This has been a wonderful year of growth and change for hospice. I am
proud to have been Board President during this unprecedented time in High
Peaks Hospice and Palliative Care’s history. May 28th marks the 25th
anniversary of the founding of High Peaks Hospice and Palliative Care.

New look, same heart. Public relations and promotion has always been
important for hospice, yet sometimes a significant stumbling block. “Dying
well” with the support of hospice isn’t always what people in our death
denying society want to hear about. In celebration of our 25th anniversary,
High Peaks Hospice and Palliative Care, through hours of dedicated and
creative work by staff, has approved a new “look” with a new logo and
promotional materials. We hope that you will like it as much as we do. The
new look brings a little something extra to our ability to educate the
community about hospice and promote our signature fundraising events.
We have been providing care to people in the final months and days of
their lives for 25 years. Volunteers played a key role in our founding and
still are fundamental to all hospice work. We continue to emphasize the
need for volunteers throughout our organization and successful recruitment
continues year round. These individuals bring a wealth of knowledge,
caring, experience and enthusiasm to the organization.
A new program starting this year is Transitions. This is a program that assists
clients who have a life-limiting illness with making decisions about their
healthcare options. Staff and volunteers will act as health coaches and
navigators for clients who do not yet qualify for hospice, and their caregivers as they deal with complex care issues and medical systems.
High Peaks Hospice and Palliative Care has never, in all the years of our
existence (25 years!) denied care to a patient due to a lack of funds. We
hope we can maintain that track record of extraordinary achievement, but
serious cuts from Medicare, rising costs of care, and more patients who are
in need of charity care are effecting our financial picture. On behalf of all
of us at High Peaks Hospice and Palliative Care, and on behalf of all the
future patients and families who will depend on us for their end of life care,
we sincerely thank you for your support in our fundraising
campaigns
over the years. We hope we can continue to count on your support in the
future.
Congratulations to the staff and volunteers, board of directors and friends of
High Peaks Hospice and Palliative Care for making our first 25 years of
caring be a great success.

The Patient and Family Support Program
Palliative Care and the Hospice Care Team
Bereavement and Grief Services
Palliative care is a continuum of care and services provided to
relieve the symptoms and stress of serious illnesses. Symptoms
may be physical, emotional, or spiritual. Our goal is to improve
the quality of life for patients and the people who care about
them. Palliative care can be done in one’s own home, in an
assisted living facility, nursing home, or the hospital. In addition
to the nursing care provided by the hospice team are social
workers, spiritual care and bereavement counselors, and the
volunteer coordinator. Working in partnership with the patients’
own primary doctors, the team:
The journey of grief can be overwhelming and unpredictable.
Grieving the death of a loved one can be a painful and lonely
experience. It is the realization that our lives are changed
forever. It alters our relationships and our sense of identity. It
disrupts our daily habits and can leave us grieving for hopes
and dreams of an imagined future together. Frequently, it is
difficult for us to know if the feelings and reactions we are
experiencing are normal ones.
Society often encourage us to “get over it and get on with life,”
leading to further pain and isolation. Grief is the natural response to profound loss and is the expression of the strong
bond you had with the person who died. No two people
grieve in exactly the same way and how we express our grief is
influenced by many things such as our religious and personal
beliefs, the circumstances of the death, and how we dealt with
previous losses.


Care Companions and Volunteers
Supportive Counseling with Social Work
Our social workers provide end of life care that listens with
respect, cares with compassion, supports with choice and
comforts with understanding.
Licensed social workers play a pivotal role in the care of
patients and families. They initially meet with new patients,
their loved ones and caregivers to explain and discuss hospice
services and offer counseling and practical support. They
ensure that patients and their loved ones are treated with
dignity and that we respect diverse cultural and religious
beliefs. They share their knowledge about issues related to
advanced illness, dying, bereavement, and community supports
that may be of help.
Our social workers help patients and loved ones articulate and
reach their end of life goals addressing care and comfort;
emotional support; and guidance related to one’s illness and
comprehensive care.
Our volunteers understand the importance of hospice care.
Some have had experience with hospice themselves and
want to help other families. Others are simply caring people
dedicated to supporting those in need. They all see their
volunteer experience as an opportunity to give back to the
community.
Our volunteers are now referred to as “care companions” to
better reflect the important role they play.
Hospice care companions assist patients and families in
myriad ways including companionship and support,
running errands, special projects, respite care, meal and
snack preparation, musical interludes, reading, pet therapy,
knitting and sewing projects, and vigil volunteers. They also
provide general office assistance and help with fundraising.
We welcome other creative ways to use the talents and
interests of volunteers.
Our volunteers donated 5,857 hours in 2010!
Spiritual Counseling
Spiritual care is an important factor in the holistic care concept of High Peaks Hospice and Palliative Care. Often primary concerns
of a dying patient are spiritual questions related to their reflections on their sense of purpose, the meaning of their life, self-image,
and hope for the future. The kind of spiritual care hospice provides depends upon each person’s needs and desires – and what spirituality means to him or
her. Our spiritual care coordinators take great care to respect everyone’s unique understanding of faith, religion, or spirituality and
to work with them within that framework. Everyone is asked, when they are admitted, if they would like to receive spiritual care,
or they can contact a spiritual care counselor at any point to request spiritual care.
Spiritual care can also provide support in exploring how religious beliefs are helping or hindering someone in dealing with current
challenges. For many, their religion provides a great source of comfort and strength as they face their own death or that of a loved
one. Others may feel frustration or disappointment in what their faith has to offer. They may value the opportunity to re-visit their
religious beliefs and discern, for themselves, what they choose to embrace or discard.
Spiritual care often means sitting with people and talking about their life experiences, in light of their faith. It may include praying,
reading sacred texts, meditating and singing together. We also connect patients with pastoral leaders from their own faith traditions
and arrange for visits.

Message from the Executive Director
As we reflect on the past year of 2010, “change is the constant” seems especially relevant.
Without a doubt, these are changing times in American healthcare and economy. Yet our
53 staff members and over 250 volunteers across the Adirondacks and North Country are
playing an instrumental role in living the tenets of the vision of High Peaks Hospice and
Palliative Care to offer “dignity, comfort and peace at the end of life” to all individuals
wherever they call home.
The footprint of our organization in the communities it serves is expanding, and our commitment to
transforming the way hospice and palliative care is delivered in every corner of our region is becoming
a reality.
Our expanded, collaborative care approach means we’re closer to delivering the right care, at the right
time, in the right place. When you consider the growing health care needs and the rural and aging
population that lives with limited access to medical care it has never been more important to deliver
positive growth in every community, family and home we touch.
At home or close-to-home hospice care is especially critical for our rural neighbors and is the guiding
force behind High Peaks Hospice and Palliative Care’s efforts over the past 25 years to partner with local
physicians and hospitals in small towns across the region.
In 2010 the national standards of practice for hospice and palliative care were upgraded; healthcare
reform promises to have a significant impact on hospice care in both scope of service and level of
funding; the age range of those entering hospice was from 3 to 92; and best practices in improving
quality of patient and family care offered great prospects for those we serve.
And so in 2010 we began a renewed effort to refresh and develop all of our programs: bereavement and
grief counseling, spiritual care, social work, nursing, community education, volunteer roles - all based
on the three core tenets of our mission: Dignity, Comfort and Peace.
We have also strengthened our focus on the role of technology. We continue to take a lead in local
efforts to incorporate new technologies in treatment, records management and patient safety, sharing
best practices, optimizing efficiencies and streamlining communications that result in superior patient
care at every turn. Our service area covers 4,600 square miles and the ability to keep staff skills sharp
and advanced is a challenge often best addressed through the use of new technologies.
Through education, palliative care and safety initiatives, health advocacy, counseling and support
groups, our staff, volunteers, medical directors, hospice physicians and our healthcare partners form a
High Peaks Hospice and Palliative Care that is passionate about its mission to provide end of life care
that listens with respect, cares with compassion, supports with choice and comforts with understanding
now and well into the future.
Ultimately, we’re working on a daily basis to help people understand and be a part of hospice as a
partner for life whether as a volunteer, a donor, a family member or a patient. That means that High
Peaks Hospice and Palliative Care is here, nurturing the understanding and role of hospice in every
community, providing you with resources to maintain a quality of life and appreciation for every day
you have.

A special bond...
While all those who High Peaks Hospice and Palliative
Care serves are treated with the utmost care, dignity and
compassion, every once in a while, a very special bond
is formed between one of our staff and one of our
patients.
The story of Shane Benham and Pastor Jeff Gray is one
such bond.
Shane Benham, a 29 year old from Saranac Lake was born
with Huntington Disease. His symptoms began at the age of
15. Jeff Gray, a High Peaks Hospice and Palliative Care Chaplain,
spent many hours beginning
November 2009 with Shane, as a
Chaplain and as a friend. In September 2010 Jeff was planning on
running in the Lake Placid Half
Marathon and dedicated the race
to Shane. Jeff wrote, “After enjoying many hours with Shane, I grew
to love him as a friend. Knowing
that Shane had been a crosscountry runner before Huntington Disease, I knew he would
enjoy being a part of my half-marathon. It was an honor to
push him in his wheelchair the last half mile of the race.
We’ve been together for the last part of Shane’s journey on
Earth. Being together for the final half mile of the race was
symbolic of our time together. Shane has blessed me in immeasurable ways!” Shane passed away peacefully at his home
with his loved ones, including Jeff Gray, at his side.

Other 2010 Highlights
On November 3, during National Hospice and Palliative Care
Month, three local hospices - HPH&PC, Hospice of the North
Country and Hospice & Palliative Care of St. Lawrence Valley hosted a conference in Lake Placid. “Write Your Own
Ending,” was a daylong event presented in the morning by
nationally renowned end-of-life advocate, investigative
journalist and author Stephen Kiernan and in the afternoon by
Linda Piotrowski, chaplain and Pastoral Care Coordinator in
the palliative care unit at Dartmouth Hitchcock Medical
Center.

After intense training 2010 saw the
implementation of electronic patient
records through Suncoast Solutions.


state, Vermont, and we welcomed
Shawn Galbreath as our new
Executive Director. Shawn brings
with her many years of experience in
the health care field.

Our logo was redesigned to better
reflect the interconnectedness of
where we live and the services we
provide.

To best educate future patients,
families and the community at large,
the process of redesigning and
updating all of our brochures and
other materials was begun.

Our website is undergoing redesign
to make it more user friendly and
informational for all and to better
facilitate use by healthcare
professionals.

Staff attended the yearly two day
Hospice and Palliative Care
Association of New York State’s
conference where they were able to
network with their peers from
throughout New York and attend
workshops that offered the most up
to date information on everything
from pain management to Veterans
Programs.

The clinical office staffs instituted
“paper routes” to provide, on a
regular basis, up to date information
regarding the benefits of hospice care
to professionals such as physicians.
Over 100 individuals including hospice staff, therapists,
clergy, other professionals as well as the interested public
were treated to a day of compelling information about the
benefits of palliative care at the end of life.
Our yearly Holiday Luncheon, where we thank and recognize
our staff for their dedicated service to the agency and the
patients we serve, was held at the rustically beautiful Point O’
Pines Camp on Brant Lake. There was an exciting raffle but the
highlight of the day was featured speaker Cal Walker.
Cal is a community activist in Ithaca, NY who has worked
extensively with people in all walks of life. Cal spoke of his
personal experience when his parents were dying and learning
about hospice care - clearly highlighting the need for greater
awareness. His words were uplifting and encouraging to all
who work and volunteer for hospice.

Dear Friends,
A message from our founders
1986 to 2011
Twenty five years ago who would have known how far our efforts, which grew
from the need for an essential community service, along with the help of many
dedicated individuals and a great deal of energy, and dedication would go?
In February of 1986, David and I were having lunch with Dr. Robert Kasulke,
one of David’s surgical partners, when the subject of the recent decision by
Medicare that being terminally ill was no longer a reimbursable condition
came up. Patients no longer had Medicare coverage for their care during the
last stage of their life.
In some urban areas of New York State and the nation, coverage was provided
through certified hospice organizations. However, rural areas were deemed
difficult to serve due to the many services and therapists needed under the Medicare hospice regulation.
We decided to design a plan for the delivery of rural hospice services.
The supportive response from community members, county directors of public health agencies,
hospitals, churches, nursing homes and local volunteer organizations was immediate and amazing.
Soon 96 hospice volunteers were trained and certified for patient care. Volunteers also staffed the office.
The required Certification of Need application was compiled and delivered to the New York State
Department of Health. Then presentations before review boards such as the Health System Agency in
Albany and the State Hospital Review and Planning in New York City won approval for High Peaks
Hospice to deliver care to patients and families.
Office space was provided first by the Trudeau Institute, followed by Adirondack Medical Center and
then American Management Association as High Peaks Hospice developed and grew.
Further expansions of service by staff and volunteers resulted in High Peaks Hospice and Palliative Care
as it is today. Today the agency’s Saranac Lake office now occupies an office complex donated by
Adirondack Internal Medicine and Pediatrics. Through the dedicated efforts of Frances Filshie, the
agency also expanded to include offices in Port Henry and Queensbury. High Peaks Hospice and
Palliative Care now provides care in all of Essex and Warren counties as well as portions of Franklin,
Hamilton and St. Lawrence counties. There are currently 53 professional staff members and more than
250 active volunteers. Over the past twenty five years, more than 5,200 patients and their families have
been served.
Twenty five years after that initial discussion over lunch in 1986, we are still amazed when we meet with
High Peaks Hospice and Palliative Care staff and volunteers. Their dedication and loving support for
terminally-ill patients and their families are truly remarkable.

25 years - looking back
In 1986 High Peaks Hospice began as an all volunteer agency and served
a modest 3 patients in its first year of operation. In the past ten years alone
High Peaks Hospice and Palliative Care served 3,900 patients - averaging
390 per year.
Much has changed over twenty five years. From a small service area (see
diagram right) the agency now encompasses over 4,600 square miles of
cities, mountains, lakes and villages. Staff drive tens of thousands of miles
each year to provide care to our patients and families.
From a small core group of founder volunteers in 1986, the volunteer base
quickly grew to 96 before the end of that year and has numbered in the
thousands since then.
The one constant through the years has been the dedication and
professionalism of the staff and volunteers of High Peaks Hospice and
Palliative Care and their unfailing belief that quality of life really does
matter.
Andy Fortune was one of the founders.
He provided community outreach and
bereavement for many years and continues his support to this day as he roars off
on his motorcycle during our rally each
year!
Keela and Jim Rogers were
dedicated volunteers for many
years and their son, Nip, follows in their
large footsteps. Bill Stowe (right), hospice
staff, along with the Rogers were
instrumental in starting Radio Day 19
years ago which continues to be
HPH&PC’s most successful fundraiser.

not be complete without paying tribute to
Fran Filshie’s exceptional contributions.
Fran was on the Board of Directors for
many years. She served as Executive
Director more than once, continues to
serve on the Governance Board and was
instrumental in the establishment of both
the Essex and Warren County offices.
Judy Courtney, LPN, and longest
serving staff member, started
working for High Peaks Hospice in
January 1989. 22 years later she
still provides care to our patients!

Over the past 25 years thousands of your friends and neighbors, and many of you, have donated your time,

Fundraising Highlights of 2010
The 2010 fundraising calendar got off to a rousing start with the Claire Sweet Memorial Golf Tournament
held at the spectacular Top of the World Resort overlooking Lake George. The extended Sweet family,
once again, greeted and thanked the players for their support of the event. 100 enthusiastic golfers enjoyed
a beautiful day of golf and raised over $16,000 Three days later, the 18th Radio Day was off and running
at 6am at Adirondack Medical Center in Saranac Lake. Fourteen hours later the phones were finally quiet
but only after $55,000 was pledged. On a lovely mid-August evening a crowd of well over 100 partiers
went to the rustic and lovely stone home of Paul & Lelia Matthews in Keene and enjoyed a wonderful
evening of camaraderie A few weeks later, in early September, Norman and Marian Wolgin hosted a
delightful party at their stately home on the shores of Lake George The Tri-Lakes Motorcycle Rally
changed venues in 2010 beginning at our offices in Saranac Lake with an enthusiastic crowd on a cool
September morning and ending at Charlie’s Inn in Lake Clear with a well-earned and delicious lunch
The inaugural 5K Fun Run for Hospice took place on a frigid but sunny morning in early October in
Warrensburg. Runners and walkers of all ages came out to show their support of hospice services in
Warren County
The final event of 2010 was the Halloween Happening which celebrated the spooky
and eerie season of mischief and pranks at the Hand House in Elizabethtown with music, food and fabulous
costumes!

Following the ride, which included two NYS Troopers

enjoyed by casual walker pushing baby carriages as
well as seasoned runners. HPH&PC Medical Director,
Dr. Jim North, clocked the fastest time in 20 minutes,

2010 Giving
2010 was a challenging year of fundraising for not-for-profits across the nation and the world. It was no
different for High Peaks Hospice and Palliative Care. While our total fundraising income was down from
2009, we were heartened by the generosity of all of our committed supporters, and by the large number of
first time donors. As the world’s economies continue to fluctuate and struggle to recover, High Peaks
Hospice and Palliative Care staff and volunteers will continue to work hard to provide the funds needed to
offer the best possible end of life care to your families, friends and neighbors.
