This exam room provides the foundation for a basic caregiver-patient workflow, with separate zones for patient interviews and exams, and the option for additional privacy with public/private zones. While this traditional setup allows for immediate patient care, historical patient data and any additional medical data that supports patient care is not immediately available in the room, so caregivers must leave the room to retrieve this information. TRADITIONAL EXAM ROOM WORKFLOW A For better access to the patient, the examination table is positioned at an angle, providing access from three sides. The exam room is organized into four zones, including entry, patient care, family/visitor and organized storage with an integrated sink. A lowered writing surface is adjacent to the storage zone, as is a chair for the initial patient interview. The overall room size is 10 ft. x 10 ft. with the entry door reverse-hinged for patient privacy. While my practice is evolving, evo I need to re remember that healt healthcare really takes place at the point of care with my patients. Traditional Workflow Public/Private Zones Patient care is divided into separate zones for the patient interview 1 and patient exam area 2 . While the traditional exam room supports immediate patient care, many times historic patient data and additional medical data that support patient care are retrieved from outside the exam room. This setup provides a public zone with side chairs to seat family/ visitors, as well as a dressing nook for patients 1 . The patient care zone, also known as the private zone, allows access to equipment and supplies 2 . The integration of electronic medical records (EMR) and computers into the exam room will help, not hinder, caregiver-patient interaction, keeping patients informed and engaged. This layout builds on the traditional exam room with the integration of EMR for real-time access to patient histories and diagnostic data at the point of care. The patient is seated adjacent to the computer, then moved to the Barrier-Free accessible exam table. ® EMR BARRIER-FREE ACCESS WORKFLOW B The computer work zone consists of a work surface with adjacent seating for the patient. The work zone is located just inside the exam room for separation between the family/visitor and the exam zones. Including the computer allows access to past patient medical records and the entry of new data as part of the interaction with the patient. This exam room supports barrier-free access for patients, including wheelchair accessibility with a central turning diameter of 60 inches. The overall room size is 10 ft. x 10 ft., based on the guidelines of the Americans with Disabilities Act (ADA). ® The Barrier-Free exam table provides optimal patient comfort and positioning during exams, while supporting improved working postures for the caregiver as part of the patient interaction. This reduces fatigue in the short term and also the risk of caregiver injury. EMR Transition Barrier-Free Access Incorporating the computer as part of an efficient patient care environment gives the caregiver more direct contact with the patient and enables her to remain in the exam room to retrieve information. Uninterrupted time with the patient is increased while the overall exam time is decreased. The center of the room is kept open to allow a 60-in. wheelchair turning area and the transfer of the patient to the exam table. The distance from the door to the corner of the room (18 in.) is needed for patient egress. Enhanced Caregiver Ergonomics Three-sided access is provided by angling the exam table. A distinct family/visitor zone with an adjacent dressing nook is incorporated without interfering with the care zone. The movement of the physician is minimized, and constant contact with the patient can be maintained. Care providers must rethink the design of exam rooms to improve patient experiences and workflow. DUAL ACCESS FOR EFFECTIVE FLOW WORKFLOW E In collaboration with leading architectural firm NBBJ, Midmark highlights key design concepts for a dual access exam room. Two doors – one for patient entry and one for caregiver entry – enable safe, clear patient navigation away from clinical clutter. The 10 ft. x 12 ft. exam room has a distinct separation of care zone and patient consultation zone. With separate doors and defined areas, patients self-room through a clearly marked way finding system without having to move through any of the care zones. The layout of the care zone is focused on efficiency at the point of care. The consultation zone is easily accessible from the patient entry side, providing seating, a work surface and a large display to review educational information. The care team work area provides line of sight to all exam rooms for better flow management – eliminating waste and maximizing value. The workflow also allows for better integration among providers for ease of collaboration. Specialty equipment is placed on carts that can easily be pulled in and out of rooms as needed, eliminating fixed elements in rooms and enabling integration of new technologies. Lockable sliding doors from the patient and caregiver areas maximize floor space and allow for easy transitions in and out of the room. The caregiver door can be covered by a curtain from the inside for additional privacy. Workflow and patient flow is further enhanced with one stop for patient weight and vitals readings. By including printers in exam rooms, additional time is saved. Consultation Zone Efficient Care Zone The consultation zone is easily accessible from both entrances and includes seating and a mobile work surface to exchange information. A display is incorporated to view patient-related information and educational information. The display is large enough to view patient information during the care exchange. This consultation zone enables the caregiver, patient and family/visitor to review critical information and data for continuous care. An interactive display, which can be viewed jointly with the patient, helps to recall information, review current health status and develop therapies or schedules for medications, callback and/or feedback options. The flexible care zone includes a table-mounted computer work surface that raises and lowers with the table. It supports optimum access and posture for the caregiver. All essential data and information is accessible to the physician within the exam room. Flexible Care Zone Consultation Zone e-Visits The care zone is designed with efficiencies for left- or right-sided access and the ability to access instruments and supplies at the point of care. The consultation zone is designed around a shared interactive display monitor for the caregiver and patient. It allows the caregiver to review medical records and educational materials, as well as set schedules for medication, reinforcing a long-term care relationship. Communication is focused on engaging the patient beyond the actual visit by using online tools through reminders, educational materials and means of feedback to monitor progress.
