On Monday, November, 16, 2015, nurses in all levels and all roles from across the
CHI St. Vincent health system gathered at Wildwood Park in Little Rock to participate
in a Staffing Innovation Summit. The group participated in team building activities and
heard about challenges across the United States as well as regionally and locally
contributing to a national RN shortage. Current strategies were reviewed including
recruitment and retention initiatives, skill mix on medical/surgical units, and new programs that could be implemented promptly after the
first of the year. The event and outcomes will be profiled in our next edition of Nursing News. (see p. 5)

When Leaders facilitate Unplanned Change
Magnet Source
of Evidence TL5
On October 30, 2015, the organization experienced a water link that resulted in a Code Green. During this event,
there were numerous other codes that occurred. It took teamwork to get the job done! I was lucky to have some
great team member working that night. I first would like to thank Sharon Crawford, RN. She was staffing on 4N. She
helped facilitate 7E patient movement to 4N. This assistance allowed me to handle other codes, assist the 8E
staff, and assess the situation for the leadership team. I would also like to express my gratitude to Crawford Matthews and Kimberly Bloodsaw who were both working in the staffing office that night. They went above and beyond by helping transport the 7E patients to 4N. The staff of 7E handled this stressful event very well. I would like to
thank Reima Cambi, Greg Pelton, Lauren McCurry, and Richard Page. The staff of 4N were extremely supportive
and accommodating with the influx of patients. I would like to thank Joseph Djiega, Madolyn Matchet, and Raven
Nowden. I would like to thank Shawnte Hill Nurse Manager of Behavioral Health and the 8E staff: Gloria Greathouse,
Serena Harris, Rose Williams, Karla Robles, Jenia Thurman, Manythia Wilburn, Charles Watts, and Mark Funderward. This team did great making the best of the situation. Lastly, I would like to express my gratitude to the leadership team that came in to help with the Code Green: Polly Davenport, Amy Funderburk, Angie Longing, Chris Stines,
Kevin Cullinan, Deb Foster, Neil Gaiser (for turning off the water), and Paul Gaskin. Submitted by Angela Willis

Clinical Nurse Leader Role
Magnet Source
of Evidence
TL7
The Clinical Nurse Leader is an emerging nursing role developed by the American Association of Colleges of Nursing
(AACN) in collaboration with an array of
leaders from the practice environment.
Two AACN task forces were convened to
identify (1) how to improve the quality of
patient care and (2) how to best prepare
nurses with the competencies needed to
thrive in the current and future healthcare
system. CHI St. Vincent Infirmary is
proud to recognize Brittany Lewis MSN,
RN, CNL as one of only four Certified
Clinical Nurse Leaders in the state of Arkansas! Brittany obtained her certification on September 15, 2015. During her
third year of medical school, Brittany discovered her real passion: nursing. Brittany believes that her unique skill set will allow
her to provide, safe, cost-effective, evidenced-based care. Her goal is to improve patient outcomes by helping patients and their families put together “the big picture” of
health care. Congratulations Brittany!
Impella Comes to CHI SVI
Magnet Source
of Evidence
SE2EO
The Impella is an FDA
cleared, percutaneous ventricular assist device designed to provide a patient’s
heart with hemodynamics
support.
The catheter-based heart
pump is manufactured by
Abiomed, headquartered in
Danvers, Massachusetts.
The Impella is currently the world's smallest heart
pump. The whole device is less than the width of a
pencil, and the pump is tinier than an eraser. This
tiny device acts like a temporary artificial heart as it
maintains blood flow through the body during a
cardiac procedure. It works by pumping blood
throughout the body when the heart is too weak to
do it itself – providing time for the heart to rest and
heal. Roger Swayze, BSN, MBA/HCM, CCRC,
Nurse Manager CHI SVI_CVICU and CCSD was
instrumental in arranging the eLearning and Simulation Van to stop in Little Rock for training in our
healthcare community.

Focus on Safety
A fifty-six year-old female presented to the Emergency Department complaining of chest pain.
Safety Event
Shortly after arriving she was admitted to inpatient. The patient had previous admissions to this facility.
The home med list from her most recent stay four months earlier was carried over with no changes. The
home med list indicated that she took 20 mg Zanaflex (Tizandine) TID. The home medication reconciliation was completed by the attending physician with no changes.
Good Catch, but…
After admission to inpatient the attending nurse gave the patient 4mg of Zanaflex even though the order
called for 20mg, because she said she knew it was not right. However, the nurse did not investigate the
order further to confirm and/or correct dose. The following morning the next scheduled dose was administered, but only 4mg was given. The nurse charted that 16mg was given, but stated that she removed
the 5 tablets and she knew that it could not be right. Therefore, she put 4 of the tablets back in the drawer.
Error Continues
The next day a third dose of 20mg was administered. The nurse did not notice anything unusual about
the dose. Shortly thereafter, the patient’s blood pressure began to drop and the medication was discontinued.
Results
The error caused additional medications to be given and additional monitoring. The patient is now fine.
Reinforcement of the Error Prevention Tools and Safety Behaviors:
When completing task that are critical use STAR (Stop Act Think Review). Performing in the face of uncertainty makes you ten times more likely to make a mistake! Stop and Resolve; Review your plan, Resolve the concern, Reassess your actions – Stop when unsure and check with an expert source.
Challenge Questions
What happened when you were doing something and did not pause, did not stop to think about what you
were doing?
When has there been a time when something was not right, something seemed odd but you did not stop
to think it through and ensure your actions were the best?

New Knowledge, Innovations & Improvements
Perinatal Bereavement Program
Magnet Source
of Evidence
EP4
The Perinatal Bereavement Program held the annual Journey Memorial Service Sunday, Nov. 1 at CHI St.
Vincent Infirmary. The event brings together parents, family members and friends who have experienced
miscarriage, stillbirth or newborn death. Historically, these loses are seldom discussed or even acknowledged. The service provides support for families who are grieving and who can sometimes feel isolated in
their grief. An ornament is placed on a Christmas tree in honor of each child. This program was recognized on the CHI SVI Magnet document is an exemplar in practice.
Nurses Health Study

NK3
Join the next generation of the world's largest, longest-running study of health and wellness. Are you a female nurse or nursing student between 20 and 46 years old, who lives in the U.S. or Canada? If so, in the
Nurses’ Health Study 3 to contribute to groundbreaking research on “lifestyle, environment, nurses’ worklife,
and women’s health.” Researchers at Harvard School of Public Health and Brigham and Women’s Hospital
in Boston conduct the study, which builds on the two previous Nurses’ Health Studies.
To complete the study, please visit: http://nhs3.org/
What is New Knowledge, Innovations, & Improvements? Innovations are novel or new ways of doing things. Who do we use Evidence Based Practice and Nursing Research at the bedside? How
does technology impact practice? How do nurses18impact work flow and work space?

Student Project Update
St. Vincent Infirmary
St. Vincent Morrilton
Magnet Source

SE4EO
Are you a student and a project is part of your degree work? CHI St Vincent
has NEW Nursing Integration Practice & Project Guidelines that will become a
part of our student policy and approved through NEOC.
The student project should demonstrate their knowledge acquired through the educational program and
should align with the organizational and nursing goals. Students that are required to complete a final
project and plan to include CHI St. Vincent data must complete the following steps.
The student is required to contact the Director of Nursing Excellence & Education via email, to advise
CHI St. Vincent of the proposed clinical project.
Medication Safety
DID YOU
KNOW?...
~ Both oral and IV Amiodarone potentiate the
effects of warfarin and
can increase the INR.
~ Furosemide given too
rapidly IV push can
cause ototoxicity and
should therefore be administered over 1-2
minutes.
~ Famotadine IV needs to
locked in the refridgerator. It is a Joint Commission requirement that
any medicine that is not
patient specific (i.e. does
not have a patient label)
must be locked up either
in the medication pyxis
or the refrigerator. Since
IV famotadine requires
refridgeration, comes in
bulk supply and is not
assigned to a patient, it
must be in a locked container.
The Education Department will confirm a current Affiliation Agreement with the school.
The Education Department will confirm that the student has completed a hospital facility orientation if the
project requires a clinical component with patient or staff? (Research could involve staff instead of just
patients would just remove the whole sentence and state …completed facility orientation “as appropriate”.) involvement.
The student will be responsible for engaging an onsite supervisor (who will assign/approve the supervisor?) to act as a coach and a guide within the clinical environment and notify the Clinical Education Department. Who will then approve the supervisor??
The Site Supervisor will assist with the process through the organization. The Site Supervisor will facilitate contact with the Quality Department to assist with data collection. If there is a research or a quality
improvement component there will be a need to present to the Nursing Research Council and possibly
the Research Review Council.
The student will be asked to give a final presentation of the project. Internal dissemination of the project
will be decided on by the internal site supervisor and will also decide on who reviews the final presentation. The site supervisor is also responsible to ensure a final project report is provided to the NRC/RRC
as appropriate to close the project tracking.
The student will provide the hospital with a final copy of the project.
If the student is not approved or does not follow the Nursing Integration Practice & Project Guidelines the
student will not be able to proceed with the project and will not be endorsed by CHI St. Vincent for validation of data.
Any questions about this process should be referred to Lana Lambert MNSc, RN, APRN, BC, Director of
Nursing Excellence & Education at (501) 622-1834 or [email protected]
Alarm Fatigue in LEARN
Magnet Source
of Evidence
SE4EO
Repeated exposure to large numbers of clinical alarms creates an environment resulting in sensory overload. Alarm fatigue occurs due to the caregiver’s desensitization to the clinical alarms alerts. As a result, response times may be delayed, or alarms missed altogether. Development of strategies for alarm
management to reduce alarm fatigue and improve patient safety are critical.
In an effort to meet the NPSG for Clinical Alarms a LEARN module titled. “Alarm Management –ANCC”
will be assigned to all RNs, PCTs, and UCs. The completion date for this module is January 15, 2016.
