on Accreditation through the joint providership of the University

Development and Outreach and the University of Wisconsin School of
Medicine and Public Health.

Conference Fee
The conference fee of $425 for physicians, $325 for residents, physician assistants, nurses,
and others includes the cost of tuition, materials, continental breakfasts, lunches, refreshment
breaks, and a nonrefundable processing fee of $50. Should you cancel your registration up
to 72 hours prior to the conference, you will be refunded the entire conference fee except
the $50 nonrefundable portion. No refunds will be made after that time.
Conference Housing
A block of rooms has been reserved at the conference site, The Madison Concourse Hotel.
Please write or call the hotel directly (800.356.8293) to arrange your accommodations.
Please mention “Primary Care 2014” to the hotel staff when making your reservation to
receive the special room rates of $136 (s), $146 (d), plus tax. The Madison Concourse
Hotel will release the room block on October 14, 2014. After this date, rate and availability
cannot be guaranteed.
Conference Attire
Casual attire is appropriate for this conference. Since meeting room temperatures and
personal comfort levels vary, it is recommended that you bring a sweater or jacket to the
educational activity.

The course syllabus will be provided on a USB drive. Participants are welcome to bring
laptop computers for use during the conference. Supplemental course materials will be
provided in hard copy.

Intended Audience & Scope of Practice
This CME/CNE activity is intended for primary care physicians, physician
assistants, nurses, advanced practice nurses, and other health care
professionals.
Elements of Competence
This CME/CNE activity has been designed to change learner competence
and focuses on the American Board of Medical Specialties areas of patient
care and procedural skills, and medical knowledge.
Practice Gaps and Needs
Based on documented gaps in current practice, this activity will focus
on critical topics in primary care identified through professional practice;
local, regional and national data in healthcare quality; and requests from
participants in related CME/CNE activities.
The overall focus of this activity is to discuss these practice gaps and
provide participants with the knowledge, skills, tools and resources needed
to change their professional practice and improve the care of their patients.
Learning Objectives
The 2014 Primary Care Conference is designed to:
• Describe the latest evidence regarding diagnosis and evaluation of
medical conditions commonly seen by primary care clinicians
• Summarize state-of-the-art management of common medical conditions in
clinical practice
• Employ new strategies to enact behavior change in patients
