The ADRC of the Wolf River Region serves people living in Oconto,
Shawano, and Menominee Counties, as well as the StockbridgeMunsee Community. Services are available to individuals who meet any
of these criteria:

Dementia Diagnostic Clinics in Wisconsin
Obtaining an accurate diagnosis of dementing discorder is often the most important first step made by
patients and families in their effort to secure the education and support needed to cope with Alzheimer’s
disease. Diagnostic clinics provide evaluation and management services to patients and their families. A
typical patient will undergo neuropsychologoical testing, a comprehesive history and physical examination
and, if appropriate, laboratory and x-ray testing during the initial evaluation. The involvement of social worker

At home with Dementia: Assistive Technology
Support is a unique program developed to address
the home safety needs of individuals with dementia
and to assist their caregivers through the use
of assistive technology. Home assessments are
provided free of charge by trained specialists.
Specialists come into the home and learn about the
needs of the individual with dementia in an efford to:
improve or maintain physical and emotional health;
improve safety; improve independence; enhance
the care giving experience by reducing stress; and
lengthen the amount of time the person can safely
remain in their own home.
The Specialist will then provide recommendations
for the individual and caregiver. The program serves
those with dementia in Brown, Kewaunee, and
Oconto county.

the Aging and Disability Resource Center (ADRC)
of Brown County, N.E.W. Curative and Options for
Independent Living Inc. For more information contact

help you see if you might be able
to get health, nutrition and other

How to Use ACCESS in 5 easy steps!
1.Open a web browser and go to www.access.wisconsin.gov
2.Click the “Begin ACCESS” button to continue.
3.After you read the Welcome Page, click on “Next” to begin.
If you would like to learn more about using ACCESS, click the
link at the bottom of the Welcome Page.
4.You will be asked between 6 and 25 questions about the
people in your home, their income and their bills. Your
answers will stay private and secure. You can always click on
“Help” if you want to know more about any question.
5.When you are done with the questions, ACCESS will let you
know if you or other people in your home may be eligible for
state and federal programs listed.
Family care is a public program that offers individuals
a wide variety of long-term care services, making
it possible for seniors and adults with physical and
developmental disabilities to live at home and stay
independent as long as possible.
while family Care provides the added long-term care
support you need. Your team may make suggestions
and assist in coordination, but you and your doctor
will continue to be responsible for managing your
health and medical care decisions.
Family Care members receive support from a Care
Management Team made up of a care manager and
nurse. They perform a comprehensive assessment
of your needs, abilities, preferences and values to
develop a personalized plan to meet your outcomes
and goals.
The Family Care program can help if you:
•Have long-term care needs expected to last
more than 90 days; and
•Have a physical or developmental disability; and
•Are at least 65 years old, or are at least 18
years old with a physical or developmental
disability; and
•Are Medicaid eligible.
Members continue to choose their own physicians
IRIS (Include, Respect, I Self-direct)
IRIS allows for self directed services and puts the
individual in charge of his/her own long-term care.
An individual creates plan outlining long-term care
supports and services with an assigned monthly
budget. Individuals are able to hire their own service
workers directly or may utilize an agency
or community provider. Individuals enrolled in
IRIS work with an IRIS Consultant and the IRIS
Financial Service Agency which pays for the
services participants have authorized.
Persons enrolled in IRIS manage goods and services
to meet their long-term care needs and use IRISfunded, waiver eligible supports and services to
remain in the community and avoid institutional care.
IRIS program participants are supported in selfdirection by IRIS Consultants and Fiscal Employer
Agents.
Other Safety Tips
• Keep emergency numbers in large print near
each phone.
• Wear shoes both inside and outside the house.
Avoid going barefoot or wearing slippers.
• Think about wearing an alarm device that will
bring help in case you fall and cannot get up.
• Walk carefully near pets or small animals.
Their movement can be unpredictable.
• Clean spills immediately to avoid slipping.
• Nightlights are an inexpensive way to light up
dark passages at night.
• Keep exits, stairways and hallways open and
clear of telephone and electrical cords and
books, shoes and other objects.
• Avoid rushing to answer the phone.
• Spread sand or salt on icy walkways.
• Use high chairs and chairs with solid armrests
or a lift so it is easier to get in and out of.

healthcare and Advance planning
Healthcare and advance planning allows you to make choices regarding your medical care and/or finances
in the event that you become incapacitated. This is your opportunity to express your wishes rather than
potentially having a court appoint a guardian to do this for you. Advance Planning may be made by any
competent individual 18 years and older.
The Wisconsin Department of Health Services (DHS) has healthcare and advance planning forms available
online. These forms are designed to be completed without the assistance of an attorney. Given your
particular circumstances or concerns, however, you may want to seek legal advice from an attorney. The
forms available online are:
• Declaration to Physicians (Wisconsin Living Will)
• Power of Attorney for Health Care
• Power of Attorney for Finance and Property
Copies of the forms may be obtained by:
1) These forms, or variations of these forms may also
be available from your attorney, physician or hospital
2) Pick-up forms at the Aging & Disability Resource Center
3) Printing forms from the Internet

If printing these forms from the Internet, please assure that you print and complete all pages of the forms
you are using. To be valid, the forms must be complete and signed. The Declaration to Physicians (Living Will)
and the Power of Attorney for Health Care have accompanying letters that are not part of the legal forms
but contain information that should be read prior to completing the forms. The forms contain standardized
wording specified in the Wisconsin Statutes. If these forms are filled out and signed correctly, the completed
forms are presumed to be valid in the State of Wisconsin. Do not change the preprinted wording of the forms
unless instructions for the forms indicate you may do so. Changing the forms, contrary to their instructions
may invalidate the completed forms. If the forms are not sufficient for your needs, consult an attorney.
NATIONAL DO NOT CALL REGISTRY
The National Do Not Call Registry is open for
business, putting consumers in charge of the
telemarketing calls they get at home. The federal
government created the national registry to make
it easier and more efficient for you to stop getting
telemarketing sales calls you do not want. You
can register online www.donotcall.gov or call
(888) 382-1222; (866) 290-4236 TTY from the
number you wish to register.
WISCONSIN DO NOT CALL LIST
Signing up identifies you as someone who does
not wish to receive telemarketing calls. It’s free
and available for residential telephone customers
in Wisconsin. Your number will remain on the list
for (2) years. Adding your residential telephone
number to the list will help reduce, but not eliminate,
telemarketing calls to your home. Consumers may
sign up 24 hours a day, 7 days a week, 365 days
a year by visiting the website nocall.wisconsin.gov/
web/registration.asp or calling (866) 966-2255.
The date you sign up determines the date when
telemarketing calls should decrease. The updated
Do Not Call List is given to registered telemarketers
quarterly. Therefore, it may take 30 – 120 days for
your telephone number to get to telemarketers.
HOW CAN I STOP RECEIVING THOSE
ENDLESS CREDIT CARD OFFERS:
To get your name removed from the credit bureau
marketing lists, call (888) 567-8688 or go online
www.opoutprescreen.com.
Tell the Customer Service Representative that you
want your name removed from the credit marketing
lists that are issued by all three major credit bureaus:
Equifax, Experian and TransUnion.

• Manage your personal information wisely; make sure you know why your personal information is required
and how it will be used.
• Shred discarded personal records and documents.
• Don’t give out personal information on the telephone, mail or Internet unless you are sure who you are
speaking with.
• Pay attention to billing cycles.
• Guard your mail from theft.
• Do not carry your Social Security card, extra credit cards, birth certificate or passport, except when
necessary.
• Order copies of your credit report yearly.
• Keep personal information in a safe place.

Medicare has expanded its coverage to include
prescription drugs as of January 1, 2006. Any
person who receives benefits through Medicare
(Part A and/or B) is eligible for the Medicare
Part D drug benefit. However, Medicare Part
D is a voluntary program and if you want the
benefit you will need to enroll in a Medicare Part
D plan. Please be aware that if you do not enroll
in Medicare Part D or have other "creditable
coverage" you will be subjecting yourself to
a premium penalty if you decide to enroll at a
later time. Unlike traditional Medicare coverage,
private insurance companies whose plans have
been approved by the federal government offer
drug coverage through Medicare Part D. The
private insurance company will administer the
drug coverage not the federal government.
FREQUENTLY ASKED QUESTIONS
1. Will all my drugs be covered?
Not necessarily - you will need to check the
insurance companies' formulary (list of drugs) to
see what drugs it will cover.
2. How much will the plan and my drugs
cost?
It all depends on the Medicare Part D plan you
decide to enroll in. Medicare Part D plans have
different monthly premiums, and different copayments for the drugs. The plan could also have
a deductible and other out-of-pocket costs.
3. W
hat if I already have prescription drug
coverage?
If you already have prescription drug coverage
and have received notification that the coverage
is creditable, then you do not need to enroll
in a Medicare Part D plan. If you are currently
enrolled in a Medicare Part D plan you can keep
that plan or change plans during the annual open
enrollment period, which is October 15th to
December 7th.
4. What if I have SeniorCare?
SeniorCare will continue until December,
2015 and is considered creditable coverage.
If SeniorCare ends, there will be a special
enrollment period to enroll in a Medicare Part D
plan.
"EXTRA HELP" PAYING FOR
MEDICARE PART D COVERAGE
If you are a Medicare beneficiary with limited
income and assets, you may receive "Extra Help"
paying for your Medicare Part D plan. "Extra
Help" provides help paying for the monthly
premium and deductible for your Medicare Part
D plan. It will also give you lower co-payments
for your prescription drugs. You can apply for
"Extra Help" anytime throughout the year. If you
are receiving Medicaid and Medicare or are
enrolled in a Medicare Savings program you
are automatically eligible for "Extra Help". For
income and asset information, contact the Aging
& Disability Resource Center (ADRC). “Extra
Help” applications can be requested from Social
Security (800) 772-1213 or online www.ssa.gov.
Social Security will review your application and
send you a letter to let you know if you qualify.

• Click on “Health and Drug Plans”.
• Click on “Find & Compare Drug Plans”.
• Under Select a Search Option, click on “Begin
General Search” or “Personalized Search”.
STEP 2
• Enter requested information.
• Enter zip code where you live.
• Answer all the questions and CLICK
“Continue”.
STEP 3
Review Current Coverage and Consider
Options, CLICK “Continue”.
Under “Get Drug Costs for Available
Plans, CLICK “Enter my Drugs”
• Enter your first drug with correct dosage and
drug type.
• CLICK on “Add selected to your drug list”.
• Continue to enter your drugs until you have
entered all of your drugs; CLICK “Continue”.
• Review Your Drug Dosage and Quantities
to make sure the milligrams and doses that
appear are correct, if YES, CLICK “Continue”.
If not, correct the milligrams and doses for
each drug on the list.
NOTE: You now have the option of saving
your drug list on the Medicare website to refer
to or update your drugs on the site at a later
date. If you would like to do so, enter a security
password date and CLICK “Continue”. Make
sure you print or record your Drug List ID and

PREPARING MAKES SENSE. GET READY NOW.
Get a Kit of Emergency Supplies.
The first step is to consider how an emergency
might affect your individual needs. Plan to make it
on your own for at least three days. It’s possible that
you will not have access to a medical facility or even
a drugstore. It is crucial that you and your family
think about what kinds of resources you use on a
daily basis and what you might do if those resources
are limited or not available.
EMERGENCY SUPPLY KIT ITEMS:
• Water, one gallon of water per person per day for
at least three days, for drinking and sanitation
• Food, at least a three-day supply of non-perishable
food
• Battery-powered or hand crank radio and a NOAA
Weather Radio with tone alert and extra batteries
• First aid kit
• Whistle to signal for help
• Dust mask to help filter contaminated air and
plastic sheeting and duct tape to shelter-in-place
• Moist towelettes, garbage bags and plastic ties
for personal sanitation
• Wrench or pliers to turn off utilities
• Can opener for food (if kit contains canned food)
• Local maps
