The Lake Natoma Inn, located in one of Northern California’s most charming and historic towns, is
nestled between Old Town Folsom and the American River. The relaxed atmosphere emphasizes
friendly and hospitable service and is a welcome alternative to the hustle and bustle of the big city, yet
within easy reach of Sacramento.

A room rate of $95 is secured when asking for the Directors of Public Health Nursing room block and
when your reservations are made before September 13, 2016. Rooms include parking as well as complimentary Wi-Fi , refrigerators, and microwaves. Although there is a block of rooms reserved, once
they are filled, availability can not be guaranteed.

Transportation
The Folsom Tourism Bureau and The Lake Natoma Inn have an agreement with Super Shuttle to
provide transportation from the Sacramento Intl. Airport at no charge to conference attendees.
You must schedule your pickup to bill directly to this account.
Go to www.supershuttle.com and book your reservation from the Sacramento Intl. Airport to the Lake
Natoma Inn. At the billing/payment screen, select “direct bill” to a corporate account and use account
code 092FOL33895.
If you do not schedule your reservation, the cost through Super Shuttle is $40 one way.
You can also schedule an Uber for $32-42.

Meals
Breakfast and lunch are included with the conference rate for all three days. There are several locations within
walking distance for dinner on your own or with your region.

Please join us Wednesday evening for our Annual Auction! It is always such a fun event and a wonderful opportunity to network with other Directors.
Don’t forget to bring items to donate and check with your rep about a regional basket!
Continuing Education Units
Continuing education units will be offered for September 27.
As a result of this training, participants will be able to:
1. Understand how leadership roles can influence a change in culture;
2. Discuss the implications of transformational leadership on Public Health programs;
3. Discuss how increased quality improvement can have a positive benefit on Public Health Programs;
4. Increase knowledge of the evolving roles of Public Health and Public Health Nursing in independent and
collaborative practice in a changing health care environment
The Directors of Public Health Nursing is an approved by the California Board of Registered Nursing,
Provider Number 13750, for 6.5 contact hours.

Full Conference— Payment postmarked before September 12—$425
Late Registrations– Payment postmarked after September 12—$525
To receive a DPHN jacket, your registration must be received by
September 9 (registration only, not payment).
Cancellation Policy:
All cancellation requests must be received in writing by Monday September 19, 2016.
No refunds will be made after this date. Any cancellations prior to this date will be refunded in full, minus a $50 processing fee.
Please note that in the event of a cancellation, any expenses incurred by the registrant, such as travel, will not be reimbursed.
