS Quote Price ‐ Offeror must set forth the rate for the dielectric testing of ASPA’s bucket trucks as stated on the attached Quote Form. The ASPA will award this RFQ in whole or in part to selected Offeror(s). IV. CONTRACT TERM One (1) year contract with an option to renew for an additional year. V. BASIS FOR AWARD Quotes for specified items will be evaluated by a Source Evaluation Board (“SEB”). SEB members will be chosen by the Chief Financial Officer based on recommendations from the Procurement Manager. Submission of a quote shall constitute a waiver of any challenge or dispute of the SEB members, as well as the choice of methodology set forth on the SEB score sheets. The awards will be made by the ASPA in accordance with the evaluation criteria set forth herein and with Section 3‐111 of ASPA’s Procurement Rules. The results of the SEB’s evaluation shall be documented and a determination shall be made by the SEB of those responsible Offerors whose Quotes are susceptible of being selected for award for each type of equipment, in descending order. The determination shall be included in the contract file. The results of the SEB evaluation will be documented and the SEB determinations will be sent to the Procurement Manager. The Procurement Manager, upon receiving the Source Evaluation Board’s written recommendations for the best responsive offer shall make a written determination of which quotation is most advantageous to ASPA and forward to the CFO. The CFO will forward the final determination to the Chairman and CEO for final approval. The Procurement Manager shall notify the winning Offeror(s) accordingly. RFQ FY12.1070.FSM Dielectric Testing Services Page 4 of 11 The ASPA reserves the right to make the award to the Offeror who submits the quotation, which meets the requirements, set forth herein and best meets the needs of the ASPA after taking into consideration the aforementioned factors. The ASPA also reserves the right to select portions of a quotation, or to reject any and all Quotes. VI. WITHDRAWAL OF QUOTATION Any quotation may be withdrawn prior to the scheduled time for the opening of Quotes by notifying the ASPA in a written request. No quotation may be withdrawn after the time scheduled for opening of Quotes. VII. OPENING AND COMPARISON OF QUOTES In accordance with Procurement Rule § 3‐110, Quotes will be opened and recorded as part of the record for the Source Evaluation Board on the date and at the time indicated above at the Procurement Office in Tafuna or in another place designated by the ASPA Procurement Manager in writing. VIII. RFQ CONDITIONS The ASPA reserves the right to issue any addendum to this RFQ. Offerors shall send the ASPA a signed form confirming receipt of any addendum, and shall submit supporting/additional information as required by any addenda. In the event that any Offeror fails to acknowledge receipt of any such addendum in the space provided, his or her quotation shall be considered irregular and will be accepted by the ASPA only if it is in the ASPA’s best interest. In the event that addenda are not received until after the Offeror has submitted his or her quotation, a supplementary quotation may be submitted revising the original quotation. Such supplementary Quotes must be received by the ASPA prior to the scheduled time for opening of Quotes. IX. EVALUATION CRITERIA These criteria will be used by the Source Evaluation Board to evaluate Quotes. Electrical Test Total Costs Total: 0‐50 points 0‐50 points 100 points (best possible score) X. SELLER’S INVOICES Invoices shall be prepared and submitted in quadruplicate (one copy shall be marked ʺoriginalʺ) unless otherwise specified. Invoices shall be ʺcertified true and correctʺ and shall contain the following information: Contract or Purchase Order number (if any,) item numbers, description of supplies or services, sizes, quantities, unit prices, shipping and handling insurance, extended total, and any other non product costs associated RFQ FY12.1070.FSM Dielectric Testing Services Page 5 of 11 with this order. Bill of lading number and weight of shipment will be shown for shipments made on Government bills of lading. XI. INDEMNITY PROVISION The Successful Offeror shall indemnify and hold harmless the American Samoa Power Authority from against any and all claims, damages, losses, and expenses, including attorney’s fees, arising out of or resulting from the performance of the work, provided that such claim, damage, loss or expense is attributable to bodily injury, sickness, disease, death or damage to property of whatsoever nature, including the loss of use resulting there from, and whether or not is caused in whole or in part by any negligent act or omission of the successful Offeror, its subcontractors, or anyone employed by any of them or anyone for whose acts any of them may be liable, regardless of whether or not it is caused in part by the negligence of the ASPA and/or the strict liability of the ASPA, its agents and employees. XII. AWARD, CANCELLATION & REJECTION Award shall be made to the lowest responsible and responsive Offeror, whose quotation is determined to be the most advantageous to the American Samoa Power Authority, taking into consideration the evaluation factors set forth in this solicitation. No other factors or criteria shall be used in the evaluation. The right is reserved as the interest of the ASPA may require waiving any minor irregularity in Quotes received. The Procurement Manager shall have the authority to award, cancel, or reject Quotes, in whole or in part for any one or more items if she determines it is in the public interest. Award issued to the lowest responsible Offeror within the specified time for acceptance as indicated in the solicitation, results in a binding contract without further action by either party. In case of an error in the extension of prices, unit price will govern. It is the policy of the ASPA to award contracts to qualified Offerors. The ASPA reserves the right to increase or decrease the quantity of the items for award and make additional awards for the same type items and the vendor agrees to such modifications and additional awards based on the quotation prices for a period of thirty (30) days after the original award. No award shall be made under this solicitation which shall require advance payment. RFQ FY12.1070.FSM Dielectric Testing Services Page 7 of 11 The undersigned, (hereafter called a “Offeror”) ________________(Corporation, Partnership or Individual), hereby proposes and agrees to furnish all of the necessary information required to complete the above quotation in accordance with the Technical Specifications, Scope of Work, General Terms and Conditions, and other procurement requirements specified in these Contract Documents for the prices stated in the itemized quotation form(s) attached hereto, plus any and all sums to be added and/or deducted resulting from all extra and/or omitted work in accordance with the unit and/or lump sum prices stated in the itemized Quotation form attached hereto. The undersigned Offeror has read and understands the quotation requirements, and is familiar with and knowledgeable of the local conditions at the place where the work is to be performed. The Offeror has read the Invitation for Quotation Instructions and General Terms and Conditions attached to the Quotation Invitation to ascertain that all requirements of the quotation are submitted in the quotation envelope that includes an original, one (1) PDF electronic copy, and three (3) hard copies at the date and time of quotation opening. RFQ FY12.1070.FSM Dielectric Testing Services Page 8 of 11 ATTACHMENT B Scope of Work Dielectric Test Regular annual inspection and testing is required by OSHA and ANSI, which shall include but not limited to testing of the aerial and articulating devices namely Bucket trucks as well as Digger Derrick equipment. The scheduled inspection and testing will provide and identify developing problems before they cause injuries or downtime. Inspections will provide identifications of defects and identifying problems needing immediate repair/replacement. All equipment is carefully inspected for any damage or wear. All Tests and Inspection are performed in accordance with the latest version of OSHA rulings and ANSI Standards. Plus any other areas, not listed, required under OSHA standards to be inspected. The units to be inspected and tested are: 1. WP‐27 2008 Isuzu Digger Derrick 2. WP‐28 2000 GMC C‐6500 Bucket Truck 3. WP‐43 2000 GMC C‐6500 Bucket Truck 4. WP‐58 1997 GMC C‐7500 Chipbox Bucket Truck 5. WP‐70 2008 Isuzu Bucket Truck 6. WP‐81 2000 GMC C‐7500 Bucket Truck 1986 International Digger Derrick Truck 7. WP‐148 A.
Dielectric Testing Verifies the electrical insulating strength in the FRP Upper and Lower booms, Bucket, liners, FRP extensions, tools circuits and upper control system a. Highly trained and skill technicians perform all dielectric testing and inspection. b. A dielectric AC or DC test voltage standards should be tested at the rated unit line voltage as specified by ANSI standards. B.
Load Test a. A functional and operational test is performed to load test in accordance with OSHA and ANSI Standards. b. Description of Load and calculated weight per Manufactured Load Chart. c. This test will find worn swing bearings or gearboxes, worn pins or bushings and other pins/gears critical to the operation of the aerial. d. Test will find the stability and strength to the performance on the calculated test load in accordance with manufactured load chart and OSHA/ANSI Specifications. Safety inspection The visual inspection includes removal of inspection cover plates as necessary to do a thorough job as follows: a. Vehicle Body, Tires, parking brake, safety grab handles, entry steps to cab and body, vehicle lighting, all motor guard and shields, required decals, exhaust system, torsion bar and PTO shaft and lockout. b. Outriggers functions to make sure mounting bolts are secured, rotation bearing tolerance in compliance with standards, no cracks on bearing housing, gear box fluids sufficient, check key and key way for wear, PTO functions, PTO lines and fittings for wear, hydraulic lines and hoses for wear, wire harness for wear, lift cylinders for wear and to check wear pads. c. Platform to inspect the alignment of Basket and Bucket, to check all latches, safety attachments and hinges, pivot bearing, mounting hardware for tightness, basket locking pins for wear and to check safety rails and guards d. Controls to make sure they are properly marked (lower and upper control), lower control override system activated, return all operating control to neutral positions, lower control equipped with emergency stop, outrigger control properly marked, and limit switch/switch operational. e. Chassis; Truck frame, aerial sub‐frame, suspension, PTO, pintle hook, and components of the brake, steering, electrical, air, hydraulic, exhaust, and cooling systems. f.
Pedestal; Mounting bolts and welds, pedestal structure, diagonal brace, attachment welds or pins, hydraulic swivel joint, hydraulic components, swing drive gearbox mounting bolts, backlash between swing pinion or bull gear, electric collector ring and brushes and lower control operation. g. Rotation Bearing; Upper and lower bearing attachment weld and bolts, vertical movement of bearing, and proper torque on accessible bearing bolts. h. Turntable; Turntable structure, bucket leveling cables, leveling cylinders, compensating chains or sprockets, and hydraulic system components. i.
Lower Boom; Boom structure, welds, lift cylinders and attachment, hydraulic system, leveling cables or rods, upper or extend cylinder and attachment, push links, boom rest supports, tie‐down straps, lower insulator and mounting, extension roller assembly and wear pads. j.

Upper Boom (Extension); Structure, welds, leveling cables or rods, wear pads, upper insulator and mounting, hydraulic lines and components, jib structure and mounting, tool circuit hoses and fittings, pole claw arms and mounting brackets. l.
Winch; Mounting brackets, bolts, pins; gearbox, hydraulic components, load line, and controls. m. General; Load rating chart, electrical hazard placards, MADDDC placards and upper or lower control operation placards. D. Deficiency Report & Certification a. Defects and problems found during the inspection and test are classified in category 1 to 5 of its critical and handwritten /electronic copy report. b. Inspectors (technician) review the report with ASPA designated representative. A signed and dated copy of the test report listing the description of conditions to be corrected. c. Inspector (technician) Company should retain a copy of the test report on file, Aerial equipment need rework and require verification or repair and retest with ASPA. d. The technician(s) shall submit a full detailed written Safety Inspection report, Deficiency report, Load test report and Dielectric test report for each unit tested. These shall clearly identify results of the inspection and test conducted on the Bucket Trucks. e. A certificate of inspection certify that ASPA Equipments described were tested and examined by the undersigned, all test and examination has been conducted in accordance with the requirements of ANSI and OSHA.
