USANA Japan Inc. (hereafter USANA) is a US based company that focuses on selling high quality
nutritional supplements and skin care products. USANA headquarters in Utah, USA. It is a global
company with 10 subsidiaries in more than 12 countries at present. In Japan, USANA has successfully
attracted many people who become Associates with superb achievement.

There are two types of membership to choose depending on your lifestyle.
Associates
Once USANA accepts an Associate’s Application and Agreement, the benefits of the Cellular Compensation Plan and
available to the new Associate. These benefits include the right to:
Purchase USANA products and services at Member price;
Participate in the USANA Cellular Compensation Plan (receive bonuses and commissions, if eligible);
Sponsor other individuals as Preferred Customers or Associates into the USANA business and thereby build a Downline
Organization and progress through the USANA Cellular Compensation Plan;
Receive USANA literature and other USANA communications;
Participate in USANA-sponsored support, service, training, motivational, and recognition functions upon payment of
appropriate charges, if applicable;
Participate in promotional and incentive contests and programs sponsored by USANA for its Associates.
Retail USANA products or services and profit from these sales.
Preferred Customer
Preferred Customer can purchase all USANA products at member price or Autoship price for personal consumption.
Registration fee and renewal fee are free of charge.
How to apply for membership registration
Registration
Registration for both Associates and Preferred Customers can be processed through fax, mail, or at the USANA
Service Center. Each Associate is required to submit an Associate Application Form to USANA. Please provide
a copy of the form to your sponsor, and keep another copy for yourself. If you register via fax, please send the
original copy of application form to USANA.
To become an Associate, Registration Fee: 4,200yen (including sales tax) are required. This fee may be paid by
Cash at USANA Service Center, Credit Card and Bank Transfer.
Renewal of Membership
Associate membership expires in one calendar year from the day when USANA accepts the membership. To
continue the membership, Associates must pay annual renewal fee of 2,100-yen (including sales tax).
Membership will be renewed automatically. Failure to renew the Associate membership is considered as
voluntary cancellation, and the contract may be terminated.
Requirements to become Associate:
Be at least 20 years old, (students are prohibited from enrolling);
Applicant’s spouse and dependent are not maintain a membership;
Reside in Japan. Registered address must be identical to the address in the certificate of residence;
At least 6 month has been passed from the termination of previous membership
Have a bank account in Japan

Method of Purchasing Products
After membership registration is completed, you are entitled to purchase products at the Member price,
which is at a discount from the suggested retail price. Moreover, Associates and Preferred Customers may
participate in the USANA Autoship Program.
Regular Orders
Order by: telephone, fax, mail, at USANA Service Center (Will Call) or through Online Shopping (through
USANAtoday website).
Payment methods can be selected from following ①,②,③ and ④.
Autoship Program
Associates and Preferred Customers can identify the products they wish USANA to automatically send to them
on either a 4 week or 8 week cycle.
Ordering Method: fax, mail or at the USANA Service Center.
Orders can be changed online (through USANAtoday website or USANA mobile site)
Payment methods can be selected from following①,②,③,④ and ⑤.

*Automatic withdrawal from your designated account which you authorize payment in accordance

Please deposit Autoship amount to one of USANA’s bank accounts by Friday prior to the
Autoship week. Members are responsible for the handling fee when using bank transfer.

④ USANA Conveni-Card (Smart-Pit-Card)
Payable at Lawson, Family Mart, Three F, Mini-stop, Circle-K Sunkus. Please pay Autoship amount
between Thursday prior to Autoship week and Friday in the Autoship week.
*This payment method is not applicable for the first (initial) order. The USANA Conveni-Card is
available upon request by a product order form. (1st issuing fee is free)
⑤ Automatic Withdraw through Post Office Account (only for Autoship)
USANA will electronically withdraw the Autoship amount from an authorized Post Office account on
Monday, every 4 or 8 weeks. Please submit the “Automatic funds transfer form” at the post office when
you apply for it. As it takes about two to three weeks to complete the set up, you might be asked to pay
by different methods for the first Autoship order. (Please prepare the adequate funds of withdrawal
before Friday prior to Autoship week.)
Delivery Fee
630 yen are charged for both regular and Autoship orders
*Delivery fee will be free of charge for the starter kit, first Autoship, first-time order, orders of 150 points or more,
online orders, and Autoship orders which are changed online.
Product Delivery
Orders are delivered from USANA Japan Shipping Center after confirming a payment. Normally it takes about three
business days to deliver, but the time is dependant on the destination of delivery. Products ordered at year end and
New Year, Bon holiday, or under conditions of bad weather and unforeseen accidents may delay the receiving time.

Voluntary Cancellation of membership
A USANA member may cancel his/her membership agreement at any time, for any reason by providing a notice in
writing to USANA. USANA reserves the right to involuntarily cancel any Associate who violates USANA’s Policies and
Procedures.
Product Returns
Returning Products and Voluntary Cancellation within 30 Days – Cooling Off
If an Associate elects to cancel his/her Distributorship during the 30 day period immediately
following his/her enrollment, USANA will refund 100% of the price of the Starter kit and product
purchased as part of the Associate’s initial order. The following policies apply to returning product
within 30 days.
Cooling off will become effective from the date when the written notice is sent (with postmark).
Damages and penalties are never claimed.
USANA will pay for returning charges for the Starter Kit and any products that have been
already received.
USANA will refund full price of the registration fee and products that have been received.
If an Associate has been threatened, misled or otherwise deceived, the cooling off period will be
extended 20 days after the sponsor provides you a document to dissolve cooling off
interference.
All Other Returns
You may return to USANA products (including promotional materials) purchased within the past
365 days for a refund of ninety percent (90%) of the purchase price if products are in resalable
condition. Members are responsible for returning charges. USANA may deduct from the amount
of the refund previously paid commissions and bonuses the member may have received as a result
of the products that he/she is returning. However, if the total amount exceeds 10,000 yen, it will
be considered voluntary cancellation and the distributorship will be automatically terminated. In
addition, in this case, a registration fee will not be returned. This also applies to the case when
cancellation of membership is occurred after the cooling off period.
*Resalable products: The products are unused, unopened, and have not yet expired when USANA
receives them.
*In all returns, Associates are required to include the packing slip with the returned items.
Temporary Suspension of payment,
According to Installment Sales Law, a member can refuse to pay credit card companies or other credit
companies under the following circumstances with either USANA or other Associates. The following are
conditions in which a member can refuse to pay applicable charges:
1. Grievances with USANA or Sponsor
① No or partial delivery of products
② Unwarranted delay of product delivery
③ Exaggeration of products, rights, or services
④ Disparity between samples and images in the catalog, and the actual products
⑤ Breach of contract
2. Grievances with other Associates relating to USANA products and services
① Products and/or service purchases are promoted as required or threats are used
② Products and/or service purchases are promoted fraudulently
③ Products and/or service purchases are promoted misleadingly
Compliance and Prohibited Practices
In recruitment and business activities, Associates must explain the following items thoroughly:
Category of products, performance and quality
Specific charges required for registration and product purchases
Cancellation of contract (including cooling off policy)
The correct way to earn commissions, and the typical Associate earnings
Other items that may affect decision-making
Upon recruiting a new member or interfere the cancellation of contract, you may be punished by “Specific
Commercial Transaction Law” if you tell what is not true about the above mentioned items. Similar
punishments may apply if you threaten and embarrass prospects, or take prospects to a non-public place
without telling its purpose.
Do not present, advertise or explain that products have pharmaceutical effectiveness, such as
treatment, cure and relief of health problems.
When you visit prospects, you must tell them your name, give the company name, and explain the
purpose of visit, such as explaining you will be providing information about nutritional
supplements. Before inviting people to seminars and lectures, you must tell them the purpose of
the meeting.
Do not invite prospects to a place where many people don’t come in and out freely without
providing them with a proper explanation of all required charges.
Do not force prospects to purchase unnecessary products.
Do not mispresent or mislead prospects as if profits are guaranteed. Furthermore, do not
misrepresent USANA by saying that anyone can easily earn income.
Do not pay for others, encourage financing or debt, allow others to use your name or purchase
products under your downlines.
When a prospect refuses to try the products or enroll, do not continue to persistently pressure
him/her to enroll.
Privacy Policy

USANA acknowledges the importance of protecting personal information and making a best effort to
manage personal information. Under the circumstances described below, personal information (name,
address, phone number, e-mail address, product purchase and return record, points and other
information) is possibly disclosed to the third party.
Disclose to upline if necessary (including DLM)
Provide to business contractors and venders for distributing products, printed
materials or conducting market research.
Provide to business contractors and vendors to pay various sorts of income by
electric fund transfer.
Respond to requests in writing from the court, police, or other authorities
