HERITAGE VALLEY HEALTH SYSTEM
Heritage Valley Health System is an integrated delivery network
providing comprehensive health care for residents of Allegheny,
Beaver, Butler and Lawrence counties, in Pennsylvania; eastern Ohio;
and the panhandle of West Virginia. In partnership with 3,700
employees and more than 480 physicians, Heritage Valley offers a
broad range of medical, surgical and diagnostic services at its
two hospitals, Heritage Valley Sewickley and Heritage Valley Beaver;
in 60 physician offices; and 18 community satellite facilities.

AND THE PRESIDENT / CEO
The health care industry in western Pennsylvania and across the entire nation
continues to experience an unprecedented transformation, bringing forth
challenges as well as opportunities for hospitals and health systems. The primary
goal of Heritage Valley Health System’s Board of Directors, senior leadership,
physicians and employees is to remain at the forefront of this rapidly changing
environment, in order to confront and develop solutions to the challenges while
taking advantage of opportunities that arise.
To successfully accomplish our mission “to improve the health and well-being of
all people in the communities we serve,” Heritage Valley has developed strong
connections to advance our strategic imperatives: Quality, Safety, and Customer
Experience; Human Resources; Information Technology; Market Expansion and
Community Health; and Fiscal Responsibility.
Heritage Valley Health System realized many achievements in fiscal year 2015.
Several areas of the Health System were recognized by national organizations for
delivering quality care as a result of dedicated work by Heritage Valley’s
physicians and employees.
Our long-standing commitment to information technology continued to evolve in
areas such as Healthlink Patient Portal, Clinical Connect Health Information
Exchange and our most recent foray through our iHVHS and aHVHS mobile Apps.
Through innovations like these, information technology can result in efficiencies
and improvements in the delivery of quality patient care. Heritage Valley also made
strides in growth and expansion through a number of partnerships and joint
ventures as well as additional community based locations and services through
our Medical Neighborhoods.

approach our mission “to improve the health and well-being
of all people in the communities we serve.” The Board of Directors,
physicians and employees take the strategic imperatives endorsed
by the Board — Quality, Safety and Customer Experience; Human
Resources; Information Technology; Market Expansion and
Community Health; and Fiscal Responsibility — and translate
those ideals into action. While the delivery of health care
continues to be ever-changing and ever-challenging, the Health
System made great strides with each of these strategic imperatives
to further our connection with patients, employees, other health
care organizations and the community.

With Quality.
Quality has become increasingly important in the changing health care industry. At Heritage Valley, physicians and employees
strive to improve and ensure that our patients receive the highest quality health care. Heritage Valley Sewickley was recognized
as part of The Joint Commission’s 2014 annual report “America’s Hospitals: Improving Quality and Safety.” The Sewickley campus
was one of 1,224 hospitals in the United States to achieve Top Performer distinction for attaining and sustaining excellence in
performance for heart attack, heart failure, pneumonia and surgical care. In the past year, the Health System received recognition
of quality health care delivery in several additional areas.
• Heritage Valley Beaver and Heritage Valley Sewickley each received the Get With The Guidelines®
- Stroke Silver-Plus Quality Achievement Award for implementing specific quality improvement measures
outlined by the American Heart Association/American Stroke Association for the treatment of stroke patients.
• Heritage Valley Beaver and Heritage Valley Sewickley were named Screening Centers of Excellence by the Lung
Cancer Alliance (LCA) for their ongoing commitment to responsible lung cancer screening.
• Heritage Valley WoundCare and Hyperbaric Medicine received accreditation by the Undersea and Hyperbaric
Medical Society (UHMS) for demonstrating commitment to patient care and facility safety in clinical
hyperbaric medicine.
• Heritage Valley Women’s Health Center is designated as a Breast Imaging Center of Excellence by the
American College of Radiology (ACR).
• Heritage Valley Sewickley is a Bariatric Surgery Center of Excellence (BSCOE) as designated by the American Society for
Metabolic and Bariatric Surgery (ASMBS). The accreditation formally acknowledges Heritage Valley Sewickley’s
commitment to providing quality improvement and patient safety for bariatric surgery patients.
Beyond the recognition that Heritage Valley received for specific service lines, the Health System is committed to performance
improvement and transparency. Nationally the bar is set high to achieve clinical outcomes and Heritage Valley continues to
improve on clinical and customer service measures. Additionally, Heritage Valley has voluntarily worked with the Leapfrog Group,
a private non-profit dedicated to performance transparency and benchmarking among organizations.

Heritage Valley Health System is a community health system and remains committed to providing quality care to the people
that live and work in southwestern Pennsylvania. Heritage Valley’s physicians and employees are connected to the community
in a very personal way. An overwhelming majority of staff live in our service area. When they come to work they are not just
caring for patients; they’re caring for their friends, family and neighbors. That personal connection and commitment to
remaining a community health system is what sets Heritage Valley apart from the competition.
Heritage Valley Health System received a 2014 Achievement Award from the Hospital and Healthsystem Association of
Pennsylvania (HAP) for Workplace Innovation. The Health System was chosen for implementing an employee pay for value
program. The program places emphasis on aligning clinical and customer service performance with the new methodology of
reimbursement being used by the federal government and other payors. Additionally, Heritage Valley was recognized as one
of southwestern Pennsylvania’s “Healthiest Employers” by the Pittsburgh Business Times.
The great care and compassion of Heritage Valley’s employees is personified in the Cameos of Caring award winners for
2014, Pauline Hutsler, BSN, RN, CCRN, Critical Care Unit/Progressive Care Unit at Heritage Valley Sewickley and Scott
Rodenbeck, BSN, RN, Cardiopulmonary Unit at Heritage Valley Beaver. The Cameos of Caring Award honors exceptional
bedside nurses who work in acute care hospitals.
Heritage Valley Health System is the largest employer in Beaver County with more than 3,700 employees. Each year employees are recognized for
their commitment to Heritage Valley at an Employee Recognition Dinner. The employees above are just a few of whom reached a significant tenure

With Technology.
Heritage Valley’s Board of Directors has long been committed to investing in technology as a facilitator of improving health
care delivery. Increased efficiency, accessibility and improved care management that occurred from investments in technology
infrastructure have been realized at Heritage Valley. As such, the Health System received recognition and reached
milestones related to information technology.
• Heritage Valley Health System was one of only three Pittsburgh-area hospitals
to be named “Most Wired,” in 2014 and 2015 according to a survey published
by the American Hospital Association in the July issue of Hospitals & Health
Networks (H&HN) magazine.
• Heritage Valley was ranked 35 on the 2014 IMS Health Top 100 Integrated
Delivery Networks (IDNs) list, which recognizes the way that integrated delivery
networks are reshaping the way care is provided and treatment decisions
are made.
The Health System has launched a number of initiatives that aim at keeping physicians,
employees and patients connected. The electronic medical record connects the services and
physicians of Heritage Valley in order to streamline orders and increase access of clinical
information. ClinicalConnect, the health information exchange in which Heritage Valley
participates, enables patient information to be shared among Heritage Valley and other hospitals
and health systems in the region.
The Health System’s information technology department has strengthened the connection that
individuals have with their personal health information. Heritage Valley Health Link, powered by
FollowMyHealth, is a secure online patient portal that enables patients to conveniently and
securely review their health information online. More than 18,500 patients utilized Health Link in
fiscal year 2015. In a related action, Heritage Valley re-launched its App on Apple and Android
platforms. The HVHS App was accessed nearly 74,000 times.

With Community.
A strong connection with the community has been a driving force at Heritage Valley Health System. Continually, the
Board of Directors has sought to determine the most important health care needs and provide services that address
those needs in the communities Heritage Valley serves. Two such strategies are the development of medical
neighborhoods as well as retail medicine walk-in clinics.
In April 2015 Heritage Valley completed an expansion and renovation of current services in Imperial, PA resulting
in the creation of its sixth Medical Neighborhood location, Heritage Valley West Allegheny. Like other Heritage
Valley Medical Neighborhoods in Chippewa, Edgeworth, Ellwood City, Moon Township and Robinson Township,
the site offers a variety of health care services in one convenient location.
Heritage Valley opened a ConvenientCare walk-in clinic in the West Allegheny medical neighborhood. This represents
Heritage Valley’s sixth ConvenientCare location. Other ConvenientCare locations include Chippewa, Edgeworth,
Ellwood City, Monaca and East Liverpool, Ohio. Heritage Valley’s ConvenientCare locations were the home of more
than 36,500 patient visits in fiscal year 2015.
Along with expansion of locations in the community, Heritage Valley is closely connected with its host of Community
Health Services programs. In fiscal year 2015, the department participated in 72 outreach events and touched
the lives of 4,371 individuals through its disease management programs that are free or covered by insurance.
Heritage Valley West Allegheny and Heritage Valley Robinson Township are two of the Health System’s Medical Neighborhoods. Each
location offers primary care, lab draw and radiology services. Additionally, the West Allegheny location offers a ConvenientCare walk-in
clinic, physical rehab and behavioral health services.

With Partnerships.
At Heritage Valley we continue to expand our services to provide better care for patients in our communities. In doing so,
we’ve taken a unique approach by partnering with other health care organizations that offer services that
complement those of Heritage Valley. By securing an ownership interest in the complementary services, such as those listed
below, Heritage Valley is better positioned to provide a full continuum of care to support our core mission.
• Home Infusion Therapy. Heritage Valley entered into a partnership with Chartwell Pennsylvania LP, Butler Health
System, Washington Health System, Great Lakes Home Healthcare and UPMC Community Provider Services to offer
home infusion therapy and specialty pharmacy services in the tri-state area.
• Medical Equipment. Heritage Valley is a partner in MedCare, a Home Medical Equipment (HME) and Medical Supply
Company that furnishes supplies and services to patients in western Pennsylvania, eastern Ohio, and northern
West Virginia.

• Senior Living. In collaboration with Lutheran SeniorLife, Heritage Valley supports the LIFE Beaver County and LIFE
Lawrence County programs. Living Independence For the Elderly (LIFE) provides an alternative to nursing home care
by helping to restore and support independence for older individuals who wish to remain in their homes.

• Health Information Technology. ClinicalConnect is a health information exchange (HIE) facilitated by collaboration with
ten area hospitals and health systems, including Heritage Valley, and UPMC. The HIE provides timely access to vital
patient information leading to more informed clinical decisions and reductions in repeat testing.
Additionally, in 2014 Heritage Valley Health System, Duquesne University’s John G. Rangos Sr. School of Health Sciences and
Mylan School of Pharmacy solidified a student affiliation that promotes collaboration in clinical care, research and medical
education in the clinical areas of pharmacy, physical therapy, occupational therapy, and speech therapy.

Heritage Valley Sewickley Foundation and Heritage Valley Beaver Foundation are committed to raising funds to
support the Mission, Vision, and Values of Heritage Valley Health System and its hospitals, Heritage Valley Sewickley
and Heritage Valley Beaver. The Foundations, in conjunction with Heritage Valley Health System and hospital
administration, identify major funding needs for the Sewickley and Beaver campuses, design their fundraising programs
toward those needs and raise funds from key constituents that choose the hospitals as a “charity of choice.”

The Fourth Annual Shane Conlan Golf Classic was
held May 18, 2015 at the Sewickley Heights Country
Club. More than 120 golfers participated and the

Sewickley 5K Heart & Sole Run and Walk
Kids, adults and whole families were among the
hundreds that participated in Heritage Valley
Sewickley’s 6th Annual 5K Heart & Sole Run and Walk
on September 20, 2014. Proceeds from the event
totaled more than $12,000. The event benefitted Faith
in Action, a community-based program that provides
transportation for senior citizens to doctor’s appointments
and medical treatments. Heritage Valley Sewickley
Foundation partnered with the Sewickley YMCA to
support Faith in Action’s efforts.
The Unique Boutique
The Unique Boutique welcomed hundreds of
shoppers on November 8, 2014 at the Edgeworth
Club in Sewickley. The event featured more than
50 unique artisans and vendors. The event raised
$26,000 in support of Staunton Clinic Behavioral
Health Services at Heritage Valley Sewickley.

The 25th annual Tito Francona Golf Classic was held on
August 18, 2014. More than 200 golfers turned out for
the event that was held at Olde Stonewall Golf Club and
Connoquenessing Country Club. The event raised more

Heritage Valley Beaver Auxiliary Holiday Ball
The Heritage Valley Beaver Auxiliary Holiday Ball was held
on November 8, 2014 at the Club at Shadow Lakes. During
the Holiday Ball, the Heritage Valley Beaver Auxiliary
Caregiver Award was given posthumously to Edward W.
Heinle, Jr., MD. On behalf of the family the award was
received by Dr. Heinle’s son, Dr. Michael S. Heinle. The
Caregiver Award honors a retired Heritage Valley Beaver
physician who has been a longtime advocate for the
Auxiliary and the hospital. More than $50,000 was raised
to benefit capital projects at Heritage Valley Beaver.
