What’s New for February 2016:
Supplemental Benefit elections will become effective
2/1/2016 (elections requiring evidence of insurability, such as
life Insurance, may have a later effective date, if approved).
After annual enrollment closes, benefit changes can only be
made if you experience a qualifying event (and changes must
be made within 30 days of event).
Medical annual enrollment is done during the summer for 9/1.
For mid year changes, see your benefit administrator within
31 days of status change. For complete TRS medical

Reminder Health Savings Accounts for employees enrolled in
ActiveCare 1HD with HSA Bank. Funds roll over every year
(but are not available in advance). You are not eligible if you
are also enrolled in the Medlink plan, medical flex spending or
have access to medical flex funds thru spouse or rollover.
NEW Guaranteed Issue Critical Illness Insurance from VOYA pays a
lump sum benefit if diagnosed with a covered illness or condition
like heart attack, stroke, coma, organ failure or paralysis. No limits
on Pre-Existing conditions! Medicare/Medicaid insureds ineligible.
NEW Family Protection-Terminal Illness Plan with Quality of Life
Rider from 5Star Life is individual term life insurance protection that
pays a lump sum advance benefit on terminal illness diagnosis and
pays a monthly benefit if Long Term Care becomes necessary. Can
be purchased for spouse, children and grandchildren thru 23.
CHANGE Flexible Spending Accounts: Limit increased to $2550/yr.
Current participants in Medical or Dependent Care Flex MUST reelect new contribution amount each year to continue to participate.
Please KEEP your debit card! Your card will be reloaded in MidFebruary. Current participant in a Healthcare FSA (flexible spending
account) must spend current funds by the deadline grace period of
4/15/16 to avoid forfeiture. Login thru the Benefit Website (CHECK
FSA) or use the smart phone app to view balances and card swipes.
New participants enrolling in a HealthCare FSA will receive medical
flex debit card in unmarked envelope from NBS in late February.
Funds are available thru electronic or paper claim mid February.

$1000 age 55+ catchup.
MEDlink Insurance - American Public Life (APL)
This supplemental coverage helps offset out-of-pocket costs due to deductible & coinsurance. The available plan options include $1,500
or $2,500 inpatient hospital benefit option, $200 outpatient benefit and $25 Physician Outpatient Benefit. You are not eligible for
MEDlink if any of the following apply: employees (or dependents) not covered on PBTISD’s medical plan, anyone covered by TRS-Care
(retiree plan), Medicare, Medicaid, or Medical Savings Accounts, employees who have an HSA that is being actively funded, nonresidents of the US, employees not actively at work on the plan effective date.
Telehealth - MDLIVE
This Employer Paid plan provides you and your family with around-the-clock access to U.S-based, licensed physicians for free telephone
consultations. It’s easy to connect with a doctor in real-time for treatment or diagnosis of common non-emergency conditions.
Dental Insurance - CIGNA PPO
In-network benefits are 100% for preventive, 80% for Basic, 50% for Major services and 50% Orthodontia with a Lifetime max of $1,000
for dependent children to age 19. Out-of-Network charges are paid based on usual reasonable & customary fees and usually involve
greater out of pocket expense. $50 deductible for Basic and Major services per person, to a max of $150. Calendar year plan max per
insured: Year 1 -$1,250; Year 2 - $1,350; Year 3 -$1,450 and Year 4 -$1550. Participants with certain medical conditions eligible for extra
free services, register in advance for CIGNA Dental Oral Health Integration Program, see registration form at PBTISD Benefit website.
Vision Insurance - Superior Vision
Members pay a copay for in-network benefits. Out-of-network vision services are reimbursed up to a certain dollar amount for covered
expenses. The in-network exam co-pay is $10.00, the materials copay is $25.00 and the contact lenses fitting exam copay is $25.00.
Exams, lenses & frames (within plan allowance) are covered in-network with a co-pay, once every 12 months.
Long-Term Disability Insurance - UNUM
Provides a monthly income to an employee disabled due to an accident or illness. There are different plans available with benefits
available from the 1st day of disability to as late as the 180th day. Benefits can be payable to age 65 if disability occurs prior to age 65.
All new or increases in coverage are subject to pre-existing condition exclusions.
Cancer Insurance - Loyal American
Cancer insurance is designed to be a supplement & pays in addition to your medical insurance. There are 3 plan options available, with
Guaranteed Issue and no medical questions, portable. Some plan benefit are: cancer screening tests, first occurrence, chemotherapy/
radiation and surgical benefits. All new or increases in coverage are subject to pre-existing condition exclusions.
Accident Insurance -American Public Life (APL)
This Plan pays an ambulance, hospital and physician expense benefit benefit amount for covered accidental injuries. Coverage is issuable for ages 18-64 and is portable, you can choose to keep your benefit even if you leave the district or retire until age 70.
Voluntary Group Term Life & AD&D Insurance - The Hartford
Employees can enroll up to max of 5x salary not to exceed $500,000 and $250,000 for spouse and $10,000 on each eligible dependent
child under 26. Employees must enroll in order to enroll dependents. One step up to Guaranteed Issue with no application required.
Increases of more than of 10k for employee and 5k for spouse are subject to health questions & underwriting. New employees can
enroll for up to $150,000, $50,000 spouse and $10,000 children on Guarantee Issue Basis within 31 days of hire. Employees must be
actively at work and spouse/child cannot be disabled for new or increases in coverage to take effect.
Universal Life with Long Term Care Insurance - Trustmark
Permanent Life Insurance will be grandfathered, no new enrollees.
Heart & Stroke Insurance - Allstate
Heart & Stroke Insurance will be grandfathered, no new enrollees.
Healthcare & Dependent Care Flexible Spending Account by National Benefit Services
Tax-sheltered flexible spending accounts allow an individual to set aside dollars to pay for future healthcare & dependent care
expenses. Eligible expenses must be incurred within the plan year 2/1/2016—1/31/2017 & 75 day grace period. Contributions are useit-or- lose-it. The healthcare reimbursement maximum is increasing to $2,550/plan year. The dependent care reimbursement maximum is $5,000 married or $2,500 for married individuals filing separately. Remember to keep all your receipts! Do not throw away card!
