Scheduling Appointments
Call for appointments as far ahead of time as possible. Inform the patient service specialist that you wish to schedule an
appointment, and of your physician and service of choice. Please have your insurance card information available when
you call, and bring it with you to every appointment as it will be checked each time for verification of health insurance
coverage. Copayment may also be collected before seeing your physician depending upon the requirements of your
health insurance plan.
Watson Clinic has an automated appointment reminder system. This system will call to verify most scheduled
appointments 1-2 days prior to your visit. If you must cancel an appointment, please call your physician’s appointment
scheduling number no later than 24 hours prior to the appointment. This will enable us to reschedule the appointment
time to meet the needs of another patient. Appointment scheduling phone numbers are located toward the back of this
handbook.
Non-urgent appointments may also be scheduled online at www.WatsonClinic.com.
Patient Parking
All Watson Clinic locations have free parking on site for patients, including designated handicapped spaces. Our Main
Clinic offers a courtesy tram that runs Monday through Friday from 8 am to 5 pm. If you have specific transportation
needs, please call the hospitality office at 863-680-7462 or 863-680-7414 and we will do our best to meet your needs.
Valet parking is offered at the Main Clinic entrance, and is free of charge to handicapped patients.

If you are seeing your provider for your first appointment at the Clinic, please arrive at their check-in desk approximately
30 minutes early. We’ll obtain your correct personal information and provide you with a Clinic number. On return visits,
arrive about 15 minutes prior to your appointment time.
Medications List
Each time you visit Watson Clinic, please bring a list of all medications you are currently taking, both prescriptions and
over-the-counter items, plus your current insurance card.
Additional Services
Most Watson Clinic locations offer fully accredited and licensed diagnostic services such as lab and x-ray on site. The
Main Clinic houses numerous additional services so that many of your healthcare needs may be met in one convenient
facility.
Addressing Problems & Concerns
As patients, it’s crucial that we ask questions of our healthcare providers. If you have a health concern, be sure to discuss
the situation with your doctor. If you don’t fully understand your diagnosis or treatment, ask that it be explained again.
The care we provide is of greatest value to you when you fully understand and follow your provider’s instructions.
General Patient Information
Patient Amenities/Hospitality Staff
Vending machines for beverages and snacks are available at most facilities. The Main Clinic has a full cafeteria serving
breakfast and lunch, as well as many a la carte items. Other facility amenities include an on-site pharmacy at our Main
Clinic and free WiFi access at all of our locations. Please note that patients on home oxygen are requested to bring their
own oxygen supply.
If we can help you with directions or calling a cab, please ask for assistance at the main desk of each location. Courtesy
phones are available at each location as well. At the Main Clinic, our hospitality staff will be glad to escort you to your
car or assist you should you have mechanical difficulties.
Prescription Refills
If you need a prescription refill, please contact your physician’s office or go online to www.WatsonClinic.com to request
a non-urgent prescription refill from your NextMD patient portal account. Keep in mind the following:
• Not ALL prescriptions can be refilled by phone request; most pain medications require a prescription printed and
signed by a doctor and cannot be called in.
• Most medications will NOT be refilled if you have not seen the physician within the past 12 months; some
medications may require more frequent physician visits.
• In order to be sure your prescription does not “run out,” please allow at least two business days to get your
prescription renewed.
• Your preferred pharmacy is kept in our medical records system. Please keep your physician’s office records updated
with your pharmacy of choice.
• Online prescription refills are to be used for non-urgent refills only. If you need immediate or more time sensitive
assistance, please contact your physician’s office directly.
• We recommend that you call your pharmacy prior to picking up your prescription to make sure it is ready.
• If we mail the prescription to you, please allow five business days for processing.
Patient Registration
After you schedule your first appointment at Watson Clinic, a pre-registration specialist will attempt to contact you by
phone to complete your clinic registration. This will expedite the check-in process for your new patient visit. Please
bring all of your most recent insurance identification cards and a photo ID with you. Thereafter, you will be asked for
your most recent insurance ID card at each appointment with any Watson Clinic providers. You will also be asked to
update any information about yourself that may have changed.
Patient Concerns
Watson Clinic’s mission is to provide comprehensive medical services of the highest quality, in the most cost effective
manner, while constantly improving excellence in the best interest of patient care. If your patient care experience,
for any reason, fails to meet with your expectations, please let us know by contacting our patient advocate at 863680-7269 or by email at [email protected]. You may also receive a survey following one of your
appointments. We encourage you to take the time to express any complaints or grievances at that time. Your feedback
is important to us and will help us to improve our healthcare delivery process going forward.

MEDICAL INSURANCE
Medical Insurance
Watson Clinic accepts Medicare assignment of benefits for standard and supplemental Medicare. The clinic is also
contracted with most medical insurance plans including many managed HMO’s and PPO’s. Contact your insurance
carrier to verify our participation with your plan or go to our website www.WatsonClinic.com/Insurance to see a list
of contracted and accepted insurance plans. Watson Clinic files all insurance claims on behalf of the patient for both
primary and secondary insurance. If your plan requires a copay, we will collect it at the time of service. We accept cash,
personal checks and credit cards.
• If you have insurance with a plan that is contracted with Watson Clinic, you will be responsible for all applicable
copays, co-insurance annual deductibles and any non-covered services. Please understand that we cannot waive
any patient balances. All copays are due at the time of service.
• In the event you have an insurance plan that Watson Clinic does not contract with, you will be responsible for
payment of all charges not paid by your plan. Most insurance plans refer to this as “out-of-network.”
• In the event you do not have any medical insurance coverage, you will be expected to make payment on the
day of service. Our financial counselors are available to assist you in making alternative payment arrangements
if necessary.
After Hours Immediate Care
For urgent medical problems that occur after hours and cannot wait until the next day, you may contact your
physician’s office which is directed to an answering service after hours. An operator will contact the physician on call.
Anytime you feel there is a life threatening emergency, call 911. Do not delay by calling the doctor’s office first.
Walk-In and After Hours Care
Watson Clinic provides a number of flexible healthcare options that work on your schedule. Many of our specialists
offer extended hours to better serve you. For those who require immediate treatment of minor everyday illnesses and
injuries, Watson Clinic houses convenient walk-in options that require no appointment, feature day, weekend and
evening hours, and provide a comfortable and cost-effective alternative to emergency room visits.
Urgent Care, located at the Main Clinic location, provides same-day medical care seven days a week (excluding major
holidays). You may contact Urgent Care by calling 863-680-7271.
Two additional walk-in options, convenient for residents in South Lakeland and eastern Polk County, are XpressCare at
Lakeland Highlands (open 7 days a week excluding major holidays) and XpressCare at Merchants Walk (open Monday
through Friday, 7:30 am - 4 pm). Please call 863-393-9472 for our Lakeland Highlands office and 863-904-6222 for our
Merchants Walk location.
All of the above locations are open to existing Watson Clinic patients and non-patients alike.
In Winter Haven, Watson Clinic operates the Better Now Health Center, offering fast, effective primary care services
exclusively for Florida Blue members, or those with a Blue Cross and Blue Shield healthcare plan. For more information,
call 863-837-5738. Walk-in care is available.
An option exclusively for existing Watson Clinic patients, Priority Care, has same day scheduling and appointments
available within 24-48 hours of request. Contact Priority Care at 863-680-7190 for an appointment at our Main and
South Lakeland facilities.
All of these locations feature a staff of expert Watson Clinic physicians. They do not treat serious or life threatening
conditions, or provide treatment for assaults, rape, children under two years of age, or for children with asthma attacks
or other serious medical conditions. Children 2-18 years of age must be accompanied by a legal guardian.

Patient Financial Services
Watson Clinic is one of the largest physician medical groups in the State of Florida and, as such, our Patient Financial
Service representatives are well trained in all aspects of patient accounts – from your initial visit to the last billing
statement. We are a Medicare Assigned Facility. This means that we accept what Medicare allows for payment, and
submit 100% of the claims electronically. We also file supplemental insurance for our Medicare patients.
Billing
If you ever have a question or concern about your billing statement or your account at Watson Clinic, please call
our Patient Financial Services staff at 863-680-7206 between 9 am and 4:30 pm or visit our Business Office at
our Main facility at 1600 Lakeland Hills Boulevard between the hours of 8 am – 4:30 pm. You may also email us at
[email protected]. We realize that insurance and billing issues can be confusing at times, and want
you to know that we are here to help.
General Patient Information
Electronic Medical Record
Watson Clinic maintains your medical record electronically. That means your medical information will automatically
follow you wherever you go within Watson Clinic, and your Clinic physicians will have instant and easy access to your
medical history at the time of your appointment with them. You are encouraged to enroll and access your medical
information through the NextMD patient portal.
If at any time you need a portion or all of your medical record forwarded to a physician outside of Watson Clinic, please
call our Medical Release of Information department at 863-904-2652.
Transferring medical records directly to another physician is free. If you require copies of your medical records
for purposes other than medical care, you will be required to pay a nominal fee. Please go online to
www.WatsonClinic.com to download and print the Authorization to Disclose Protected Health Information
form required before records can be transferred.
Please note that it is the patient’s responsibility to provide us with any medical history information related to visits to
physicians outside of Watson Clinic.
Patients With Special Needs
If you or a family member are deaf or hearing impaired and require an interpreter of American Sign Language or a
foreign language, please let us know when you make your appointment so this may be arranged in advance.
Handicapped parking spaces for physically impaired patients are available at all clinic locations. Wheelchairs are located
at the entrance to each facility. At the Main Clinic, valet parking is offered at the Main entrance for a nominal fee, but is
free of charge to handicapped patients.
After Your Visit
If you have not received follow-up information about your visit in the reasonable time frame indicated by your doctor
and you have questions, please call your physician’s office or contact them through the NextMD patient portal.
Likewise, if your physician provides your reports by mail, please call if you have any questions.
NextMD Patient Portal
In our efforts to create a complete healthcare experience for you, we are pleased to offer an internet-based patient
portal that allows for enhanced communication between you and your Watson Clinic doctor’s office, and much more.
The NextMD patient portal is a free, confidential and easy-to-use website that gives you 24-hour access to your medical
records, the ability to submit communications with your doctor’s office without the burden of playing phone tag, and
a forum where you can request appointments, access important health information including medication lists, test
results, prescription refills and educational materials.

Watson Clinic’s Bella Vista Building is a sprawling and elegant 51,000 square foot, three-story medical office located
on the corner of North Florida Avenue and East Bella Vista Street. The facility offers a diverse slate of specialties and
services, including otolaryngology (ear, nose & throat, facial plastic surgery, head & neck surgery), dermatology (the
treatment of skin and its diseases), dermatopathology (with on-site Pathology Lab), Mohs micrographic surgery,
gynecology, obstetrics, plastic & reconstructive surgery, the Bella Vista Spa, which is open to Watson Clinic patients and
non-patients alike (offering day spa services to refresh and rejuvenate), speech-language pathology and our hearing
center (audiological examinations and a full inventory of hearing devices).
Better Now Health Center
Better Now Health Center provides primary care services exclusively for Florida Blue members, or those with a Blue
Cross and Blue Shield plan, age 2 and up. Any member from anywhere in the country can receive exceptional care from
this convenient setting, and benefit from the expertise of a highly qualified staff provided by Watson Clinic LLP. The
team of medical professionals we’ve assembled at this location include board-certified family medicine physicians and
certified nurse practitioners, as well as an assortment of skilled nurses and medical assistants.
Watson Clinic Cancer Center (Center for Cancer Care & Research)
The Watson Clinic Cancer Center (WCCC) houses a collaborative group of oncology specialists working to enhance the
quality and breadth of services for cancer patients, their families and the community. Nurturing the latest breakthroughs
in research and treatment, including a new state-of-the-art TrueBeam™ linear accelerator, WCCC is a member of
the Moffitt Oncology Network. They’re accredited by the American College of Surgeons’ Commission on Cancer
and are Florida’s only facility to receive the 2013 Outstanding Achievement Award. The WCCC team features surgical
oncologists, medical oncologists, and radiation oncologists working in collaboration with pathologists, surgeons,
urologists, radiologists, nurses, social workers, dieticians, and other medical professionals. There are also, highly
trained nurse navigators to help counsel and assist patients through every step of the process – from diagnosis through
treatment to survivorship and follow-up care.
Center for Rehabilitative Medicine
The Center for Rehabilitative Medicine offers comprehensive rehabilitation services to include physical and
occupational therapies. In addition to treating an array of orthopaedic, sports, and neurologic injuries, their expert
staff hold advanced clinical board-certifications in orthopaedics, hand therapy, and wound care. Further sub-specialty
certifications include manual therapy, lymphedema management, athletic training and vestibular rehabilitation.
Their outreach programs include orthopaedic screenings with high school physicals as well as osteoporosis and
lymphedema education within the community. The Center for Rehabilitative Medicine offers appointments and has
extended hours Monday through Friday.
Dermatology at Lakeland Highlands
Watson Clinic’s Dermatology at Lakeland Highlands office brings expert dermatology care closer to home for patients
of South Lakeland, Bartow and surrounding communities. Located in the same plaza as Watson Clinic’s XpressCare
and Lakeland Highlands facilities, this exciting practice offers comprehensive dermatology services, including routine
evaluations, ongoing monitoring and treatment of skin cancers, and the treatment of a variety of common skin
disorders, such as acne, rosacea and dermatitis.

Dermatology at Sun City Center
A full-time, full-service dermatology and dermatological surgery center, our Sun City Center location provides a broad
range of specialized services related to the skin, hair and nails, including all forms of skin cancer and other maladies,
such as psoriasis, eczema and sun damage. Additional services include dermatologic surgery, skin cancer screenings
and cosmetic procedures such as Botox® and other facial fillers.
Dermatology at Zephyrhills
Watson Clinic Dermatology at Zephyrhills marks our first foray into the Pasco County area. This office offers a full line of
dermatology services including treatment and monitoring of skin cancer, acne, eczema, psoriasis, dermatitis, and hair
and nail diseases, as well as dermatologic surgery and a full range of cosmetic procedures.
Kidney Center
Our nephrology team at the Watson Clinic Kidney Center treats a variety of kidney disorders and provides state-of-theart dialysis services when kidney failure occurs. Our team offers expanded evening hours to satisfy the busy schedule
of our patients. In addition to Peritoneal dialysis (CAPD), they also provide Cyclers CCPD machine for night time dialysis
that can be performed at home. Our expanded team includes board-certified nephrologists, a transplant coordinator,
registered dietitian and social worker. The Kidney Center is strictly a non re-use facility.
LabCare Xpress at Merchants Walk
This facility provides a level of convenience for patients who live and work in South Lakeland. Routine blood draw
services are offered for Watson Clinic’s adult patients by appointment or as a walk-in. This convenient location also
offers early morning hours, nearby parking, and is located just off the Polk Parkway on South Florida Avenue in the
Merchants Walk Plaza.
Lakeland Highlands
Families come first at Watson Clinic’s Lakeland Highlands location
under the care of an exceptional team of board-certified specialists.
The obstetrics and gynecology department (which also operates out
of our Bella Vista location) offers a broad range of services including
general exams and 3D and 4D advanced ultrasound imaging. Digital
mammography services are also available at this location. An on-site
laboratory assures our patients faster test results, as well as x-ray
services conveniently located at XpressCare at Lakeland Highlands.
Lakeland Regional Medical Center
LOCATIONS
Watson Clinic offers a number of physicians who are hospital-based, such as our growing number of adult and pediatric
hospitalists, critical care intensivists, anesthesiologists, an inpatient neurologist and pulmonologist, a trauma surgeon
and a team of radiologists, providing round-the-clock coverage throughout the year. All Watson Clinic physicians based
at Lakeland Regional Medical Center work closely with the patient’s primary care doctor to coordinate their care, having
access to their comprehensive electronic medical record. These hospital-based physicians are skilled and experienced
in treating health problems unique to hospitalized patients, such as pneumonia, infections, heart attacks and congestive
heart failure.

Plant City
Watson Clinic Plant City offers quality services, compassionate care and a convenient location for Plant City residents.
The physicians and medical staff encompass the fields of pediatrics and internal medicine, and an on-site laboratory is
available for quicker test results.
South
Conveniently located in the center of Southwest Lakeland and just off the Polk Parkway, Watson Clinic South provides
an impressive number of services including digital mammography, board-certified specialists, ample parking and an onsite laboratory. Specialties include allergy, asthma & immunology (pediatric and adult), dermatology, family medicine,
internal medicine, pediatrics, podiatric surgery (foot surgery), priority care and psychiatry (child, adolescents and adult).
Women’s Center
The Watson Clinic Women’s Center holds multiple accreditations and encompasses two “suites” with a dedicated
professional staff offering mammography, breast surgery, and plastic and reconstruction services. Designed with a
comforting, feminine flair, the Women’s Center was constructed to inspire a sense of well-being for women throughout
Polk County and beyond. Suite A houses a comprehensive mammography department where a team of fellowshiptrained mammographers, radiologists and medical support staff offer patients state-of-the-art care in a relaxed and
compassionate setting. These specialists utilize the latest digital technology, including breast tomosynthesis, also
known as 3D mammography. Suite B offers breast and plastic surgery services, a nurse navigator and a social worker.
XpressCare at Lakeland Highlands
Watson Clinic XpressCare at Lakeland Highlands is a busy walk-in location on the corner of Lakeland Highlands Road
and County Road 540A in South Lakeland. This facility provides a sensible, cost-effective alternative to emergency
room visits and is staffed by board-certified physicians who specialize in treating minor illnesses and injuries such as
earaches, small lacerations, bladder infections, sore throats, minor injuries, upper respiratory infections, and much
more! No appointment is required and patients receive prompt, convenient care for ages 2 and up. The physicians
welcome new patients as well as those with an established Watson Clinic physician who need to be seen quickly. An
on-site lab drawing station and x-ray services are available for timely results. This facility offers extended hours (closed
Thanksgiving, Christmas and New Year’s) – convenient for the entire family and close to your home!
XpressCare at Merchants Walk
Watson Clinic’s popular XpressCare location in the Merchants Walk Plaza offers walk-in care from board-certified
physicians available to treat a variety of everyday ailments, including minor lacerations, sore throats, earaches, sports
injuries and upper respiratory infections. New patients are welcome, as well as patients with an established Watson
Clinic physician who need to be seen quickly. On-site lab service is also available.

Your Rights
When it comes to your health information,
you have certain rights. This section explains
your rights and some of our responsibilities to
help you.
Get an electronic or paper copy of your
medical record.
• You can ask to see or get an electronic or
paper copy of your medical record and
other health information we have about you.
Contact Release of Information at 863-9042652.
• We will provide a copy or a summary of
your health information, usually within
30 days of your request. We may charge a
reasonable, cost-based fee.
Ask us to correct your medical record.
• You can ask us to correct health information
about you that you think is incorrect or
incomplete. Contact Release of Information

• We may say “no” to your request, but we’ll
tell you why in writing within 60 days.
Request confidential communications.
• You can ask us to contact you in a specific
way (for example, home or office phone) or
to send mail to a different address.
• We will say “yes” to all reasonable requests.
Ask us to limit what we use or share.
• You can ask us not to use or share certain
health information for treatment, payment,
or our operations.
• We are not required to agree to your
request, and we may say “no” if it would
affect your care. This notice was published
and becomes effective on September 23,
2013.
• If you pay for a service or health care item
out-of-pocket in full, you can ask us not
to share that information for the purpose
of payment or our operations with your
health insurer. We will say “yes” unless a law
requires us to share that information.
Get a list of those with whom we’ve shared
information.
• You can ask for a list (accounting) of the
times we’ve shared your health information
for six years prior to the date you ask, who
we shared it with, and why.
• We will include all the disclosures except
for those about treatment, payment, and
health care operations, and certain other
disclosures (such as any you asked us to
make). We’ll provide one accounting a year
for free but will charge a reasonable, costbased fee if you ask for another one within
12 months.
Get a copy of this privacy notice.
• You can ask for a paper copy of this notice at
any time, even if you have agreed to receive
the notice electronically. We will provide
you with a paper copy promptly.
Choose someone to act for you.
• If you have given someone medical power
of attorney or if someone is your legal
guardian, that person can exercise your
rights and make choices about your health
information. A copy of this document must
be provided to the Privacy Office.
• We will make sure the person has this
authority and can act for you before we take
any action.
File a complaint if you feel your rights are
violated.
• You can complain if you feel we have
violated your rights by contacting us by
calling the Privacy Message Line 1-800-5699720.
• You can file a complaint with the U.S.
Department of Health and Human Services
Office for Civil Rights by sending a letter

• We will not retaliate against you for filing a
complaint.
Your Choices
For certain health information, you can tell
us your choices about what we share. If you
have a clear preference for how we share your
information in the situations described below,
talk to us. Tell us what you want us to do, and
we will follow your instructions.
In these cases, you have both the right and
choice to tell us to:
• Share information with family, close friends,
or others involved in your care.
• Share information in a disaster relief
situation.
• Contact you for fundraising efforts.
If you are not able to tell us your
preference, for example if you are
unconscious or not present, we may go
ahead and share your information if
we believe it is in your best interest. We
may also share your information when
needed to lessen a serious and imminent
threat to health or safety.
In these cases we never share your
information unless you give us written
permission:
• Marketing purposes
• Sale of your information
• Certain information regarding alcohol or
drug abuse treatment
• Most sharing of psychotherapy notes
In the case of fundraising:
• We may contact you for fundraising efforts,
but you can tell us not to contact you again
by writing the Privacy Officer.
Our Uses & Disclosures
How do we typically use or share your
health information? We typically use or share
your health information in the following ways.
Treat you.
• We can use your health information and
share it with other professionals who are
treating you.
Example: A doctor treating you for an
injury asks another doctor about your
overall health condition.
Run our organization.
• We can use and share your health
information to run our practice, improve
your care, and contact you when necessary.
We can share your information with our
business associates as described here.
Example: We use health information
about you to manage your treatment
and services. We may use it to create deidentified health information to use for
all lawful purposes.
Bill for your services.
• We can use and share your health
information to bill and get payment from
health plans or other entities.
Example: We give information about
you to your health insurance plan so it
will pay for your services
Health Information Exchange.
• Health information exchange allows
doctors, hospitals, and other health care
providers to share health information
about patients electronically. This is done
for several purposes, including, but not
limited to, treatment, quality assurance,
state law reporting requirements, health care
operations, and payment. Watson Clinic
physicians and staff, hospitals, and other
health care providers may share and receive
your health care information electronically
through various health information
exchange connections with other health care
providers.
Example: We may exchange your
information electronically with
providers including, but not limited
to, Lakeland Regional Medical Center
or Lakeland Surgical and Diagnostic
Center, as needed for treatment or health
care operations purposes.
How else can we use or share your health
information? We are allowed or required to
share your information in other ways – usually
in ways that contribute to the public good, such
as public health and research. We have to meet
many conditions in the law before we can share
your information for these purposes. For more

Help with public health and safety issues.
• We can share health information about you
for certain situations such as:
• Preventing disease
• Helping with product recalls
• Reporting adverse reactions to medications
• Reporting suspected abuse, neglect, or
domestic violence
• Preventing or reducing a serious threat to
anyone’s health or safety
Do research.
• We can use or share your information for
health research.
Comply with the law.
• We will share information about you if state
or federal laws require it, including with the
Department of Health and Human Services
if it wants to see that we’re complying with
Federal privacy law.
Respond to organ and tissue donation
requests.
• We can share health information about you
with organ procurement organizations.
Work with a medical examiner or funeral
director.
• We can share health information with
a coroner, medical examiner, or funeral
director when an individual dies.
Address workers’ compensation, law
enforcement, and other government
requests. We can use or share health
information about you:
• For workers’ compensation claims
• For law enforcement purposes or with a law
enforcement official, or certain information
relating to inmates
• With health oversight agencies for activities
authorized by law
• For special government functions such as
military, national security, and presidential
protective services
Respond to lawsuits and legal actions.
• We can share health information about you
in response to a court or administrative
order, or in response to a subpoena.
Business Associates.
• There are some health-related services
provided through contracts with third
parties, called “business associates,” that
may need the information to perform
certain services on our behalf. Examples
include software or technology vendors we
may utilize to provide technical support,
attorneys providing legal services to us,
accountants, consultants and other. When
such a service is contracted, we may share
your protected health information with
such business associates and may allow
our business associates to create, receive,
maintain or transmit your information
on our behalf in order for the business
associate to provide services to us, or for the
proper management and administration of
the business associate. Business associates
must protect any health information they
receive from, or create and maintain on
behalf of, the Provider. In addition, business
associates may re-disclose your health
information to subcontractors in order for
the subcontractors to provide services to
the business associate. The subcontractors
will be subject to the same restrictions
and conditions that apply to the business
associate. Whenever such an arrangement
involves the use or disclosure of your
information, we will have a written contract
that contains terms designed to protect the
privacy of your information.
Our Responsibilities
• We are required by law to maintain the
privacy and security of your protected
health information.
• We will let you know promptly if a breach
occurs that may have compromised the
privacy or security of your information.
• We must follow the duties and privacy
practices described in this notice and give
you a copy of it.
• We will not use or share your information
other than as described here unless you tell
us we can in writing. If you tell us we can,
you may change your mind at any time.
Let us know in writing if you change your
mind.
• If more stringent laws apply, they will be
included in an addendum to this Notice.
• We will comply with State law. We will
obtain your written consent for certain
disclosures if your consent is required
under State law. For example, Florida
requires us to obtain your written consent
to disclose for payment purposes.

Changes to the Terms of This Notice
We can change the terms of this notice, and the
changes will apply to all information we have
about you. The new notice will be available upon
request, in our office, and on our web site.
The effective date of this Notice is May 1, 2014.
Prior to converting to the federal government’s
model form, Watson Clinic LLP implemented
a prior Notice that was compliant with and
effective as of the date of the HITECH Act
compliance date (September 23, 2013).
