Dear Readers,
In preparing this edition of the Born
To Age Alameda County Senior
Care Directory, I realized that most
people will only need the services
and resources listed here, once in
their life. This directory provides all
the information that someone might
want to know about, if they are
caring for, or becoming themselves,
frail, chronically ill or even dying.

Despite all those nice looking,
smiling people on the cover, this is not the magazine
anyone might want sitting out on their coffee table. In
fact, most of us would probably want to avoid the situation
of age or fraility that would bring us into contact with the
businesses, organizations and services featured in these
pages.
That’s what makes this business difficult at times, no one
wants us around, yet when the time comes, they’re very
glad we’re there. My parents, for example, while proud
of a daughter who publishes a senior care directory,
won’t open the pages to see what’s inside because, they
tell me, “we’re not even seniors, yet.” Of course, if I’m a
senior this year…it stands to reason that they’re getting
mighty close.
It seems as if planning for a time when one might need
help is still a tremendous stigma in our society.
On the logical side, this stigma doesn’t make a lot of
sense. I remember when I got pregnant with my first
child. I read almost every book I could on what to expect
in pregnancy, how to avoid problems, stages of embryonic
growth, diet, etc. This was a stage in life where I definitely
wanted more information so that I could be in control of
my choices. I wanted to know what to eat, what swollen
ankles meant and how much weight gain was normal I
also was invested in being the best I could be in this time
of my life.
Why can’t we look at aging the same way? Well, for
one thing, when I look in the mirror, I don’t recognize the
woman looking back. Inside, I still think I’m a 35 year
old…and, when I talk to my parents, they believe that
they will live another 40-50 years, even though the record
setting miracle of that age has yet to be achieved by
anyone.
That’s what we call denial. The inevitable march to the
end is to be avoided at all costs. Or, so we think…and,
that’s how we build our make believe lives.
But, reality is often a different story. And, somehow,
somewhere life is better when we are prepared to deal
with whatever circumstances we are living.
This year, I lost a 90-year-old friend who absolutely
hated getting old. She suffered through pain, heart
problems, using a walker, hiring caregivers, losing her
eyesight, losing her ability to hear and mostly, seeing
her independent spirit become more and more stifled by
all the things she could no longer do. She once was a
voracious reader, a volunteer for numerous causes and
talked frequently of the many loves of her life. As each
part of her past life slipped from her grasp, she became
more irritable, impatient and depressed. She’d lash out in
anger at herself for failing short of how she’d expected to
grow old.
She simply wasn’t prepared for all the losses, nor had she
considered what assistance she might need to improve
her life over this period of time. And, when it came to
aging, she’d never even read a book about it.
When I recognized that she was in the process of
dying, she refused to admit that was true. She was still
angry at the process of aging, dying was more than
she could handle. We waited three weeks before she
finally settled on letting Hospice in to help care for her
basic needs. And, guess what…she LOVED all the staff
from Hospice…the visits from nurses, social workers
and caregivers. In the last few weeks of her life, she
received more personal attention than the prior four years,
combined. She could’ve had this attention and love for
six months…instead, she barely got a taste before she
passed.
In pushing away help, and living in denial, my friend’s
quality of life actually suffered. I look back and think of
how less lonely she might’ve been in her last year of life is
she actually could’ve said, “yes” to bringing in caregivers
and other services. She might have been happier,
formed closer relationships and yes, even less pain, if only
she could have seen that asking for help wasn’t a sign of
weakness, but one of strength.
So, I urge all of us to be prepared. To realistically
prepare for what might be ahead. And realize that
our dependence on others gets us through so many
situations, why make aging the exception?

Falls Are Preventable
There is a Fall Prevention Program
in Alameda County. They give
talks, make assessments and hold
workshops for professionals and the
public on how to prevent falls. Call

My mother lives in Michigan. By
herself. In a very big house. She is
77 years old and believes herself to
be very capable and independent.
Last year she fell. It was in the
middle of the night, she was on her
way to the bathroom. It was pitch
black in the bedroom and she tripped
over something. She isn’t sure what.
All she remembers was the blood
pouring from her forehead and
crawling on the carpet to find the
phone so she could call the neighbor
for help.
How To Prevent Falls
The trip to the emergency room
resulted in 26 stitches.
It was her third fall in as many years.
Mother doesn’t understand why she’s
falling. Adamant that these mishaps
are rare and unpredictable, she
refuses to get a medical alert button
(to press for help), pick up her throw
rugs or clean up any of the boxes
that clutter her many rooms and
hallways. She does admit to gripping
the banister on the stairs more tightly
ever since the carpet has gotten,
somehow, “slippier.”
Mostly, people need to learn practical
strategies to manage falls. This
includes exercising to improve
balance, flexibility and strength;
recognizing fall hazards at home, and
learning more about what contributes
to the risks of falling.
What mother doesn’t believe is that
many falls are preventable. She still
can’t explain her most recent fall, last
March, in a parking lot at the local
community college. If there wasn’t
a security guard on duty, she may
have lain there all night, screaming for
help. Though uninjured this time, she
simply couldn’t get up without help
and her fear and anxiety just added to
her confusion.

This has been a tough year for our
family. Two of our members are in
the process of dying. This translates
to: No one would be surprised if
they died within a year.
My aunt, at age 53, has stage 4
colon cancer. This means that the
cancer has spread throughout her
body and cannot be cured. She can
undergo clinical trial chemotherapy
and radiation, but the hope is that
she can live a bit longer with these
treatments, than if she had nothing
done at all. However, no one anticipated that she would suffer through
uncontrollable pain throughout this
last year. Pain so intense, she only
leaves her home to go to the next
chemotherapy session. She has
lost over 50 pounds, spends most of
her days in bed, and there’s some
concern that she’ll be asked to discontinue the clinical trial treatments
if she continues to lose weight. Her
husband vows that he will keep her
alive, “no matter what,” because he
believes that if he can keep her alive
long enough, eventually a cure will
be found.
At age 73, my uncle has been in
very poor health for the past five
years. His kidneys failed a couple of
months ago, and now he’s on kidney
dialysis. He was also diagnosed
with congestive heart failure, and
recently had emergency surgery
and seven stents put in his heart.
All seven have failed. He now tires
easily, walks with assistance and his
wife is always at his side. He does
participate in family activities and
hopes to live as long as possible
because, even in his current condition, he believes there is much life
left to enjoy.
While both have a very poor prognosis for the future, only my uncle
has completed the Advance Directive and the Physician Orders For
Life Sustaining Treatment (POLST)
forms that give his wife and children
very clear directions on what to do
with medical care/treatment if his
heart stops beating, if he endures a
massive infection or stroke, or if the
quality of his life deteriorates to the
point where futility is recognized by
all. His Advance Directive appoints
an agent to speak on his behalf, if
he’s ever unable to, and the POLST
gives clear directions on what to do
in a crisis, or when the ambulance
arrives and a decision must be made
immediately.
Filling out the forms, he told me, was
the easy part. Having the conversation with his wife, doctor and adult
children…well, that was difficult.
“You know, in looking at all that might
happen and talking about it,well…
it really makes you face the fact
that there are lots of decisions to be
made in just a split second,” he told
me at a recent family reunion. “I always thought that I’d want to be kept
alive, no matter what….but, when my
doctor explained to me the consequences of CPR if my heart stopped
at home, well, I just had to say that
when my heart stopped beating, to
let me die a natural death.”
But, even as we talked, his youngest
daughter, overhearing the conversation, interjected that she wanted
her father to be kept alive, that they
should do everything they could
to keep him alive. She wanted to
argue each point, but most of all, we
recognized that she didn’t want her
dad to die at all.
His youngest daughter is not his
appointed agent, nor will her opinion
override his, in any given situation.
My uncle, knowing that there were
differences of opinion in his immediate family, made clear that only his
wishes were to be followed in terms
of his own life.
Now, my aunt with cancer, she’s
talked to me some about not wanting
to be put on a machine. But it was
only a conversation we had during a
car ride. Nothing has been written.
She has no Advance Directive, nor
has she considered a POLST. Her
point of view of how she wants to live
out her last days is radically different than what her spouse wants her
to do…and, she is doing nothing
to insure that her wishes are to be
followed.
Her husband, my other uncle,
refuses to accept that she might die
from this stage 4 cancer, and even
forbids us from mentioning the “d”
word in her presence. They only
speak of treatments, procedures and
hope. There is no acceptance of any
reality, and it looks as if my aunt’s
wishes probably won’t be considered
at the end of life struggle between
hope, reality and a miracle cure.

But, the truth is, if my aunt truly
wishes to stay alive “no matter what,”
she can also state that in her Advance Directive and on her POLST
forms. Both of these forms only offer
a place to make your own wishes
known before a medical crisis, so
that doctors know what to do in any
given situation. And, the benefit of
having the conversation between
themselves and their doctors, well, it
can open up much more in the arena
of understanding and treatment in
the little time she may have left. As
her gut seizes in pain several times
an hour, she doesn’t dare ask for
better pain relief because it might
eliminate her from the clinical trial
treatment. So, she suffers. Every
day, she suffers. And, we, as family,
stand by feeling helpless because
we don’t know what to do.
Clearly defining how I want to live is
important to me. I have completed
an Advance Directive. I have had
conversations with my children and
loved ones. I have made my wishes
known. And, I urge all of you to do
the same. It doesn’t matter what you
might want, it only matters that you
write it down, have the conversation
and make sure that everyone involved in your life/care, have a clear
picture of your personal preferences.

The City of Alameda sponsors several
rehabilitation programs providing loans,

perform minor emergency home repairs.
The city has a new grant program
exclusively for accessibility modifications
that can assist by adding ramps and
lifts, as well as making limited interior
accessibility adaption (e.g., installation

The substantial rehab program
creates new affordable rental units

structures. Income limits do not apply
