With the evolution of healthcare reform an increasingly
important area of focus is the continuum of care. This
program will provide different perspectives on leading
quality change across that continuum of care.
Presenters will address components of the Triple Aim
(Population Health Management, Improving Patient
Experience, and Reducing Costs). Attendees will
increase understanding on best practices in behavioral
health, acute care transitions, ACO coordination
practices, patient experience/engagement initiatives, and
acceleration tools for leading that quality change.
