Children’s Hospital Los Angeles designates this educational activity for a maximum of 13.5 category 1 credits. Each physician should
claim only those hours of credit that they actually spend in the activity. 2012 Conference attendees should be aware of current
procedures for obtaining their CME credit. Attendees must complete, sign and return a CME documentation form to record the
sessions that were actually attended. Signing the form also functions as verification of your attendance at the meeting. Forms that
are not signed will be returned. To comply with current policies of the ACGME and the American Medical Association, the Pediatric
Critical Care Colloquium will award CME certificates noting the actual number of hours that each physician participated in educational
activities. Certificates will be sent after the meeting.
Registration for the Optional workshops (How to teach leadership and professionalism in pediatric critical care and Managing the
difficult airway in the PICU) will be limited. Participants who choose to attend these sessions will be awarded an additional 1.5 CME
category 1 credit hours for each session. Please register for these sessions by contacting Ginger Mallas.

The appropriate registration fee for full or partial attendance must accompany the application. In the event of cancellation, $25.00
will be retained as an administrative fee. No refunds will be made after August 15, 2012.
