GENERAL DIRECTIONS
ADMISSION TO UNC
In order to be screened for admission to the nursing program, you must first be admitted to the University
of Northern Colorado.
 If you have never attended the University of Northern Colorado, you must apply to the University as
well as to the School of Nursing. Formal applications to the University are obtained from and
submitted to the UNC Admissions Office. Admission information and application can be found at
www.unco.edu/admissions.
 If you are presently enrolled at the University of Northern Colorado or were enrolled during the past
year and withdrew in good academic standing, you need only apply to the School of Nursing.
 If it has been more than one year since you have attended, you may need to reactivate your status
through the UNC Registrar’s Office.
SUBMISSION OF APPLICATION TO THE SCHOOL OF NURSING








Applications for the Clinical Nursing Program must be submitted on or after January 5, 2015.
DUE DATE FOR ALL MATERIALS is Monday, JANUARY 28, 2015 by 5pm, IN THE SCHOOL
OF NURSING OFFICE. Materials received after that date/time will not be accepted.
Please Type all answers into the appropriate boxes.
Submit all required documents including the checklist on the cover page of application.
You can hand deliver to: UNC School of Nursing, Gunter Hall Room 3080
You can mail your application to: UNC - School of Nursing, Attn.: Undergraduate Applications, 501
20th Street, Campus Box 125, Greeley, CO 80639.
Online applications are not accepted.
Repeat applicants must submit all required documents.
TRANSCRIPTS – ALL SCHOOLS (EXCEPT UNC)
Students must submit OFFICIAL UNOPENED transcripts to the UNC Admission’s Office. These
transcripts must:
 Be from all attended schools (if not previously submitted to Admissions or have new coursework)
 Include fall 2014 semester grades (if applicable).
 Include Proof of Registration in any final non-completed prerequisite courses. Proof can either be
official to the Admissions office or unofficial attached to the School of Nursing application.
 Include Interim 2014-2015 semester grades (if applicable).
Guidelines for submitting official transcripts:
 If the school issuing the transcript will send the transcript directly to you:
o DO NOT OPEN the official transcript.
o Submit to the UNC Admissions office.
 If the school will not issue the official transcript directly to you, have it mailed to:
o UNC, Admission’s Office, 501 20th Street, Campus Box 10, Greeley, CO 80639.
 Official Transcripts may be electronically sent directly from the University/College Registrar’s office.

TRANSCRIPTS – UNC COURSEWORK
Students must request an unofficial UNC transcript be faxed to the School of Nursing.

Last four digits of Bear number or last four digits of social security number. Please
do not include full Bear number or SSN for security purposes.

Request on or after Monday, December 22, 2014 to insure all fall 2014 grades are included.
CERTIFIED NURSE AIDE

A Certified Nurse Aide course and clinicals must be completed by January 28, 2015. Attach a copy
of CNA certification or proof of Nurse Aide course completion to your application. Active, official
certification is required by April 15, 2015. Out-of-state active CNA certifications are acceptable.

than 1 page in length and no smaller than 11 point font. If a section does not apply, you may omit
that section from your resume.
The resume is used only in the event of a tie for admissions.

SCHOOL OF NURSING
ADMISSION PROCESS & CRITERIA
1. Admission to the University of Northern Colorado.
2. Potential for successful completion of all required prerequisite courses by the end of Spring
Semester, 2015.
3. Submit School of Nursing Application by January 28, 2015 at 5pm.
Screening Criteria:
 Completion and verification of a Nurse Aide course.
 Submission of proof of registration in all remaining prerequisite courses. An unofficial
transcript is acceptable to document this.
 Grade Point Average (GPA).
a. A minimum grade point average of 3.0 on a 4.0 scale of most recent 45 credit hours.
b. Computation of GPA
 GPA will be calculated using the most recent 45 credit hours of coursework
and GPA from all of the completed nursing prerequisite courses except BIO
110/BIO 101. The BIO 110 grade will be used in the GPA only if it was
completed in the most recent 45 college credits.
 High School Advanced Placement scores are not factored into GPA.
 Grades from technical courses such as CNA, EMT, etc. will not be included.
 Grades from remedial course work will not be counted.
 A grade of “C” or higher is required in each of the prerequisite courses (or course equivalent).
“C-” is not acceptable. If a student is admitted to the Nursing program and earns less than a
grade of “C” in any of the prerequisite courses or course equivalencies, their admission to the
Nursing Program will be rescinded.

IF ACCEPTED INTO THE NURSING PROGRAM
1. NOTIFICATION - Notification letters will be sent to applicants in March 2015.
2. REQUIREMENTS AND INFORMATION FOR NEW NURSING STUDENTS - Information
regarding program requirements such as purchase of malpractice insurance, specific health
clearance requirements, CPR certification, purchase of uniforms and ATI materials, as well
as course information will be sent to admitted students once they have signed the statement
of intent indicating they plan to attend the UNC Nursing program.
3. AN OPTIONAL MEETING for all newly admitted nursing students will be held to answer any
questions about Nursing Program requirements.
FOR YOUR INFORMATION—EXAMINATION FOR REGISTERED NURSE LICENSURE
Upon completion of the nursing program, you become eligible to take the NCLEX, a National Board of Nursing
licensing exam for the Registered Nurse license. The following questions are asked on the application for
licensure. If you answer “yes” to any of them, you may not be eligible for licensure.

Has any nursing or other health care license held by you ever been denied, revoked, suspended,
reprimanded, fined, surrendered, limited, or placed on probation in any state other than Colorado or in
any territory of the United States?

Are you under investigation or is a disciplinary action pending against your nursing license or other
health care license in any state or territory of the United States?

Have you received notification from the Department of Health and Human Services, Office of the
Inspector General, that you have been excluded from participation in Medicare, Medicaid or any
federal health care programs?

Have you ever been convicted, entered a plea of guilty, nolo contendere, or no contest for any felony,
misdemeanor or petty offense?

Have you ever been convicted, pled no contest/nolo contendere, or had a court accept a plea to a
criminal motor vehicle offense of DUI/DWI/DWAI/OWI or any traffic offense involving drugs or
alcohol?

Has any final judgment, settlement or arbitration award for malpractice been paid by you or on your
behalf?

In the last five years, have you been diagnosed with or treated for a condition that significantly
disturbs your cognition, behavior, or motor function, and that may impair your ability to practice as a
professional nurse safely and competently, including but not limited to bipolar disorder, severe major
depression, schizophrenia or other major psychotic disorder, a neurological illness, or sleep disorder?

Do you now abuse or excessively use, or have you in the last five years abused or excessively used,
any habit forming drug, including alcohol, or any controlled substance that has
a. Resulted in any accusation or discipline for misconduct, unreliability, neglect of work, or failure
to meet professional responsibilities; or
b. Affected your ability to practice as a professional nurse safely and competently?

Have you been terminated or permitted to resign in lieu of termination from a nursing or other health
care position because of your use of alcohol or use of any controlled substance, habit-forming drug,
prescription medication, or drugs having similar effects?

Have you been arrested for an alcohol or drug-related offense other than stated in bulleted item No.
5?
You should also be aware that the School of Nursing is required to report knowledge of any of the above
behaviors to the Colorado State Board of Nursing. Although an affirmative answer to one of these questions
does not automatically mean you will be denied licensure, it may mean the Board will not consider you
eligible even if you pass the NCLEX exam. Each case is considered on its individual merit.
