Statement of Nondiscrimination

The Clinical Laboratories of Hu Hu Kam Memorial
Hospital (HHKMH) and Komatke Health Center (KHC)
provide practitioners and patients with laboratory
services of the highest standards, consistent with
the goals and missions of GRHC and the community.
The laboratory offers a complete spectrum of tests
using up-to-date analytical equipment. Most tests are
completed in-house; an outside laboratory handles those
that are not. Current computer technology is used to
monitor tests and report results. The laboratory has an
extensive internal quality control program, in addition
to participating in proficiency testing programs. The
laboratories are accredited by The Joint Commission
(TJC) and certified under the Clinical Laboratory
Improvement Act (CLIA 1988).

The Family Planning and HIV Program Department
provides innovative outreach medical services directly
to the community using a Mobile Medical Unit (large
RV-type vehicle) that features professional medical staff
and state-of-the-art medical equipment. The goal of the
program is to bring services directly to the community
with an emphasis on the prevention of unintended
pregnancies, sexually transmitted diseases (STDs), and
human immunodeficiency virus (HIV). Services include a
wide variety of birth control options, pregnancy testing,
post-partum care, emergency contraception, and
reproductive health education for men and women. The
Mobile Medical Unit and staff serve the Gila River Indian
Community and the Ak-Chin Indian Community.

The Inpatient Department is a 15-bed Critical Access
Hospital which provides care for admitted patients
and is staffed by physicians and registered nurses. The
Wound and Infusion Clinics, housed within the Inpatient
Department, offers services that provide patients with a
variety of treatments and/or maintenance of wounds and
infusions per physician orders. These outpatient services
allow patients to receive necessary treatments and
return home. These services often eliminate the need for
hospital admission. Nurses evaluate patients during each
visit, and a physician is contacted if further assessment is
needed.

The Optometry Department provides eligible patients
of all ages specific services such as: routine eye

The Podiatry Department was established in 1987 and
has grown to be one of the best-respected programs in
all of Indian Country. The podiatry team consists of four
full-time and one part-time podiatrists, two podiatry
physician assistants, two wound care nurses, one
pedorthist (shoe insole maker and shoe modifications)
seven medical assistants, one front office assistant, and
one administrative assistant.
We provide comprehensive foot care to patients of all
ages. Our clinical services include diabetic and general
foot exams, routine or preventative foot care, nail laser
clinics, orthotics, pediatric screening, pediatric and
diabetic shoe programs, casting, therapeutic injections,
nail procedures, management of infections, broken
bones, treatment of musculoskeletal issues, and minor
surgical procedures. Our wound clinic provides superior
care via the latest advancements in technology and
supplies. It is recognized as one of the top programs

Women’s Health offers a full range of outpatient
obstetrics and gynecology services. We provide
pregnancy testing, comprehensive pregnancy care for
both normal and high-risk pregnancies, and a walk-in

The Gila River Indian Irrigation and Drainage District
(GRIIDD) is responsible for operating and maintaining
the irrigation delivery system that will deliver water for
agriculture to lands in Districts 1 through 7. The GRIIDD
also works with Community members to develop land
and on-farm irrigation systems for use of the water
provided as part of the Arizona Water Settlements Act.

and honor guard services for patriotic and veterans
events within the Gila River Indian Community, and at
events at other locations. Veterans can join who served
at least one day of active military duty during one of the
eligibility dates and honorably discharged or are still

The Ira H. Hayes Memorial Public Library has been in
operation since 1985 and is housed in Sacaton’s first
school house which was built in 1929. The tribal council
named the facility in honor of Ira Hamilton Hayes
(1923-1955) the Marine Corporal who helped raise the
American flag on Iwo Jima. Ira H. Hayes was a national
icon, and a revered hero to the local community for his
service in World War II. The library provides a variety
of book selections and educational programming and

Crime Victim Services (CVS) provides services to victims
of violent crime. Assistance is given to victims in a
variety of areas such as: finding emergency shelter, court
preparation, accessing local resources needed for safety,

CVS is a partner and team member to the Gila River Fire

“active efforts” are being made. ICWA case managers
work closely with the Office of General Council and

listing in the United States. Every
month the first 2 calls are free, after that 45¢ per call.

and general information. This toll free call only
applies within the GRTI telephone exchange.

Deaf, hard of hearing, or speech
impaired telephone users can call to
be connected to TTY services. The operator then
relays the conversation for the two callers.
AZ Blue Stake should be used for
locating underground facilities such
as telephone wires, gas lines, pipelines. You MUST
call in before you dig. Utilities will be located and
marked for you at no cost by the utility companies
within 48 hours. This is the National Call Before You
Dig number.

Emergencies happen. 911 works from
any landline, cell phone or pay phone.
If you are not sure your situation is an emergency,
call 911 and the dispatcher will give you assistance.

Billing Disputes
GRTI retains the right to disconnect service for nonpayment of telephone charges. A customer may
dispute a bill, or any charge on a bill, by calling, writing
or visiting a Customer Service & Sales Representative
at GRTI offices.
The Customer Service & Sales
Representative will investigate the question or dispute
and report the findings to the customer. GRTI will
not disconnect service for non-payment of a disputed
charge while GRTI investigates, provided that all other
undisputed charges are paid when due.
If the customer is dissatisfied with the results of the
investigation, he or she may request a meeting with the
Disconnect Day

Customer Service & Sales Supervisor. If still dissatisfied,
the dispute may be taken to the General Manager, who
will have the final authority to resolve the dispute. Ay
any time in the process the customer will have the
opportunity to present any facts or documents relating
to the disputed bill.
Only after this process is completed, and if the matter
is resolved in GRTI’s favor, will service be disconnected.
The Gila River Indian Community Council has the
authority to hear and resolve billing disputes after the
procedure described above has been completed.
