1. Complete this claim form and attach the original sales receipt clearly indicating the purchase of eligible
Goodyear Tires.
2. Mail To: Goodyear Winter Rebate, 52 Six Point Road., Toronto ON M8Z 2X2
3. List of eligible tires available on www.goodyear.ca. Submissions for non-eligible tires will not be processed.
4. Not valid with any other Manufacturer’s Rebate Offer including, without limitation, any “Instant Rebates”.

1. Complete the claim form below, including store name and location. This must be completed to qualify for your rebate.
2. Mail this coupon together with the original copy of your sales receipt (debit card, credit card and bank statements are not acceptable) to:
GOODYEAR WINTER REBATE, 52 Six Point Road, Toronto ON M8Z 2X2. Invoice must be dated within the period from
September 15 to December 6, 2008.
3. Completed coupon and original sales receipt must be dated between September 15 to December 6, 2008 and mailed with a postmark no later than
January 6, 2009. Goodyear Canada Inc. is not responsible for lost, delayed or incomplete claims. Please allow 6-8 weeks for receipt of your rebate.

The Goodyear Winter Tire Rebate Event is a manufacturer's mail-in rebate. Mail-in coupons and eligible tires are available on the www.goodyear.ca website. Rebates apply to purchases of a set of four selected tires as listed on www.goodyear.ca. Costs for installation, balancing or other
labour/parts are not eligible for rebate. Mail-in offer for eligible purchases between September 15, 2008 and the expiry date of December 6, 2008. One mail-in coupon for 4 selected tires only per invoice will be accepted. The consumer must provide an original copy of the invoice along
with the printed rebate coupon and must send to the address printed on the coupon postmarked on or before January 6, 2009. Offer is valid for Canadian Residents only and valid only for tire purchases from a participating Canadian retailer who operates a retail location in Canada.
Mail-in rebates are not valid with any other Manufacturer's Rebate offer, including without limitation “Instant Rebates” or with purchases from any Goodyear dealer outlets offering “Instant Rebates”, or for any purchases made at Sam's Club, Wal-Mart or Canadian Tire. The participating
retailer is solely responsible for determining the selling price of the tires without direction from Goodyear. See a participating retailer for more details.
