District Process for Referrals
1. When a student is in need of a BCIU evaluation or service, the district fills in the referral form found at
the BCIU web page. This can be done by entering Main student Referral in the find bar or going to the
Special Education Page, selecting the drop down under “About Special Education” and clicking on
special education forms.
Be sure to include any necessary forms to the e-mail that are required. Signed PTE/PTRE are
only required with the service lines that are yellow.
2. Referrals will be distributed to appropriate BCIU personnel by the referrals system. District will receive
notification regarding the person who was assigned.
3. District will receive the initial receipt and a final receipt when the case is closed. If the district does not
feel they are obtaining information in a timely manner, please call the BCIU and ask for Anne
Lesmeister.
When a student is already in an IU operated classroom,
1. The district does not need to send a referral for an additional evaluation or service. The BCIU
supervisor and team should be discussing with the district supervisor at either an IEP meeting or via email or phone any additional services or evaluations that may be needed. Examples may include the
following:
a. OT/PT/SLP screening—case manager will discuss with the related service provider. Once the
screening is completed, the related service provider should contact the district to let them know if
an evaluation is warranted. The related service person lets the teacher know who contacts the
psychologist to initiate a PTRE. Program assistant will let the psychologist know when the
PTRE has been returned and the psychologist will notify the entire team that the time clock has
begun.
b. Hearing/vision—BCIU case manager will initiate a referral to [email protected] to initiate
the process
c. SETT—BCIU case manager will initiate a referral to [email protected] to initiate the
process
d. The only exception is for Transition Services. Each District must fill in the referral and send to
[email protected] to initiate the process. The BCIU case manager will fill in the needed
forms and send to the District Supervisor to be included with the referral.
