PREPAID CARD.
Complete the online claim form at
KitchenAid.ca/specialoffers and include the
product’s serial number when completing the
online form. You will be required to upload
an image of your appliance purchase sales
receipt to receive your prepaid card by mail.
If you prefer to mail in your submission, the
form can be printed from the above site and
mailed to the address on the form with a copy
of your appliance purchase sales receipt.
ELIGIBILITY REQUIREMENTS AND OFFER DETAILS
This offer applies only to qualifying new KitchenAid® Stand Mixers,
Stand Mixer Attachments, Food Processors, Kettles, Sparkling
Beverage Makers, Juicers or Cookware purchased between April 14,
2016 and May 12, 2016. Only the models identified in the qualifying
appliances table are eligible. Prepaid card value dependent on
qualifying small appliance or cookware purchased. No substitute
models qualify.
In order to receive your KitchenAid® Visa® Prepaid Card by mail, you
must complete and submit a claim form at KitchenAid.ca/specialoffers
before June 12, 2016, or offer will be void. Submission must include a
copy of invoice for the qualifying purchase. No substitutes qualify.
Missing information will delay offer fulfillment. For online submission
allow up to 4 weeks to receive your Mother’s Day Prepaid card Offer
Visa prepaid card by mail. If you chose to submit by mail please allow
for 8-10 weeks to receive your Mother’s Day Prepaid card Offer Visa
prepaid card by mail. Offer is open only to residents of Canada
