P.P.H. Who we are And What we do! GRAYSON COUNTY Texas Grayson County Greater Texoma Area • Grayson County is located in North Texas just South of Oklahoma border and 60 miles North of Dallas, Texas. County Information • As of 2012, Grayson County had 121,935 residents. • The County covered 979 square miles. • Lake Texoma is located at the North end of Grayson County with McKinney, Plano, Richardson and Dallas to the South. • Sherman is the county seat. • There are 2 colleges in the county Austin College Grayson College • Areas of Interest Hagerman National Wildlife Refuge Perrin Field Eisenhower State Park Frontier Village Eisenhower Birthplace • Located in the heart of Denison, Eisenhower Birthplace State Historic Site showcases the home where our 34th U. S. President, Dwight David Eisenhower, was born in 1890. The visitors center celebrates “Ike” with memorabilia and exhibits featuring his many accomplishments as president and war hero. POTENTIALLY PREVENTABLE HOSPITALIZATIONS Our Mission: Preventable Hospitalization Conditions • Bacterial Pneumonia • Urinary Tract Infections • Dehydration • C.O.P.D – Stop Smoking Program IMMUNIZATIONS • • • FREE FLU AND PNEUMONIA IMMUNIZATIONS ARE BEING PROVIDED TO QUALIFYING ADULT RESIDENTS. 2013-14 PLAN OF ACTION TARGETS 2650 HIGH RISK ADULTS FOR BACTERIAL PNEUMONIA AND INFLUENZA IMMUNIZATIONS. PLAN OF ACTION TARGETS AT LEAST 1900 ADULT RESIDENTS DIAGNOSED WITH OR AT HIGH RISK FOR COPD TO BE VACCINATED FOR BACTERIAL PNEUMONIA AND INFLUENZA. CLINICS IMMUNIZATION CLINICS IN SHERMAN AND DENISON AT THE FOLLOWING LOCATIONS: * Callie Clinic * Mission Texoma * Grand Central Station Soup Kitchen * Share Ministries *Lake Texoma Dental Clinic * House of Hope * Goodwill Stores in Grayson County * Grayson County Health Dept. OUR PLAN OF ACTION FOR BACTERIAL PNEUMONIA • Vaccinate at least 2,650 high risk adult residents for bacterial pneumonia and influenza. PPH will travel to Share Ministries Food Pantry and Grand Central Station Soup Kitchen, TCC (MHMR) in Denison and Sherman, Callie Clinic, churches, nursing homes and assisted living facilities. • • • Patient Education and or Health Literacy for high risk adults PPH will provide classes, pamphlets, and other educational materials to Housing Authority residents, SNAP Centers, Callie Clinic, House of Hope, TCC (MHMR) in Sherman and Denison, Share Ministries Food Pantry, Grand Central Soup Kitchen, and TAPS transportation services. Education opportunities to general population Work through the Ministerial Alliance, TAPS, Public Service Announcements on radio and television, Health Fairs, and Public Libraries. Educate at least 450 healthcare providers. PPH will hold classes at nursing and assisted living facilities, two hospitals, TCC staff, fire departments, and health fairs for caregivers. OUR PLAN OF ACTION FOR DEHYDRATION • Provide patient education and health literacy for those at high risk PPH will provide patient education for Share Ministries and Grand Central Station Soup Kitchen, Callie Clinic, SNAP Centers, House of Hope, TCC (MHMR), and Housing Authority residents. • Provide education opportunities to general population PPH will participate in Community Great Days of Service, work with Ministerial Alliances, articles in area newspapers, public service announcements on radio and television, coordinate with public libraries, and health fairs. • Educate healthcare providers Hold classes at nursing homes and assisted living facilities, two hospitals, two TCC locations, fire departments in Grayson County and educate EMT’s/paramedics, health fairs for care givers. OUR PLAN OF ACTION FOR URINARY TRACT INFECTIONS • Provide Patient Education for those at high risk for UTI’s Provide classes and education materials to Housing Authority residents, SNAP Centers, Callie Clinic and House of Hope, TCC (MHMR), Share Ministries and Grand Central Station Soup Kitchen. • Education opportunities to general population Work through the Ministerial Alliance, TAPS, coordinate with Public Libraries, submit articles to area newspapers, public service announcements on radio and tv. • Educate healthcare providers Hold classes for providers at nursing homes and assisted living facilities, area hospitals, TCC (MHMR), health fairs for caregivers, Home Health agencies. OUR ACTION PLAN FOR C.O.P.D. • Vaccinate at least 1900 adults with or at high risk of COPD for bacterial pneumonia Set up shot clinics at Share Ministries, TCC, Callie Clinic and House of Hope, nursing homes, assisted living facilities, and churches, partner with Coalition Members in providing vaccines for patients with COPD. • Provide Smoking Cessation Services to at least 300 adult residents PPH will work with Greater Texoma Health Clinic, classes and services at Share Ministries Food Pantry, Health Fairs, refer patients to Quit Line, work with United Way, TCOG and Coalition Members to set up a program. • Provide patient education to at least 300 adult residents Hold classes at Share Ministries, SNAP Centers, nursing homes and assisted living facilities, pass out educational materials at Health Fairs, TAPS, Soup Kitchen. • Provide 4 education opportunities to general population Classes and education materials for county employees, jail inmates, Ministerial Alliances, newspaper articles, PSA announcements. …COPD CONTINUED • Educate at lest 450 healthcare providers how to prevent and/or manage COPD PPH will travel to area Fire Departments to educate EMT’s and Paramedics, classes at nursing homes and assisted living facilities, classes for area hospitals, health fair for caregivers, healthcare providers at Greater Texoma Health Clinic. HOW’S IT GOING? Neither rain nor sleet – nor gloom of night shall keep us from vaccinating residents of Grayson County. PPH Vaccinates • Grayson County PPH along with Coalition Partners are getting out in the County and have held vaccination clinics outdoors, indoors, rain or shine…at health fairs… Dr. McAnany’s Free Dental Clinic… even a drive thru flu clinic held at Loy Lake Frontier Village Want To Quit? • PPH introduces “Stop Smoking Program” • Working with Greater Texoma Health Clinic, TCOG, and other Coalition Members to provide Smoking Cessation Classes and services. WHAT’S CHALLENGING? • Transporting display boards, tables, chairs, medical supplies, vaccines. • Packing and unpacking, packing and unpacking • Taking down tent (we’re good at setting up)! Our Life On The Road • No matter how hard we try…it is still a moving job So far…this is the only thing we haven’t had happen!!!! Nursing home and family could not get her to drink water. PPH Nurse Sandra Clark went to visit and took her a gift…an insulated cup with a rubberized grip, personalized just for her. The patient’s hands could grip the cup without it slipping, and it had a drinking spout and place for a straw. The Nurse talked with her about the importance of drinking water so she would not have to go back in the hospital. The patient has not been hospitalized again for Dehydration in the last 8 months.
