JOB SUMMARY
This full-time position is responsible for helping to maintain the daily operations of the Guardianship Program
and. In addition, this position is responsible for marketing the Guardian Partner Program to organizations and
individuals in the community.
ESSENTIAL DUTIES include the following. Other duties may be assigned.
 Marketing of Guardian Partner Program.
 Visit clients; provide for their needs and assure good care.
 Prepare and submit timely reports.
 Assist clients in transitioning from community to another setting as needed.
 Assist in the liquidation and management of clients belongings and assets.
 Maintain case notes and client files in a timely and orderly fashion.
 Preparation of legal documents.
 Maintain working relationships with program partners.
 Accurate and timely billing for services rendered.
 Manage all client income and pay all client expenses.
 Assess new clients; participate in referral process for potential clients.
 Transition clients from private pay to Medicaid.
 Assist in arranging funeral trusts, shopping for, visiting, and checking on personal needs.
 Attend care conferences and medical/business appointments as needed.
 Assist in providing off hours phone coverage.
 Maintain communication with appropriate other parties including family, friends and service providers.
RESPONSIBILITIES
 Help maintain a strong and stable work environment;
 Provide complex planning and decision making for clients including medical, personal, and financial
decisions.
 Research potential referral sources for program;
 Make presentations to community organizations and groups;
 Represent department and agency in legal proceedings:
 Administrative requirements;
 Direct arrangement of funeral trusts
 Develop and prepare benchmarks and outcome measures where applicable;
 Providing cell phone coverage for client emergencies outside regularly scheduled work hours;
 Insure and provide quality care to Guardian Partner Clients;
 This program may be paid for by Guardian Partner program income.

EDUCATION AND/OR EXPERIENCE REQUIREMENTS
Bachelor’s degree (BA or BS) from an accredited four-year college or university in Sociology, Gerontology,
Psychology, Social Work, or related field, or relevant work experience.
 Experience in direct care, advocacy or community service.
 Experience in marketing is a plus.

 May encounter difficult situations
 At times will have to deal with difficult family members and friends of clients
 Must be prepared to deal with emotional response of losing a client
 Constant multi-tasking is required
 Ability to handle frequent changes in priorities based on presenting situations.

While performing the duties of this job, the employee is usually seated, standing, or walking at will in a typical
office setting. Very limited physical effort required. Travel required in the service area. Sorting through client
homes and belongings as needed.
 Valid driver’s license and a dependable vehicle required
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
 Working knowledge of accounting, business management or legal procedures is a plus.
 Knowledge of basic computer applications
 Ability to coordinate care with other agencies and care facilities
 Ability to influence others to reach positive solutions
 Organized with exemplary attention to detail
 Familiarity with elder care and dealing with mentally incapacitated older adults
 Ability to relate well to others
 Excellent communication skills (both verbally and in writing)
 Comfortable with public speaking and marketing

 With Adult Protective Services (APS) and the court system
 With Nursing Facilities and other service providers
 With client’s family, friends, and other interested parties
 With Banks, attorneys, trust departments and other program partners
 With Programs Director, Area 2 Agency on Aging Director, and President/CEO
 With Guardianship Advisory Council
CONDITIONS OF EMPLOYMENT
 Successful candidate must submit to a pre-employment drug screening and a limited criminal history check.
 Proof of educational credentials is required at time of employment.
 This position has a 6 month probationary period.
