Thank you for your ongoing support during the transfer to
Community Choice Credit Union. On October 12, we plan to
complete our conversion and extend you full access to our
member centers, ATM network and financial services.
This guide will help you understand the conversion process
of your existing accounts and provide you with the resources
to take advantage of our innovative financial tools. This merger
expands the Community Choice footprint, which dates back to
1935. As we continue on a journey making financial services
available to more and more Michigan communities, we are
reminded that our roots trace back to serving the best interests
of our members through great service, low rates on loans and
convenient access to products and services.

What happens to my NuPath account?
Your NuPath account will become your Community Choice account on October 12.
What will my new account number be?
We will maintain your existing NuPath account number and add a “12” and zeros to the beginning of the
number to equal ten digits. For example, if your NuPath account number was 1234, it becomes 1200001234.

Direct deposit and/or automatic payments:
All former NuPath direct deposits and automatic payments will be directed to your new account number.
After October 12, please use your new account number and your new routing number when setting up any new direct
deposit or automatic payment information.
Direct deposits or automatic payments to and from your checking account follow the format given below:
• New routing number: 272079021
• New account number: 10312 _ _ _ _ _ _ _ _ (Total of 13 digits – add “103” to the beginning of your new ten digit
account number).
• For savings account deposits/payments, use the same routing number and “100” in front of your new ten digit account
number: 10012 _ _ _ _ _ _ _ _
Debit cards:
All debit card holders will receive a new Visa® debit card and instructions, which will arrive the week of September 28.
• You can continue to use your NuPath debit card through October 12.
• Please begin using your new Community Choice Visa® debit card on October 13.
• Please be sure to update any recurring payments with your new card number.
• Follow the activation instructions provided with your new card. You must call from the phone number we have on file
and activate a PIN prior to use.
Sign-n-Score RewardsNOW program:
The RewardsNOW program will end on October 1 for debit and credit cards.
• Earn points through August 31 and redeem points by October 1.
• Points will not be rolled over and will be forfeited.
• For additional information, refer to the letter received in August.
Visa® credit cards
All Visa credit card holders will receive a new Community Choice Visa credit card, which will arrive in midSeptember. Activate your new card on September 27 by following the instructions on the card sticker. You
will receive a PIN mailer a few days after you receive your new credit card. Please update any merchants
that automatically bill your credit card after you have activated your card on September 27. Effective October
15, you will be able to set up your Visa for automatic payment from your checking or savings account within
e-Banking through the Visa tab.

Important information regarding your account conversion
Line of credit:
After October 12, your line of credit will offer the following features and benefits:
• No annual fee
• No prepayment penalty
• Overdraft protection option
• Funds available through ATMs and e-Banking
• Competitive, variable rate APRs based on your credit history
• Due date will change to the 28th of the month
Checks:
• All checking account holders will receive a complimentary box of Community Choice checks, which will arrive the
week of October 5. You can begin using your new Community Choice checks on October 13.
• When ordering new checks, please use the format below:
New routing number: 272079021
New account number: 10312 _ _ _ _ _ _ _ _ (Total of 13 digits – add “103” to the beginning of your new ten digit
account number).
We will continue to honor and clear any outstanding NuPath checks on your new account until January 31, 2016.
Overdraft privilege:
• Checking accounts in good standing are eligible for overdraft privilege. Please see the enclosed account agreement
and fee schedule for more information.
• To opt-in to overdraft privilege for ATM and point of sale purchases, please use the Services tab within e-Banking,
selecting “Opt-in/opt out of overdraft privilege.” If you do not have access to e-Banking, please find the necessary opt-in
documents within the Online Forms page under the Resources tab at CommunityChoiceCU.com or at your nearest
member center.
Transactions:
The enclosed Truth in Savings (TIS) disclosure identifies the terms of your new accounts with Community Choice,
including minimum balance, APY and dividend payments. Please note, due to federal regulations, some accounts are
limited to six transfers from the account each month. In-person transfers at a member center are not limited. See item
6, Account Limitations, on the TIS for more information.
Address changes/alternate addresses:
Please notify us anytime you have an address change by completing an address change in e-Banking. If you have
an active alternate address (e.g. you live in Florida in the winter), please call us at 877.243.2528 after October 13 and
we’ll set up your alternate address.
Dividends:
Eligible NuPath accounts will receive a final dividend on October 11 and a final statement will be delivered to all
account holders. Beginning October 12, Community Choice dividends will start to accrue for all dividend bearing share
accounts.

Important information regarding your account conversion
What are the account fees?
Included with this guide are your account disclosures and a copy of our current fee schedule, effective October
12. In many cases our GetBigReward$® (page 8) credits cover fees you may incur. Starting October 12, you will
follow the Emerald/Gold/Silver account level.
You will be eligible for our GetBigReward$ program levels and credits beginning November 1. You will see a
GetBigReward$ share added to your account, which is where any earned credits will be deposited. At this time,
your fees will be determined based upon your GetBigReward$ status. View page 8 to learn more about
GetBigReward$.
Loans:
Your loan and any balance or available balance will be transferred to Community Choice during the conversion.
Your original loan contract will be honored. This means no changes to your rate or term, however, this will not
apply to your Line of Credit and you will be provided with additional terms and conditions.
Accessing your account through e-Banking:
As of 5 p.m. on October 11, please do not access NuPath’s online banking system.
Beginning October 13, you can access your account through e-Banking. If you have logged into NuPath’s
online banking within the last six months, please visit CommunityChoiceCU.com and use the following
login credentials within the e-Banking Login area. If you did not use NuPath online banking, please contact
877.243.2528 or visit your closest member center after October 13 to enroll.
Login ID: Your new login ID will be 12 characters long. It will be comprised of cc (as in Community
Choice) followed by your NuPath account number. You may need to fill required characters with zeros
depending upon the length of your account number. For example, if your NuPath account number was 123,
your login ID would be cc0000000123 or if your NuPath account number was 12345678, your login ID
would be cc0012345678. You can change your login ID within e-Banking.
Password: Your new password will be comprised of cccu followed by the last six numbers of the
primary account holder’s Social Security Number (or Taxpayer Identification Number). For example, if your
Social Security Number was 123-45-6789, your password would be cccu456789. For your security, a
password change will be required immediately after your first e-Banking login.
Telebanking:
Beginning October 13, you will have access to Community Choice telebanking by calling 800.903.0053. You
will need your Community Choice account number, and your temporary PIN number is 0000. You will be asked to
verify your identity with the primary account holder’s Social Security Number.

Access your Community Choice accounts from the convenience of your smartphone, tablet or desktop —
24-hours a day, seven days a week. Mobile banking and e-Banking provide you fast, free and secure access
so you don’t have to worry about visiting a member center. In addition, we offer the following complimentary

e-Banking
e-Banking gives you complete control over your online experience thanks to a customizable interface
and a 360 degree view of accounts. Check account balances, view transaction history, pay bills, transfer
funds, pay loans, open share accounts, view pending direct deposits, as well as many other useful
features which you can learn more about on our website. Plus, our multi-factor authentication helps
ensure your account is safe from identity theft.
If you previously used Bill Pay at NuPath, your information will automatically transfer to e-Pay. To learn
more about e-Pay, please visit CommunityChoiceCU.com/nupath.
Mobile banking
As of 5 p.m. on October 11, you will no longer have access to the NuPath mobile app. Beginning on
October 13, you can download the Community Choice mobile app and login using your e-Banking
username and password. The Community Choice mobile app gives you the freedom to deposit checks
with e-Deposit, pay bills with e-Pay, check account balances, view transaction history, transfer funds and
pay loans from anywhere. Once you have established your e-Banking credentials, simply download the
app to your device and login using your e-Banking username and password.
Community Choice Visa® mobile app
The Community Choice Visa mobile app gives you access to your Community
Choice credit card information on the go! Use the Visa app to explore recent and
pending purchases, view your account balance and due date, make payments from
anywhere, and report a card lost or stolen.

It’s time to
Get rewarded for making the most of your membership
Once the merger is complete, you’ll automatically be enrolled in our member rewards program, GetBigReward$.
Based on your level of participation you’ll receive reward credits beginning November 1. There are four levels
determined by the number of active products and services you have: Emerald, Gold, Silver and Copper. Emerald
and Gold levels receive monthly credits that can be used to offset fees or be refunded at the end of the year. Plus,
Emerald members may be eligible to receive up to a 10% loan interest rebate on qualified loans!*

The Credit Union is committed to the philosophy of people
helping people.
T
he Community Choice Foundation has generously supported countless
Michigan non-profits that improve the lives of some of our region’s most
vulnerable families. The Foundation remains committed to organizations
such as Vista Maria, Gleaners’ Food Bank, Cass Community Social Services,
Kids Without Cancer and Life Remodeled.
The Community Choice Foundation is also committed to the future of our state.
By awarding more than $705,000 in college scholarships to students who
pledge to stay in Michigan after graduation, the Community Choice Foundation
encourages our best and brightest to use their time, talents and education to
improve Michigan’s future.
Community Choice Credit Union began when a group of neighbors agreed to pool their financial resources
by placing $5 in a bowl. This money was available for those individuals to borrow in a time of need.
Community Choice Credit Union continues the tradition of strength in numbers through its annual Give Big
Weekend. For one weekend every year, Community Choice Credit Union team members and their families
Give Big™ by volunteering together with a variety of area non-profits. Community Choice team members
have spent thousands of hours volunteering with organizations such as Gleaners’ Food Bank, The
American Red Cross, Vista Maria Shelter, The Detroit Zoo, Forgotten Harvest, The Michigan Miracle League
and area parks. The Foundation also donates toy bears to local fire departments to give to a child when
they’ve experienced a traumatic event.
We invite you to get involved with our Foundation through volunteering with us during Give Big Weekend,
encouraging friends or family to apply for a scholarship, or by contributing a donation to help us maximize
our community outreach initiatives. In addition, we encourage businesses to sponsor Scholars who will play
a pivotal role in the future of our great state.
