School of Medicine, has implemented two
innovative approaches to the chronic care
model called Guided Chronic Care and
Passport to Wellness.
•The project addresses social barriers
that impede low income, ethnically
diverse, largely uninsured, patient
populations and their ability to fully
access the medical care offered.
•193 patients have been enrolled in the programs and evidence shows
that the impact of chronic disease on their lives is relatively high, prior to
intervention their quality of life is relatively low and that medication
management (patients getting and taking the right medication at the right
time) may be one of the key components for success.
