STANFORD TRAUMA CENTER PROGRAM AIMS TO PREVENT INJURIES FROM FALLS
By Sue Dremann
I
t’s happened again. Another senior has
arrived at Stanford’s Trauma Center
with a head injury. Blood is everywhere.
The patient caught his foot on the edge
of the carpet and crashed head-first into
the dining-room table. He may be lucky,
requiring only several stitches and bed rest,
but for other patients, such a fall can precipitate the end of independence: a broken
hip, brain injury or a fractured spine.
To prevent such tragedies, Stanford
Trauma Center at Stanford University Medical Center created “Farewell to Falls,” a free
assessment and agility-training program for
older adults.
The growing numbers of injured prompted the course. More than one million older
adults in California fall each year, with
200,000 of those falls resulting in injury;
and 188 people ages 55 and older are hospitalized every day from slips, trips and falls,
according to the program’s Web site.
The Centers for Disease Control and
Prevention estimates older adults who
have fallen or who frequently stumble
are two to three times more likely to fall
within the next year.
Of the 600 emergency-room cases at
Stanford annually, 150 will be trauma
related. Nearly half of those cases will
require hospitalization, according to Ellen
Corman, injury prevention coordinator for
the trauma service.
The kinds of injuries Stanford staff treat
range from lacerations and head injuries to
broken bones, including wrists and hips.
“The most serious are hip fractures, which
can cause the greatest loss of independence.
The number of people with a broken hip
who can’t return home is just huge,” Corman said.
Most falls can be prevented by improved
balance, flexibility and strength training, medication review and simple home
modifications, she said.
Adults who feel dizzy or weak should
see a doctor and have blood pressure and
medications checked. Medications play
a large part in facilitating falls. Many
seniors take as many as eight to 12 medications, some with similar side effects that
can compound, she said.
“Farewell to Falls” accepts patients who
have had a previous fall. Participants must
live in San Mateo or Santa Clara counties
and must live independently or with a
spouse. They must be ambulatory and cannot have severe dementia, Corman added.
Occupational therapist Marla Mather
recently visited Audrey Avis Aasen-Hull and
James Byrne Hull to assess risk factors in
their home. She taught the couple strengthtraining exercises, such as the Sit and Be Fit
program, which helps seniors to heal and gain
strength and flexibility while sitting in a chair
or using the chair as an exercise tool. Such
strength training will help the couple to have
the strength to stay upright or, after a fall, to
get up off the ground.
After a video and two sessions with the
couple, staff will follow up with a phone
call every two to three weeks for a year.
“As you get older, you become a victim
of your own weaknesses, so to speak,” said
90-year-old Avis, who still plays violin in a
string quartet. “I’ve fallen so many times I
can’t keep count — at least 50 times.”
Avis said no cause has been determined
for her falls, but she often finds herself tripping on the edge of a rug or having one slip
out from under her. She has hurt herself, but
so far hasn’t broken anything.
Other information
■ Subscribe to Lifeline or another
service that will bring help if you
can’t get up
■ Keep emergency numbers near each
phone
■ Consider wearing hip protectors if
you have osteoporosis or have had
multiple falls
Avis’ medical history was taken by program staff and recommendations were
made to her physician. Hand rails for the
shower and in and out of entrances were
recommended, along with lowering her
bed, she said.
She and her husband, Byrne, have found
the rails and exercises very helpful, she said.
Many programs are available to help
older adults avoid that first fall. The YMCA
and Avenidas offer classes geared toward
strengthening and balance for the older
adult.
Avenidas hosts 12 fitness classes, and
all have components of strength, flexibility and balance, said Judith Taksa Webb,
manager of education and leisure.
“Farewell to Falls” offers many helpful
tips on the Stanford Hospital Web site at
www.stanfordhospital.com/clinicsmedServices/medicalServices/emergency/fallPrevention. For more information or to
join the program, call (650) 724-9369.

EL CAMINO NAMED
DISTINGUISHED HOSPITAL
FREE LECTURE:
El Camino Hospital has been
named one of the nation’s top
hospitals for the third year in a
row, and has received the 2007
Distinguished Hospital Award
for Clinical Excellence from
HealthGrades, a leading health
care ratings company.
El Camino was the only
Northern California hospital to receive the designation
for three consecutive years.
The hospital has also received
HealthGrades’ Specialty Excellence Awards in stroke care,
pulmonary care and women’s
health services.
HealthGrades awarded the
recognition after completing
an annual study of quality at
the nation’s 4,971 non-federal hospitals. Hospitals that
ranked in the top five percent
— a total of 266 nationwide and 26 in California
— received the award.
The study found that in
addition to outperforming the
others across the procedures
and diagnoses studied, distinguished hospitals had, on average, 28 percent lower mortality
rates and 5 percent lower major
complication rates.

Heart disease is the number one killer of women in the US. It often presents different symptoms
in women than in men, so life-threatening warning signs can be missed. Fortunately, determined
cardiologists like El Camino Hospital’s Dr. Cate Collings think beyond standard diagnostics, listening
closer and looking deeper into potential red flags. If there is a problem, we’re the only South Bay
hospital that offers a truly comprehensive cardiac program from prevention
to treatment to rehab. It’s another example of how we always have your

Starting this month, certain
newly qualified Medicare
patients are able to receive
screenings for abdominal aortic aneurysms as part of their
standard Medicare check-ups.
Abdominal aortic aneurysm, or “AAAs,” is a potentially deadly vascular condition that often goes undiagnosed. About 200,000 people
in the U.S. are diagnosed with
abdominal aortic aneurysm
each year, and about 15,000
of these patients have conditions that require treatment,
according to the Society for
Vascular Surgery. The aneurysms are most frequently
seen in people over 50, and
tend to aff lict men more than
women.
The new coverage provides
for a one-time ultrasound
screening for men age 65 and
over who have smoked more
than 100 cigarettes in their
lives, plus women with a family history of abdominal aortic
aneurysms.
Camino Medical Group is one
of the very few medical facilities
with a full focus on AAA treatments. For more information

Get heart smart
for Valentine’s Day
By Dr. Jane Lombard
Q:
A:
When my chest hurts
during vigorous exercise,
is it a heart attack warning?
Heart muscle pain,
called “angina,” is
likely to be the first warning of
blocked coronary arteries,
the most common cause of
heart attacks.
It’s your heart’s
way of complaining that
Dr. Jane
it isn’t getting
Lombard
enough oxygen because of
diminished blood supply. One
in every four heart attacks produces mild symptoms that the
person doesn’t associate with a
heart problem.
If you feel pressing pain or
heaviness in your chest, seek
medical attention promptly.
There is a high probability that
pain of this type is angina. Even
if the pain subsides, the artery
blockages that caused the symptoms are still present and could
grow progressively worse. The
absence of pain doesn’t mean an
absence of damage.
Don’t ignore this sort of pain.
It may be the only warning you
get of a potential heart attack.
Consult a cardiologist.
Q:
A:
What causes heart
problems in babies?
Congenital heart defect is
the most common type of
major birth defect, responsible
for more infant deaths than any
other birth defect. Structural
defects can occur during the
early development of a baby’s
heart. Defects in the walls or
valves of the heart, and the arteries and veins near the heart, can
disrupt the normal flow of blood
through the heart.
Some heart defects heal over
time, while others will require
treatment. Congenital heart
defect may be present at birth
without obvious symptoms.
Depending on the condition,
the defects may not cause problems for many years — or ever
— and the patient will have a
normal life span enjoying normal physical activity.
Heredity plays a role, and
some prescription and over-thecounter medications and street
drugs used during pregnancy
can increase the risk of heart
defects. Expectant mothers
should receive good prenatal
care and discuss any incidences

Q:
A:
How do stents work?
Arteries can become
narrowed or blocked by
deposits called plaque, which is
made up of fat and cholesterol
that builds up on the inside of
the artery walls.
A stent is surgically inserted
to open the artery. For the vast
majority of patients, this procedure greatly improves blood
flow through the coronary
arteries and the heart and can
even eliminate the need for
coronary artery bypass surgery.
Stents can give relief from chest
pain and improve a patient’s
physical strength.
This procedure treats the condition, but it’s not a cure. Renarrowing may occur, possibly
requiring another procedure.
Patients should diet, exercise,
stop smoking (if you smoke),
and reduce stress to help lower
chances of re-narrowing.
Q:
A:
I’ve read that heart disease is more common in
some races. Why is that?
Rates of coronary heart
disease are roughly four
times higher in people born in the
Indian sub-continent countries
of India, Pakistan, Bangladesh,
Sri Lanka and Nepal (“South
Asians”) than their Caucasian
counterparts. They also tend to
develop heart disease at a younger
age (before 40 in men). Numerous studies are underway to shed
light on this community’s greater
susceptibility, but the known factors are high cholesterol, diabetes
and lack of aerobic exercise.
If you are South Asian and
have high blood pressure, diabetes, an abnormal cholesterol
profile, or a family history of
these conditions, heart disease,
or stroke, then you may be at
risk and should see a cardiologist for assessment.
Others who are at increased
risk of heart disease are smokers,
diabetics, obese and sedentary
people, African American and
Hispanic American/Latina women, and people with high blood
pressure or high cholesterol.
