How to complete your time sheet
One time sheet must be completed per week for each client that you have worked for.
• If you have worked for 2 different clients during the week then you will send 2 different
time sheets.
• If you have worked for only 1 client then you complete only 1 time sheet, no matter how
many days you worked for that client.
A timesheet week starts on a Monday and ends on a Sunday.
All sections must be completed in CAPITAL letters
Section 1
In this section you will be providing us your details and details of where you work.
Candidates Name:
Your full name.
Employee Number:
Please leave blank unless advised by a member of Core Connexions.
Name of Ward:
The name of the Ward, Floor or Department where you completed your shift.
Qualifications/ Position:
Your qualification or status, in the position for this role i.e. RMN/ RGN/ HCA/
Auxiliary/ Senior Carer.
Home/ Hospital:
The name of the Home/ Hospital where you completed your shift.
Address:
The address of the Home/ Hospital where you completed your shift.
Our Finance department processes timesheets for the whole country, the names of Wards,
Departments or homes can be very similar; the address provides the necessary information to
identify the right place your shift was completed.

PLEASE NOTE: If you have worked at Bridge House the authorisation code must be added here,
or you will not be paid.
Section 3
Total hours in words; this must be in CAPITAL letters.
Section 4
This section is authorisation for you to receive payment for your shifts.
This must be completed by the senior nurse, the manager or the shift co-ordinator.
Remember if your timesheet is not signed or is not signed by the correct person you will not receive
payment for your shifts.
