An Evening with Elder Clarke
This year we will be honored to hear from Elder Don Ray Clarke at our
Friday evening banquet.

In the 1960s, Elder Clarke served as a missionary in Argentina. He
earned a bachelor's degree from BYU and a MBA from Washington State
University. He began his career with the Ford Motor Company, but
spent most of his career as an executive in retail companies. He served
as Chairman of May Department Stores' Venture Operation, chairman
and CEO of Caldor Corporation, president of Lord and Taylor, and as a
volunteer professor of business at Southern Virginia University. He was
involved with Ascend Humanitarian Alliance in beginning microcredit
operations in Bolivia. He also served as an advisor on finances to
Rodney K. Smith, president of Southern Virginia University. Elder Clarke
has served in many Church callings, including president of the Bolivia
Santa Cruz Mission from 2001 to 2004. He was called to serve as a
General Authority in 2006. From 2007 to 2011, Elder Clarke served as a
counselor and then as president of the Church's Central America Area,
presiding at the groundbreaking ceremony for the Quetzaltenango
Guatemala Temple. Elder Clarke and his wife, Mary Ann Jackson, were
married in 1970 and are the parents of six children.

Root canal shaping was first done with hand files, then nickel titanium rotary files, followed by
reciprocating files and now a combination of rotary when you and want it and reciprocation

In 2007 and 2009, two new high-strength`, cementable, all-ceramic crowns were introduced
that have become the dominant class of restoration at the expense of the PFM. This course looks
at these two materials and compares their indications, limitations, fracture and placement
techniques.

Frank Higginbottom, DDS: "Digital Implant Dentistry: New Technology for Teeth and Implants."

Snoring and obstructive sleep apnea (OSA) can have a significant effect on relationships and a

Gordon J. Christensen, DDS, MSD, PhD
Founder, Academy of LDS Dentists
Gordon J. Christensen is founder and director of Practical Clinical
Courses (PCC) and chief executive officer of Clinicians Report
Foundation in Provo, Utah. He has presented over 45,000 hours of
continuing education throughout the world and published many articles
and books. Gordon and Dr. Rella Christensen are cofounders of the
non-profit Clinicians Report Foundation(previously CRA), which Rella
directed for many years. Since 1976, they have published the findings
to the profession in the well-known CRA Newsletter, now called
Clinicians Report(CR). Gordon is a practicing prosthodontist in Provo,
Utah and is a member of numerous professional organizations.
Steven J. Anderson
Over the last 25 years, Steve Anderson has entertained and educated
dentists and dental teams around the world. He founded the Total
Patient Service Institute, which provides dental teams with effective
management and people skills systems. He is a founder of the Crown
Council and founded Smiles for Life, which raised over $32 million for
children's charities. He is the author of five books, dozens of audio and
video learning programs, and hundreds of articles. He was named
Dental Businessman of the Year by Excellence in Dentistry.
Joseph D. Maggio, DDS, FACD, FACD
Joseph Maggio received a dental degree from Loyola University and
completed a residency in endodontics at Temple University. A former
faculty member at Temple, Loyola, and Northwestern Universities, he is
a guest lecturer at Loma Linda University. A past president of the
American Association of Endodontists, he received the Edgar D.
Coolidge Award from the American Association of Endodontists and the
Warren Wakii Award from the Japanese Endodontic Society. He recently
received the Gordon Christensen Award from the Chicago Dental
Society. Dr. Maggio is certified by the American Board of Endodontics
and a member of Omicron Kappa Upsilon. He is a fellow of the
American College of Dentists and the International College of Dentists.
Michael C. DiTolla, DDS, FAGD
Michael DiTolla practices in the largest dental lab in the US. As a result,
he has an intimate knowledge of the common habits of the dentists
getting the best restorative results. He has found techniques that give
him great restorative results with a very average set of hands. Dr.
DiTolla graduated from the University of the Pacific School of Dentistry
in 1988. He was awarded fellowship in the Academy of General
Dentistry in 1995. He is a graduate of the Las Vegas Institute of
Cosmetic Dentistry and a clinical evaluator for Clinicians Report. From
2001-2006, he was an instructor for PAC-Live's Live Patient Hands-on
Veneer Course. In 2001, he became director of clinical research and
education at Glidewell Laboratories.
Christopher A. Hooper, DDS

Dr. Hooper is a graduate of the Medical College of Virginia School of
Dentistry. He completed a residency at the University of Missouri at
Kansas City School of Dentistry. He is an associate clinical professor at
the Medical College of Virginia School of Dentistry. He provides
specialized education on cosmetic dentistry, functional esthetics, fullmouth prosthetic reconstruction and treatment planning options for the
difficult occlusal patient, as well as the demanding esthetic patient. He
maintains a private practice in Virginia Beach and lectures
internationally.
Frank Higginbottom, DDS
Frank L. Higginbottom is a 1971 graduate of Baylor College of
Dentistry, where he is a professor in the departments of Restorative
Sciences and Graduate Prosthodontics and Oral and Maxillofacial
Surgery. He has a private practice of esthetic, restorative and implant
dentistry at Baylor University Medical Center. He also serves as clinical
associate professor in the Department of Peridontics at the University
of Texas at San Antonio. He lectures nationally and internationally on
dental implants, restorative dentistry, provisional restorations, and
treatment planning. He is a member of the International Team for
Implantology, currently serving as Education Delegate for North
America.
Jonathan A. Parker, DDS
Dr. Parker is a graduate of the University of Minnesota School of
Dentistry. He has a full-time practice in dental sleep medicine in
Minneapolis, Minnesota. He is a diplomat of the American Board of
Dental Sleep Medicine and the American Board of Orofacial Pain. He has
written a number of research articles and abstracts evaluating the
effectiveness of oral appliance therapy for obstructive sleep
apnea(OSA). Dr. Parker also has lectured internationally on treatment of
snoring and OSA.
Bryan G. Judd, DDS
Dr. Bryan Judd is a graduate of the University of the Pacific School of
Dentistry. He is a Program Director in San Francisco and serves on the
Peer Review Committee of the Sacramento District Dental Society. Dr.
Judd is an established member of the American Academy of Cosmetic
Dentistry and has undertaken extensive training at both the Las Vegas
Institute and the Kois Center. He is also an active member of the
American Dental Association and California Dental Association. He
currently maintains a private aesthetic practice in Roseville, California.

At the conclusion of this conference, participants may receive up to 14 continuing education
credit hours for their professional certification or license renewal. The conference is AGD PACE
certified, so members of AGD may receive their credit through that organization by bringing

before you leave the conference.

Early conference check-in and walk-on* registration will be held Thursday, August 15, from
6:00 p.m. to 8:00 p.m. in 2297 BYU Conference Center.
For those checking in at this time, you will receive your name tag and conference materials.
*Note: Walk-on registrations are difficult and costly for the Academy and do not allow us time to
arrange for any of the meals, food breaks and conference materials. Please register and pay fees
by 5:00 p.m. Friday, August 9th. We will then be able to send you important follow–up
information before the conference and have materials ready for you when you arrive.

2013 Speakers
The Spouse Program is a unique suborganization for both men and women, whose
spouses are members of the Academy of LDS
Dentists. While your dentist obtains their
yearly accredidation, you get to enjoy a
program of your own—free of techniques in
fillings and drillings. This program provides an
opportunity to get to know other dental
spouses from around the country in between
presentations from top notch speakers.
This year's keynote will be provided by Ardeth
Kapp. Other speakers include Larry Barkdull,
Dr. Rella Christensen, Scott Marsh, Lowell B.
Anderson, and Kris Belcher.
As the theme suggests, the program this year
is packed with a variety of speakers and topics
that are sure to leave you wanting more. Of
the Spouse Program, one attendee wrote, "This
program is the 'best kept secret' of the annual
conference."

the Varsity Theater in the Wilkinson Student
Center (WSC), allowing you time to check the
rest of your family in at the Conference Center
as well as walk or drive over to the WSC.

Ardeth Kapp (Keynote)
Ardeth Greene Kapp was born and raised in Alberta, Canada. She
graduated from the University of Utah and earned a master's degree
from Brigham Young University, where she later was a member of the
College of Education faculty. From 1984 to 1992 she served as Young
Women general president. She has written 15 books published by
Deseret Book. She served with her husband when he was president of
the Canada Vancouver Mission. They later served as president and
matron of the Cardston Alberta Temple. She and her husband live in
Bountiful, Utah.
Larry Barkdull
Larry Barkdull's newest books are an eight-book series on Zion
(www.pillarsofzion.com) and Rescuing Wayward Children, a subject he
teaches at BYU Campus Education Week. He writes a weekly column for
the online Meridian Magazine. He is president of Gospel Ideals
International. (www.gospelideals.org), a foundation that promotes the
gospel on the internet. He is a longtime publisher and writer of books,
music, art and magazines. He and his wife Buffie have been married for
40 years, and live in Orem, Utah. They have 10 children and 15
grandchilden.
David Smith
David is a graduate of BYU in Media Music Studies. He currently teaches
private voice lessons at the Hale Center Theater Orem and is a member
of NATS (National Association of Teachers of Singing). He also writes
and records music for film and theater, including a children's
production of The Emperor's New Clothes. You can also check out David
Paul Smith's new solo album, For a Change, on iTunes and Facebook.
Some of his favorite credits include Caractacus Potts in Chitty Chitty
Bang Bang, Bobby Child in Crazy For You, Robert in The Drowsy
Chaperone, the Cat in the Hat in Seussical the Musical, Charlie in The
Foreigner, the Narrator in the premiere of The Girl, the Grouch, and the
Goat, Cosmo in Singin' in the Rain, Georg Nowack in She Loves Me,
Jimmy in Thoroughly Modern Millie, Lumiere in Beauty and the Beast,
Tony in West Side Story, and Smudge in Forever Plaid.
Rella Christensen
Dr. Rella Christensen leads TRAC Research, part of the non-profit
educational Clinicians Report Foundation, which she founded and
directed for 27 years. TRAC Research is devoted to oral microbiology
and dental restorative concepts. She has developed practice-based
controlled clinical protocals to study treatment and outcomes. She
recieved a BS degree in dental hygiene from the University of Southern
California School of Dentistry and practiced dental hygiene for 25
years. She founded and directed the expanded function dental hygiene
program at the University of Colorado School of Dentistry. She earned a
PhD in physiology with an emphasis in microbiology from BYU and
completed postgraduate training at Virginia Polytechnic Institute and
State University. She has taught at the undergraduate and postgraduate
levels, written many research abstracts and reports, and received

numerous honors. She married Dr. Gordon Christensen 53 years ago
and they have three children.
Scott Marsh
Scott C. Marsh is a part-time faculty member at the BYU Department of
Finance in the Marriott School of Management. He is an author,
consultant and owner of the Scott Marsh Education Institute and Scott
Marsh Financial. He has MBA and MSFS degrees in taxation,
investments, and economics. He received an MS in financial services
and the Chartered Life Underwriter and Chartered Financial Consultant
designations.
He has taught personal finance at BYU for faculty and staff since 1995,
at Campus Education Week since 1991 and to CES employees. Scott has
written three textbooks in financial services and is currently writing two
more books. He is married to the former Mary Bunker and they have
five sons.
Kristi Rogers
Kristi Rogers lives in Orem, Utah, where her husband has a dental
practice. Kristi is a former physical education teacher. She is an
interesting speaker with a passion for serving and helping others. She
and her husband do a lot of humanitarian dental service around the
world. While her husband is doing dental work, Kristi is visiting
orphanages, etc. Not only does she serve others, she exemplifies
someone who loves others and lives "after the manner of happiness."
Lowell Anderson
Born in Fillmore, Utah, Lowell Anderson graduated from Spanish Fork
High School. He received a bachelor's degree in zoology and physical
education from Southern Utah State College, then a master's in the
analysis of human motion from BYU. He and his wife Evelyn have been
married for 38 years and live in Spanish Fork, Utah. They have five
children and 15 grandchildren. Lowell has worked for the Church
Educational System for 36 years in Utah and Arizona. He is currently an
instructor at the Orem (Utah) Institute of Religion.
Rebecca Moller
Rebecca Moller was born and raised in Wisconsin. In 2004 she
graduated from BYU with a bachelor's degree in English education. She
taught English at Spanish Fork Middle School for two years. After
starting her family, she certified in group exercise, Pilates, Zumba, and
Zumbatone. She has been teaching group exercise classes at UVU and
BYU for seven years. She certified in Zumba when she was eight months
pregnant with her third child, and she is now the mother of four. She
lives in Spanish Fork, Utah.
Kris Belcher
Courage, perseverance, faith and humor are the themes of Kris
Belcher's life. Left completely blind and emotionally shattered in 2003
from her second battle with cancer, Kris was unsure how to go on. She
has since grown accustomed to life in the dark, but could do without
the bumps and bruises that accompany blindness. She lived in Northern
California until she attended BYU and graduated in therapeutic
recreation.
She and her husband, James, have two boys and live in Lehi, Utah. She
has published two books, Hard Times and Holy Places, and Facing
the Son, as well as a talk on CD called "But Now I See." She has shared
her story and laughed with audiences throughout the United States and
Canada.

Why is the spouse program at the Wilkinson Center this year?

Because The Academy is growing, they needed more space in the Conference Center, which
means that the Spouse Program had to be moved to another building, most preferably, the
Wilkinson Center.
Full-Price registration
Aug. 10-16.........................$109
The dentists' main reason for attending is to obtain their yearly accreditation. For most dentists,
they need to attend sessions all day to meet their requirements. If your spouse only needs a few
credits, they are more than welcome to attend a session with you, just remember that you'll be in
a different building this year.
Why can't we have a session with our dentist?
Why are we starting later this year (starting at 8:30 a.m. instead of 8:00 a.m.)?
The spouse program is starting a half hour later than the dentist and youth programs in order to
allow you enough time to get the rest of your family checked in and then make your way to the
Wilkinson Center.
Why are we ending early on Saturday (ending at 3:30 p.m. instead of 5:00 p.m.)?
Traditionally, in the past, we have taken a 1½ hour break in the middle of the day on Saturday to
allow for shopping, campus tours, etc. As per some feedback we received, we decided to move
this block of time to the end of the day and end early instead. Still allowing some people time to
shop at the BYU Bookstore and allowing others time to do things off campus before their dentist
or children are done. Please note that only the Spouse Program is ending at 3:30 p.m.
Why do we hold spouse elections every year?
In 2011, the duration of the Spouse Program committee positions changed from 2 years to 1
year in each position; which means that there will be a new president each year, also meaning
that each year a new vice president along with a new historian will need to be elected.
Why don't you have a nursery program, for children ages 1-5?
There has not been enough interest regarding a program for children of these ages. Should the
need be expressed by enough people, a nursery program would be considered. In the meantime,
using the babysitting services of friends or family in the Provo area is suggested.
Can we bring babies to the conference?
Yes, a mother's lounge is provided. There is a sound feed into this room, to make sure you don't
miss out on anything!

Fun in the Sun!
Youth ages 6 to 18 will have the opportunity to make new friends and enjoy friendships from
previous conferences at this year's Academy of LDS Dentists Youth Program. Two action-packed
days are planned at some of Utah's most exciting attractions, as well as new offerings especially
for this year for the 12 to 18 year old group.
We hope all 6 to 11 year olds will join us for great adventures at Thanksgiving Point's farm
country, petting zoo, hayride, dinosaur museum, and 3-D movie; the Boondocks Fun Center,
and a magician. Meanwhile 12 to 18 year olds get to scale climbing walls and a ropes course
before going to Lagoon. The second day of the program, youth get to enjoy water games and a
day at Seven Peaks Water Park with its giant half-pipe tube ride, 16 water slides, 500,000-gallon
wave pool, 100 feet of free-fall drop slides, lazy river and more!
