Certified EKG Technician:
The program goal is to provide an educational background in which students can acquire
satisfactory skills and demonstrate clinical competency in EKG procedures, thus preparing
students to successfully pass the National Certification Examination. We will be providing
guidance so that upon graduation, the student will be able to demonstrate entry level
competencies as a Certified EKG Technician.
Certified Phlebotomy Technician:
The program goal is to provide an educational background in which students acquire
satisfactory skills and demonstrate competency in Phlebotomy Procedures, thus preparing the
student to successfully pass the National Certification Examination. We will be providing
guidance so that upon graduation, the student will be able to demonstrate entry level
competencies as a Certified Phlebotomy Technician.
Certified Patient Care Technician:
The program goal is to provide and educational background in which students acquire
satisfactory skills and demonstrate competency as a Patient Care Technician, thus preparing the
student to successfully pass the National Certification Examination. We will providing guidance
so that upon graduation, the student will be able to demonstrate entry level competencies as a
Certified Patient Care Technician.
Certified Clinical Medical Assistant:
The program goal is to provide an educational background in which students acquire
satisfactory skills and demonstrate competency as a Clinical Medical Assistant, thus preparing
the student to successfully pass the National Certification Examination. We will be providing
guidance so that upon graduation, the student will be able to demonstrate entry level
competencies as a Certified Clinical Medical Assistant.

Attendance is an important issue in any course. Horizon Health Career Training Center uses the
following guidelines in determining acceptable guidelines:
Certified Clinical Medical Assistant
Course is 366 hours of class, lab and clinical. Horizon HTC guidelines set forth by NHA allows
a total of 5 days of absent time from class and 2 clinical days. This means that a student cannot
miss more than 5 days, or a total of 30 hours per 20 week training period. The breakdown
attendance states that: no more than 2 clinical days can be missed, and no more than 5 classroom
days can be missed. This will be clearly outlined separately in the course syllabus.
The student will be required to submit documentation of all excused absences to receive credit
and be allowed to make up work. 3 Tardies equal 1 absence. If a student is absent more than
that, no credit for the course can be issued. This is not affected by whether the absence is
excused or unexcused. Students may miss only 2 days from the clinical externship site. On the
6th absence from class, the student will be dropped from the program.
If the student is absent from class due to illness or unforeseen circumstances, the student will be
permitted to retake the course at a later date; this will be determined on a case-by-case basis.
Certified Phlebotomy Technician
Course is 124 hours of class, lab and clinical. Horizon HTC guidelines set forth by NHA allows
a total of 2 days of absent time from class and 2 clinical days. This means that a student cannot
miss more than 2 days, or a total of 16 hours per 8 week training period. The breakdown

In order to remain enrolled in a course, the student must maintain a 75% or better grade point
average. The student will not be sent to a clinical externship without achieving a passing grade
in the skills and abilities required for the particular course work.

The code of conduct is the professional standard for conduct, attitude, performance, and ethics
while enrolled in course work at Horizon Health Career Training Center, including the
classroom, lab, clinical facility and testing for competency. Students are expected to be
respectful to instructors, staff and patients. Disruptive behavior or inappropriate language such
as profanity, slang words and derogatory comments will not be tolerated. Students are not to
discuss personal problems or issues with externship/clinical site staff.
Students are to never perform, or attempt to perform skills for which they have not been trained
for or signed off as competent by their instructor.
While attending classroom lectures, lab skills and clinical externships, you are representing
Horizon Health Career Training Center. The externship site will be observing your work ethics,
attitude, professionalism, and dress. You will be expected to maintain professionalism at all
times, including presenting to class and externship site, be on time, in proper uniform and
maintaining a professional attitude with patients, staff, instructors etc. The first offense in failure
to do so will result in a verbal warning. The second offense will result in a written warning and
will be part of the permanent student record. The third offense will result in immediate dismissal
from the program without refund. If there are any offenses that are of a more severe nature, this
could result in immediate dismissal from the program.
The School is designated as a drug-free and smoke-free facility. The following activities are
prohibited while the student is on school property or otherwise engaged in student activities:

The manufacture, possession, use, sale, distribution, dispensation, receipt, or
transportation of any controlled substances or illegal drugs. This includes all
forms of narcotics, hallucinogens, depressants, stimulants and other drugs whose
use, possession or transfer is restricted or prohibited by law
The consumption of alcoholic beverages
Being under the influence of alcohol, illegal drugs or controlled substances in any
manner during school hours or clinical hours, whether or not it was consumed on
school premises, and whether or not it was consumed outside of school hours.
Smoking in any building, facility or vehicle owned or leased by the school on any
grounds or property, including parking lots owned or leased by the school. A
student who engages in such behavior will be subject to disciplinary action up to
and including expulsion from the school.

Classroom- Students may dress comfortably for the classroom and lab. This attire must be
appropriate and non-revealing.
Externship Site- Students must present to clinical sites appropriate uniform: Red Scrub pants,
White Top, White Lab Jacket, and White Shoes. Scrubs and Jacket must be free from wrinkles
and stains. Shoes should be non-skid and clean. Hair for females must be pulled back from the
face to the back of the neck, and long hair must be pulled up. Wild hair colors or styles are not
permitted at the clinical sites. No scarves, hats, caps, beanies, sun glasses or head coverings of
any kind are permitted at the clinical sites. No large dangling or multiple piercings are
permitted, only small hoops or posts. No body piercings are permitted. Men shall be clean and
neatly shaven with neatly styled hair. The name tag shall be worn on the left side and visible at
all times.
Smoking is not permitted in the classroom or clinical setting.
Cell phone use is not permitted in the classroom or clinical setting.
Student Privacy
Students have the right to examine certain files, academic records and documents maintained by
the school, which pertain to them.
The Program Director supervises records. Students may request a review of their records with
the Program Director. Such review will be allowed during regular business hours under
appropriate supervision. This review of records does not include records about students made by
teachers or administrators that are maintained by, and accessible to only the teachers or
administrators.

If a student receives a final course grade that he/she believes is incorrect, the matter should first
be discussed with the instructor. Direct communication between the student and the instructor
may clear up any misunderstanding.
In order to clarify any questions about a grade or grading practices, a student should use the
following procedures:
1. Discuss the final grade with the appropriate instructor.
2. If the student is not satisfied with the instructor's explanation of how the grade was
determined, the student shall submit a written appeal to the program coordinator. The student's
appeal must be signed and dated. The program coordinator will review the syllabus and how the
final grade was determined with the instructor, and will respond to the student in writing within
five (5) business days.
3. If the student is not satisfied with the decision of the program coordinator, the student may
direct an appeal to the Chief Executive Officer/President. The student's appeal must be written,
signed, and dated. The President will review the syllabus and how the final grade was
determined, and will respond to the student in writing within five (5) business days. The decision
of the President is final.
4. Any other grievances not related to grading issues will be taken directly to the Director of
Programs. The Director of Programs and the Chief Executive Officer will then review the
matter. Once the issues have been reviewed and investigated, a meeting will be arranged for all
parties involved for final resolution.

Sexual harassment is illegal and is not tolerated. Horizon HTC is committed to providing a
learning environment that is free from unlawful harassment.
The School’s sexual harassment policy focuses on prevention and encourages individuals to
report inappropriate behavior, is also insures a commitment on the part of the School to take
prompt and effective disciplinary action against any individual who violates this policy.
All employees and students are required to comply with this policy.

Legal definition: Sexual harassment may involve the behavior of a person of either sex
against a person of the same or opposite sex. This occurs when such behavior constitutes
unwelcome sexual advances, unwelcome requests for sexual favors, and other
unwelcome verbal or physical behavior of a sexual nature where:
Submission to such conduct is made, either explicitly or implicitly, a term or condition of
an individual’s educational experience;
Submission to or rejection of such conduct by an individual is used as a basis for
educational decisions affecting such an individual; or
Such conduct is sufficiently severe and pervasive as to alter the conditions of, or have the
purpose or effect of substantially interfering with an individual’s academic performance
by creating an intimidating, hostile, or offensive educational environment.
Prohibited Conduct: Sexual harassment may occur in a variety of relationships, including
employees harassing students, students harassing students, students harassing employees,
and involving other persons having business with, visiting the office or educational
environment. Sexual harassment may occur when it is directed at members of the same
gender. The School’s policy on sexual harassment prohibits but is not limited to the
following conduct:
a. Unwelcome sexual flirtations, advances or propositions
b. Unwelcome words, actions, or comments based on an individual’s gender
c. Sexually suggestive or offensive personal references about an individual
d. Subtle pressure or requests for dates or sexual activities
e. Unwanted physical conduct or contact, including touching, pinching, brushing the
body, and impeding or blocking one’s movement
f. Sexually explicit or offensive jokes and references, pictures or photographs,
suggestive objects, verbal comments, leering or whistling.

Reporting Suspected Sexual Harassment: An initial course of action for any student who
feels that he/she is being sexually harassed is for them to inform the harasser that their
conduct is not welcomed and must stop. If this course of action is not feasible, might be
feasible but unsuccessful, or the individual might be uncomfortable dealing with the
matter, the individual is encouraged to come forward, the School provides several
channels of communication for formal or informal complaint resolution.
7. Informal Complaint Resolution, Information and Advising. If an individual feels they are
being harassed, or are uncertain as to whether what they are experiencing is sexual
harassment, they may approach either an instructor or other member of the School’s
management team to seek information and advisement without lodging a formal
complaint. The third party will meet privately with each person involved to try to clarify
their perceptions and attempt to develop a mutually acceptable understanding that can
insure that the parties are comfortable with their future interactions.
8. Formal Complaint Resolution: Formal complaints of sexual harassment should be
reported first to the Programs Director and then to the School’s President. A written
complaint will be filed and if the complainant chooses not to proceed with a formal
complaint. they will be asked to state this in writing. The Programs director will be
responsible for formally investigating the complaint. During the investigation, the
Program director will meet with, and hear the complainant, the respondent, and any
witnesses identified by each party. To the extent possible, all complaints will be handled
confidentially, with the facts made available only to those who have a compelling need to
know for purposes of investigation or resolution.
9. The Program Director will make a determination as to whether there was a violation of
policy and will inform the complainant and the accused student or employee of the final
disposition of the complaint. The Program Director will decide what corrective action
will be taken or whether any discipline will be imposed. Discipline may range from an
oral reprimand, up to and including termination/dismissal from the school or employment
or any other appropriate remedial action.

1.
Students who pre-register for a course may receive a full refund of all tuition and fees
paid with the exception of the non-refundable registration fee, if they cancel prior to the
scheduled enrollment date.
2. Students who withdraw/drop the course during the first three (3) days will receive a
100% refund of tuition charges, less the non-refundable registration fee, and less the cost
of any supplies the student receives but does not return to the school.
3. The applicant requesting cancellation more than three (3) business days after signing the
contract, or the student completing no more than five (5) percent of instructional time is
refunded no less than ninety-five (95) percent of tuition.
4. The student completing more than five (5) percent, but no more than ten (10) percent of
instructional time is refunded no less than ninety (90) percent of tuition.
5. The student completing more than ten (10) percent, but no more than twenty-five (25)
percent of instructional time is refunded no less than seventy-five (75) percent of tuition.
6. The student completing more than twenty-five (25) percent but no more than fifty (50)
percent of instructional time is refunded no less than fifty (50) percent of tuition.
7. The student completing more than fifty (50) percent of instructional time is informed that
the institution is not required to issue a refund.
8. Refunds are made in full to the student within thirty (30) days of the date of withdrawal.
9. No refunds will be given for expendable supplies and equipment, or books that have been
written in.
10. The school will ensure that any deposits paid will be credited as tuition payments unless
it is designated otherwise on the student’s receipt.
11. The application fee will be charged only once for each program of study.
12. All fees for books and supplies are stated specifically with each program of study, and
due to the nature of the course of study, will not be refunded unless the student withdraws
from the class within the first week and materials remain unopened and unused.
13. Refunds will be made within 30 days of the date of withdrawal.
14. If the student experiences extenuating circumstances such as injury, prolonged illness, or
death, or other circumstances which prohibit completion of the course, student will be
offered the opportunity to retake the course at the next time the course is offered.
15. If it is necessary to change or cancel a course of study in such a way that a student who
has started the program or course is unable to continue, the school will make every effort
to accommodate the student with another scheduled course. If the alternate arrangements
cannot be met, the student will be refunded all money paid for tuition and any books and
supplies that are returned unused.

Horizon Health Career Training Center offers assistance to students and graduates who are
seeking work related to their field of study.
Assistance is provided at no charge for services such as: resume preparation, mock interviews
and interview preparation and can be scheduled for those students who are interested.
Horizon Health Career Training Center provides services to assist with all aspects of the job
search process and career development; including, but not limited to, resume writing assistance
and interviewing techniques. Placement services are available for graduates in good standing
should they suffer job loss or would like further job search advice. Disclaimer: Although we do
offer the above assistance, the School specifically disclaims any guarantee of job placement for
the student upon graduation.

HIPAA
Horizon Health Career Training Center and the participating Clinical facilities (Externship) are
governed by the Health Insurance Portability & Accountability Act, or “HIPAA”. HIPAA
requires that all employees, physicians, consultants, students and vendors abide by the practicum
rules with regard to patient confidentiality.
By execution of this Confidentiality Agreement, the student will agree not to disclose any

Conduct in the classroom must follow accepted safety guidelines. No disrespectful conduct
towards others, including the instructor or office personnel will be tolerated.
Student dress should be in compliance with Horizon Health Career Center’s guidelines at all
times.
No tape recorders are permitted in the classroom.
Cell phone use, particularly texting, is not permitted in the classroom. Students are expected to
silence cell phones and may only use the cell phone during designated breaks.
Food and drinks are not permitted in the classroom.
Smoking will be allowed outside of the building during designated breaks only. Smoking will

Jobs Outlook for Medical Assistants
According to the United States Department of Labor, Most medical assistants work in offices of
physicians. Employment for medical assistants is projected to grow much faster than average,
ranking medical assistants among the fastest growing occupations over the 2006-16 decade. Job
prospects should be excellent! Medical assistants held about 417,000 jobs in 2008. About 62
percent worked in offices of physicians; 12 percent worked in public and private hospitals,
including inpatient and outpatient facilities; and 11 percent worked in offices of other health
practitioners, such as chiropractors, optometrists, and podiatrists. Most of the remainder worked
in other healthcare industries such as outpatient care centers and nursing and residential care
facilities. Job seekers who want to work as a medical assistant should find excellent job
prospects. Medical Assistants are projected to account for a very large number of new jobs, and
many other opportunities will come from the need to replace workers leaving the occupation.
Those with formal training or experience should have the best job opportunities.
Medical Assistant Salaries:
According to the United States Department of Labor, the earnings of medical assistants vary
depending on their experience, skill level, and location. Median annual earnings of wage-andsalary medical assistants were $29, 370 in 2012. The average medical assistant in 2012 earned
$30,550 and those in the highest percentage earned more than $41,570.
Employment of medical assistants is expected to grow much faster than the average for all
occupations through the year 2012, as the health services industry expands because of
technological advances in medicine, and a growing and aging population. Increasing utilization
of medical assistants in the rapidly-growing healthcare industries will result in fast employment
growth for the occupation. In fact, medical assistants are projected to be the fastest growing
occupation over the 2002–12 periods.
Employment growth will be driven by the increase in the number of group practices, clinics, and
other healthcare facilities that need a high proportion of support personnel, particularly the
flexible medical assistant who can handle both administrative and clinical duties. Medical
assistants work primarily in outpatient settings, which are expected to exhibit much faster-thanaverage growth

Description
Certified EKG Technicians are an important member of the Healthcare team. EKG Technicians
assist physicians by performing electrocardiograms, or EKG’s, tests that monitor activity of the
heart. EKG technicians operate machines that record the electrical activity of the patient’s heart.
EKG rhythms provide important data for the diagnosis of heart conditions. As a Certified EKG
Technician, the following tests may be performed: set up and administer EKG’s, Stress Tests,
prepare patients for Holter monitors or other ambulatory monitoring equipment, edit and deliver
final test results to the physician for analysis, schedule appointments and transcribe physicians’
interpretations. The course will include theory and practical training in obtaining EKGs, Stress
Test, Holter and Ambulatory monitoring equipment.
Length of program:
The Certified EKG Technician Course consists of a total of 124 classroom and clinical hours.
Daytime and Evening classes will be offered. Evening classes will be offered on Monday and
Wednesday Evening from 5-10 p.m. Day classes will be scheduled based on demand, and will
be announced as they are scheduled. Externship/Clinical are part of the course, and the student
will be required to attend 64 clinical hours in settings which include; physician offices and
hospital settings.

The Patient Care Technician is a position that has more responsibilities than the position of
nursing assistant. The responsibilities involve skills that are more comprehensive. The Bureau
of Labor Statistics, Occupational Employment statistics 2001 surveys this position as one of the
“new and emerging occupations”. The roles and responsibilities for this position will require a
competent individual who can multi-task and maintain a professional demeanor at all times. The
CPCT works under the supervision of doctors and nurses to implement, monitor and document
patient care. The CPCT assists patients who cannot get in and out of bed, need assistance sitting
or ambulating, with meal preparation, personal hygiene and many other activities.

Description
A Phlebotomy Technician is a member of the healthcare team who is skilled in various
techniques which include: obtaining, labeling, sorting, preparing and transporting blood samples.
Phlebotomy technicians also obtain sputum cultures, throat cultures, as well as urine samples and
cultures. The Phlebotomy student is instructed in patient rights, infection control, universal
precautions, delivery of labs and centrifuging. The student is trained in observing safety
precautions as mandated by the Occupational Safety and Health Administration and by the
Clinical Laboratory Improvement Amendments. The course will include theory and practical
training in obtaining adequate and correct blood specimens by capillary and/or venipuncture
samples from patients.
Length of Program
The Phlebotomy Technician course consists of a total of 124 classroom and clinical hours.
Classes will be held on Monday and Wednesday evenings from 5pm to 10 pm.
Externship/Clinical is a part of the course, and the student will be required to attend at least 64
hours. Students must complete 100 successful venipunctures, and may need to attend more than
the required clinical hours to complete this if necessary.
