Bring Copies of all the following documentation to your appointment
1) Copy of eviction notice
2) Copy of lease agreement
3) Proof of income for all household members with income for the last 90 days and a copy of your
bank statement if your income is direct deposited. Acceptable proof of income includes:
A) Pay Stubs
B) Letter from Employer on company letterhead showing employment, pay rate hour,
amount of hours per week and/or month allowed to work and supervisor’s name and
phone number for contact
C) State or Federal letter showing type of monthly check and amount you receive
D) Any other employment and/or legal form
4) Copies of social security cards for all household members
5) Copy of driver’s license or state I.D. for person applying for assistance
6) Copy of your medical card only if you are receiving public aid cash benefits
7) If you are on Section 8 and your residence is not approved by the housing authority, a copy of
the denial letter
8) If you are a victim of domestic violence, a copy of the police report
9) Copy of your occupancy permit
10) Landlord’s name, address and phone number
All of the above information is required before assistance can be given!!!

Renting an Apartment
Inspection



The apartment should be inspected before you move in.
Contact your local housing or building department to request an inspection.
If the apartment does not pass inspection, do not move in until it does.
Lease
What should be stated in the lease?








The amount of the rent
How long the lease will last
How the lease should end
How to renew the lease
How much will be charged if rent is paid late
Who is responsible for repairs and how to get repairs made
When the landlord can enter the apartment and what type of permission or notice to you is
needed
Any other promises made by the landlord
What to do if you don’t understand part of your lease:




Ask the landlord to take the lease with you to look over before you sign it. Make sure you
understand all of the provisions. Do not rely on the landlords representations as to what
the lease means
If something in the lease is unclear, you should contact an attorney to review the lease for
you. You should ask the landlord to clarify that provision or replace it with the explanation
he or she gave you.
If the landlord refuses to clarify, you should be weary of the landlord’s explanation of the
provision.
If there are parts of the lease that you do not agree with ask the landlord politely to change
the part that you do not agree with. If he refuses, you should not sign the lease.
The Apartment Walk-Through
Do I need a walk-through with the landlord if the apartment passes inspection?


Even if the apartment has passed inspection, before moving in. you should walk
through the apartment preferably with the landlord and make a list of any damage
that exists.
You should also note any damage such a stains, broken appliances, etc. You should
also check all electrical outlets and all appliances that are provided with the
apartment to see if they are working.

You should also make sure the toilets flush properly. Turn on the water in the
kitchen and bathroom to see if they have proper water pressure, and check the
ceiling for spots, which indicate possible leaks.
What should I do if the landlord refuses to do a walk-through with me?


If the landlord is not available or refuses to walk through, you should walk through
with a friend making note of any damage and if possible take pictures.
Make sure the pictures show the damage that you noted. You should keep a copy
of the list of damages and give a copy to your landlord requesting that he or she
attach it to your lease.
Repairs
What should I do about repairs that are needed before I move in?






Some damage will be of a nature that you should note that it exists, but will not
need replacing. For instance a spot on the carpet should be noted, but as long as
the carpet is clean, the carpet does not need replacing.
On the other hand, a window may be cracked. This should me noted and you
should request that the landlord repair it before you move in.
Do not sign a lease, until all repairs that have been requested are made.
If the landlord tells you the repairs have been completed, politely request that he or
she walk you through once more before you sign the lease.
Most leases state that all promises that have been made are stated in the lease and
no promises were made. If he or she promised to make a repair and it is not
provided for in the lease, it will be presumed that the promise was not made. You
will have to prove that the promise was made.
To prevent from having to prove that the promise was made, make sure all
promises are fulfilled before signing the lease or that the promises are in writing. At
the very least, you should have a witness present at the making of the promise.
Application Fee


Application fees are charged for the cost of checking your employment, references
and credit to determine if you qualify to rent the apartment. Application fees range
form $0-$50
You should get a receipt for the application fee. Most application fees are nonrefundable. If you are told that the application fee is refundable make sure that
your receipt or other document says so.

Security Deposit

Security deposits are paid before you move into an apartment.

Services offered by the LCCC Dental Clinic include a thorough
assessment, a treatment plan, preventative (teeth cleaning) or therapeutic
periodontal treatment, oral hygiene instructions, placing preventative
agents (fluoride, sealants), and any necessary referral for treatment outside
the realm of our services, and follow-up care.
The dental hygiene students’ thorough oral assessment includes an oral
cancer exam, periodontal exam, charting of existing conditions and
radiographs (x-rays).
The students are supervised by licensed dental hygienists and dentists
during diagnosis and treatment, which may include a dental cleaning or
more intensive treatment for periodontal/gum disease.
Appointment days and times vary each semester, so please call for more
information.
