Habilitation Specialists III needed to provide Care
Coordination functions for those members with
intellectual and/or developmental disabilities. These
positions are tasked with assisting the member in the
development of the individual service plan, assessment
of support needs, monitoring the implementation of the
individual service plan, monitoring health and safety
and linkage to needed MH/DD/SA resources
Graduation from a four-year college or university with
a Bachelor’s Degree in special education, elementary
education, or early childhood education, psychology,
nursing, child development, counseling, infant mental
health, social work, child and family studies and two
years of professional experience working with the
population served (I/DD); or graduation from a fouryear college or university with a Bachelor’s Degree
in another human service field and three years of
professional experience working with the population
served (I/DD); or an equivalent combination of training.
