This is the first edition of the Community Resource Guide. It is designed to help area patients
and members of the community locate resources in Elizabethtown, Kentucky and its surrounding areas. The Guide is a directory of not-for-profit and private for-profit resources to
aid individuals with contact information for services available throughout the community. Designed and maintained by Hardin Memorial Hospital staff, the guide seeks to provide services
available, descriptions, locations, and phone numbers.

Accepts self payment and private health insurances. Specializes in persons with co-occurring
mental and substance abuse disorders, DUI/DWI offenders, criminal justices clients. Formed in
September 2001, AA & Associates began with an office in Louisville and later expanded to
having offices in LaGrange, Carrollton, and Radcliff. AA & Associates has served thousands of

Kentucky Prescription Assistance Program
This program assists the public with accessing prescription drug programs and free medications that are offered by discount drug programs, the pharmaceutical manufacturers, and also
those from various discount pharmacy programs. These prescription assistance programs provide medicines and prescriptions for free or at greatly discounted costs for both low income
individuals and qualifying families.

Kentucky Transitional Assistance Program
Families can receive temporary cash assistance and grants to help pay bills and debt. This program will also provide assistance with finding a job to participants. Also, find help getting the
job training and career development that leads to that new or higher paying job.

Kentucky Children’s Health Insurance Program (KCHIP)
This will provide health insurance for uninsured children who are income ineligible for Medicaid. KCHIP will cover eligible children until their 19th birthday. The government insurance can

Administers Sharing the Warmth energy bill assistance. The program is funded by donations,
and it can help people pay their monthly gas or utility bills. All the funding from the program
will go to local people in need that meet criteria established by the guidelines of the program.

A weatherization/energy saving program is offered for low income and other eligible households. The program can provide them with a heat pump and/or furnace, energy efficient light
bulbls, a heat pump clean and tune up, hot water heater wrap, as well as energy saving tips

offered. Depending on the condition of the home or apartments, and its total energy usage,

Help is made available to eligible customers, and they can receive up to a credit of $294 per
year in seven monthly installments of $42 that are applied to their current monthly utility bill.

Communicare—Early Intervention Program (EIP)
The Early Intervention Programs in Kentucky serve youth through collaboration
with the Court Designated Worker Program and through court referrals. Youth, age 12-17,
who have drug or alcohol related offenses are eligible for services. A variety of tools are used
to place each youth in the appropriate prevention education curriculum, or to refer them for
further assessment. If a youth demonstrates the need for education, those services will be
provided by the Early Intervention Specialist. Youth needing treatment for chemical dependency will be given appropriate referral options, which may include clinical assessment and

Facilities include four Youth Softball Fields and restrooms/concession building. Activities include the Elizabethtown Youth Softball Program for ages 5-7, 8-10, 11-13, 14-17 (Open to
