individualized services and
outcome data in accordance
with the affiliate’s uniform
electronic data management
system. At a minimum, this
includes:
a. Services or resources
provided, including dates,
providers and duration;
b. Relevant outcome data, as
defined in individualized plans,
in appropriate intervals (e.g.,
grading periods).
