Message from the President
25 Years: How Far We Have Come
Twenty-five years ago Community Connections was launched on one overriding
core belief: that all individuals with disabilities have full access and participation in
their communities, while making informed choices about their lives and personal
supports.
In the early 1980’s, Massachusetts housed thousands of children and adults with
disabilities in seven state institutions. These people slept 36 to a dorm room,
ate in overcrowded cafeterias, and rarely left the grounds. During the period
of de-institutionalization, Community Connections visited these state facilities
and helped many individuals transition back to their home town by providing
effective community-based supports.
Today, people born with physical, medical, and intellectual disabilities are leading
active, empowered, and healthy lives. Advances in technology have allowed those
without a voice to speak and the wheelchair dependent to stand. Progressive
medicines now control once debilitating life-long medical conditions. Plus,
greater access to real jobs, accessible housing, and transportation assist people
in achieving economic sufficiency and a life worth celebrating.
Our belief that anything is possible for people of all abilities creates an environment
of innumerable opportunities for personal and professional growth. Our core
values outline the framework that drives this fine organization now and into the
future. From our enthusiastic Board of Directors to our incredibly dedicated
staff; our values are put into action each and every day.
One can only imagine what will be accomplished in the years to come. For me, it
is an honor to be a part of this journey and I look forward to sharing our stories
and progress with you in this report.

Day Centers
Habilitation programs are designed to meet the
varied needs of individuals through traditional
and alternative therapeutic activities with a
commitment to consistent and innovative
programming.
Clients such as Laurie Vieira participate in physical and social therapies
that help achieve goals for communication and socialization.

Life Skills
Our Life Skills program is designed to increase
confidence, healthy lifestyles, community
participation, independence, and self sufficiency.
Individual physical, medical, cognitive, and social
goals are developed for each individual.
This Life Skills “Healthy Choices” group prepares nutritious meals
and exercises together - working to achieve their goals to lose weight,
lower blood pressure, and become physically fit.

Case Managers work closely with
individuals to design personalized
in-home supports that are essential
to maintaining independence
and community participation.
Charlie Kennedy is successfully employed in his community, lives
independently, and has an active network of friends and neighbors.

employers through skills assessment, job
coaching, and support.
With 60 years of combined experience, Laurel Sorenson, Maria
Miranda and Wally-Ann Pierce have helped hundreds of clients join
the community in ways they never thought possible.

This unique service is built upon
our deep respect for elders with
disabilities, their life experiences,
and individual needs. Social
activities, transportation
to medical appointments,
and therapies help our
seniors to remain active
and to age with dignity. Richard Duquette is known as one of the “mayors” of our Elders
Program. He is very active, social, and involved in his program activities.

adults with skills assessment,
job placement, and
daytime services.
Since starting with Community Connections, Philip Pereira has
graduated from high school, secured and maintained employment,
and is scheduled to take college classes in the fall.

Transportation
Safe, reliable, and accessible transportation is
provided by caring and committed drivers who
take their passengers to Day Centers, community
activities, and medical appointments.
Mike Nasif and Richard Morey are our transportation “go to guys” !

Community Connections is lucky to have so many
long-term, dedicated staff, board members, clients,
and volunteers. Here is just a snapshot of
the many people who have been with
us for more than 20 years.
25 years
Looking Back
In 1985, Community Connections was founded by Donna Sabecky
and community members. With help from countless others
along the way, they built an organization to assist people with
disabilities to live independently and to be actively engaged in
their communities. At the close of their first year, 45 people with
developmental disabilities received services.
A quarter century later,
we now serve nearly 600
people each day and have
helped support over 2000
people. This growth has been
accomplished with a dedicated
staff and an invested group of
volunteers and supporters. As
our agency continues to grow
and expand, our commitment
to innovative services and
programs grows as well.

Looking Ahead
Advances in technology,
nutrition, and medicine fill
us with optimism for what
lies ahead! The frequency
of corrective surgeries and
medications keep us healthy
and active. We learn more
every day about the diagnosis
and treatment for autism
and the prevention and
management of life-long
genetic disorders.
We begin our next 25 years with some impressive goals: from
implementing an online case management system to developing
emerging leaders. Community Connections has a bright future as a
leader in disability services in Massachusetts. In the coming years we
hope to see additional advances in care for all, including innovative
therapies that will enhance the lives of those we serve, abundant
competitive employment for those seeking meaningful work, and a
state-of-the-art high need center for people with disabilities.

Fundraising &
Event Highlights
We are fortunate to have numerous individuals and businesses
that provide a wealth of support for our fundraising efforts. This is
evident each October when we hold our annual Gala by the Sea.
The dinner and auction event continually draws support from our
generous donors - even in this difficult financial climate.
Another exciting event is our annual Walkathon and Family
Fun Day. This community event involves our staff, clients, their
families, and friends. Special thanks to our supporters who
collect pledges, sponsorships, and donations. From car washes
to Spaghetti Suppers - our programs are actively fundraising
throughout each year to support client needs. Their enthusiasm
is a continual source of pride.
Our mission to provide the highest quality services and
opportunities for people with disabilities shines thru at our
agency-wide cookout. Held for the past two years, this event
draws together all program participants and staff in our nine
locations for a day of fun, food, and relaxation. Held at Upper
Cape Regional Technical School with the grilling expertise of the
Hall Oil X Grill, it is truly an agency-wide event with our staff,
volunteers, and board members contributing their services and
hamburger flipping skills for the day.
Much gratitude is extended to all our donors and volunteers!
Special thanks to our Development Commiittee who devote their
time, energy and skills to supporting Community Connections.
