The Youth on Track scheme is based on early intervention principles and an interagency approach to the management of the needs of these young people and their families. It is designed to prevent young people from becoming entrenched in criminal behaviour. The key priciples of Youth on Track are: Intervening earlier to divert young people from crime One-on-one case management to manage and support juvenile offenders and those at risk of offending. Separating treatment from punishment. Responding to risk and need rather than simply to crime. Responding promptly to an immediate problem. Page Email: [email protected] Web: http://www.youthontrack.lawlink.nsw.gov.au/yot/index.html 47 A case manager will be allocated to each at-risk individual and will be rresponsible for working directly with the young person and their family, or those responsible for the young person, to change those things in their environment that are leading to the offending behaviour. They will be responsible for identifying and arranging access to services and programs. The Youth on Track model has six key elements: Identification and Referral Screening Assessment Case Management Intervention Exit Youth with a Mission (YWAM) - Youth Street Teams Youth Street Teams is a dynamic program offered every Saturday from 2:00-5:45pm, consisting of different Activity Teams of adolescents aged 12 - 17.
