GENERAL INFORMATION
Where does the Conference take place?
The MVC takes place in Columbus, Ohio, in both the
Greater Columbus Convention Center (350 N. High St.,
Columbus OH 43215) and the Hyatt Regency (400 N. High
St., Columbus OH 43215). Registration and Customer
Service are located in Hall C of the Convention Center.
Who is eligible to attend the MVC?
The OVMA welcomes individuals working in all areas
of the veterinary profession, including veterinarians;
technicians; hospital staff; shelter personnel; postgraduate residents and interns; and pre-veterinary,
veterinary, and technician students. Non-veterinary
professionals age 16 and up may attend as guests of a
registered attendee.
I am a veterinarian in Ohio, but I am not an OVMA
member. Can I attend?
Non-member veterinarians in Ohio are most welcome to
attend, but please note that your registration fee will be
higher than that of OVMA members. If you would like to
receive the member price, we invite you to join the OVMA
by filling out the “Member Dues” section on the registration
form. Not only will you enjoy the cost savings, but also
the benefits of OVMA membership throughout the year!

I am a veterinarian practicing in another state.
What fee will I pay to attend?
As an out-of-state DVM, you are eligible to take advantage
of the same reduced registration fees that an OVMA
member would pay. Please see the registration form on
the following page for these fees.
Who can register for a spouse/guest badge?
A spouse, companion, relative, friend or child age 16 or
older may register as a guest of a registered attendee.
DVMs, technicians, hospital staff members, and other
animal care professionals are not eligible to attend as
guests. Please note that CE credit confirmation cannot
be issued for this type of registration.
How many hours of continuing education can I
obtain by attending the Conference?
You can earn a total of 24 hours of CE if you elect to
attend the full Conference and attend all day each of
the four days. A total of 6 hours of CE can be obtained if
you elect to attend only one day of the Conference and
attend a session during each time slot offered.
I am traveling from out of town. How can I make
housing arrangements?
The OVMA has reserved room blocks at several hotels
within walking distance of the Convention Center. Rates,
contact numbers, how to book, and other information
about each hotel can be found on page 74 or by
visiting the Travel & Hotels section of the Web site at
www.mvcinfo.org/attendees, where you can also find
transportation and parking information.

How can I register to attend?
Register online at www.mvcinfo.org or fill out a form for
each attendee and submit it to the OVMA by mail (3168
Riverside Drive, Columbus, OH 43221) or fax (614-486-1325).
What are the registration deadlines?
Jan. 6, 2012 – Deadline for early bird registration to
receive discounted prices.
• Feb. 1, 2012 – Deadline to register and receive a
confirmation packet in the mail. If you register after
this date, please pick up your packet on site.
• Feb. 10, 2012 – Pre-registration deadline. After this
date, please register on site.

How can I pay for my registration fees?

The OVMA accepts checks, Visa, Mastercard and Discover
as forms of payment. Unfortunately, we are unable to
accept American Express. Please note that fees need to
be paid in full at the time of registration.
What if I have to cancel my registration?

What should I know about registering for wet labs
and workshops?
The registration form includes a section where you
may select which of these programs you would like to
attend. Wet labs and workshops require an additional
fee, but because these prices are fixed, there is no early
registration discount. Please note that participation in
these programs may be limited and is awarded on a
first-come, first-serve basis; as such, we advise you to
sign up as early as possible.
Am I able to register multiple people from our
clinic at one time?
Yes. If you are registering by fax or by mail, please make
a copy of the registration form or download a PDF online
at www.mvcinfo.org and complete a separate form for
each individual. If you are registering online, please
follow the instructions provided on the Web site to add
additional attendees.
How does a Hospital Pass work?
A Hospital Pass may be used by the staff of a veterinary
facility (excluding veterinarians and technicians). In
your confirmation packet, you will receive four tickets,
each of which is good for one attendee on any one
day. You may use these passes on any combination of
days you like. When the attendee or attendees arrive
at the Conference, each should bring one ticket to
the Customer Service booth in the Registration Area
to receive a badge. The pass will be listed under the
company name.
Can I and/or my guest purchase a pass just for
the Exhibit Hall?
Yes. You may purchase a one-day Exhibit Hall-only badge
for a nominal fee of $20 per person, or a full three-day
pass for $40 per person. Children under 18 are admitted
at no charge, but should still obtain a badge, as only
attendees with badges will be admitted to the Exhibit
Hall. Anyone under 18 must be accompanied by a
parent or legal guardian at all times. You may purchase
these badges at the Customer Service booth during
Registration hours.
I will need special assistance to be able to attend
the Conference. How do I make arrangements?
Attendees who need special assistance or services are
asked to notify the OVMA office no later than Feb. 10,
2012, by providing a written description of how we may
accommodate you. There is space available for this
If personal or professional circumstances prevent you
from attending the Conference, the OVMA will refund
your registration fees less a $50 administrative fee,
provided that a written refund request is received by 6
p.m. EST on March 14, 2012. No refunds will be granted
after this date. No refunds can be granted for wet labs,
workshops and entertainment functions. Written refund
requests may be delivered to the OVMA via mail, fax or
e-mail. Please note that refunds will not be processed
until March 15, 2012.
Can a registration be transferred?
If you are no longer able to attend, your registration
may be transferred to another person on your staff.
Please bring the original badge and registration
materials to the on-site registration desk to receive a
new name badge.
CONFIRMATION
When will I get my name badge and tickets?
Attendees who register by Feb. 1, 2012, will receive
mailed confirmation including name badges, any
purchased tickets, and instructions for accessing online
Conference Proceedings. These items will be mailed to
pre-registered attendees beginning the week of Feb. 13,
2012. Please be sure to bring these materials with you to
the Conference, as there is a $10 replacement fee for lost
name badges.
Will I be able to access and print notes for my
sessions prior to the Conference?
Yes. Instructions for accessing the Conference

Hello Midwest Veterinary Conference Parents!
Thank you very much for your interest in the Midwest Veterinary Conference children’s program. Our goal is to provide your
children with a program they want to attend, while providing you with that critical “peace of mind” feeling so you can attend
event activities.
KiddieCorp is pleased to provide a children’s program during the Midwest Veterinary Conference. KiddieCorp is in its twentysixth year of providing high quality children’s programs and youth services to conventions, trade shows and special events. We
take watching your children very seriously. KiddieCorp has enjoyed a long-time partnership with the American Academy of
Pediatrics, which has helped to establish KiddieCorp as a premier provider of event children’s program services.
ACTIVITIES
Activities include exciting themes, arts & crafts, group games, music & movement, board games, story time, dramatic play, etc.
We provide activities appropriate for each age group, using safe, sturdy equipment that you can feel comfortable with.
Children can make their own choices within KiddieCorp’s program.
THEME – Wild, Wild West Howdy partners! Let’s get saddled up for one wild ride. Our adventure will begin when we
dress the part. We will have a variety of Cowboy hats, scarfs, and sheriff badges. Yee Haw! All cowboys and cowgirls will
learn how to master the rope like a real wrangler when we all do a little jump roping. It’s a guaranteed good time for
everyone! Next, we’ll mosey on over to the arts and crafts table to make collages, bead creations, and wild west coloring
sheets No cowboy or cowgirl’s day would be complete without an obstacle course to test their skills and relay races to keep
them laughing. Then, we’ll wind down our long day on the range by sitting around the imagination campfire to tell our favorite
stories. So partners, saddle up and get ready to have the best time ever while we discover how the West was explored!
COMMITMENT
Our goal is to provide your children with a comfortable, safe and happy experience. Our staff to child ratios are high to
ensure that every child feels special (1:1 for children 6 weeks through 5 months; 1:2 for children ages 6 months through 11
months old; 1:3 for children ages 1 through 2 years old; 1:5 for children ages 3 through 12 years old). KiddieCorp team
members are selected according to their integrity, experience, education and enthusiasm. They must be wonderful with kids!
You will feel extra secure knowing that the KiddieCorp team is bonded and that we carry ample liability insurance.
WHERE, WHEN, FOR WHOM
The program is for children ages 6 weeks through 12 years old. The dates for the program are February 23-26, 2012 and will
be located at the Hyatt Regency in Columbus, Ohio. Snacks and beverages will be provided and meals need to be supplied by
parents or purchased when checking in your child each day. Please note, purchased meals will be from a menu supplied by the
Hyatt Regency.
REGISTRATION
See the attached registration and consent form for event information. The advance registration deadline is January 27,
2012. Register early as availability is limited and handled on a first-come, first-served basis. KiddieCorp must receive both
the registration/consent form and payment in full to hold reservations. Although every effort will be made to accommodate onsite registrations, there is no guarantee and it is not recommended.

We, the undersigned adults, agree to place our child or ward in the KiddieCorp children’s program.
For ourselves, our child/ward (or children/wards), and each of our respective heirs, assigns, and
next of kin, we hereby release and agree to indemnify and hold harmless KiddieCorp, Ohio Veterinary
Medical Association, the Midwest Veterinary Conference and their respective officers, directors, agents,
employees, assigns, vendors, and the owners and/or lessors of the facility or facilities where the program
will be held (collectively “the Releasees”), from any and all claims which may now or hereafter arise from
our child’s/ward’s (or children’s/ward’s) participation in the KiddieCorp program. We do not release claims
arising from Releasees from any of their willful misconduct or gross negligence.
We have read the above and understand this release. Furthermore, in the event of an emergency
or health concern, KiddieCorp has our permission to administer first aid, contact our pediatrician,
or obtain emergency medical treatment for our child. We agree to pay all expenses incurred due
to an emergency involving our child.
