CONFIRMATION of ENROLMENT INSTRUCTIONS
1.All applicants who paid their reservation fees are required to download
the following documents from the myuste portal:
1.1. Confirmation of Enrolment Form
1.2. Important Reminders
You will have to login first using your applicant number and surname.
2.Check your confirmation of enrolment schedule and bring all the
necessary documents found in your Confirmation of Enrolment Form.
3. Bring all the additional documents found in the College Instructions,
when applicable.
4. Three (3) days after submitting all the required documents, login
at http://pds.ust.edu.ph to accomplish the Personal Data Sheet
(PDS) online
and print the Acknowledgment Slip. There is no
need for you to submit the Personal Data Sheet accomplished online.
5. The Acknowledgment Slip is a requirement for enrolment. With your
Acknowledgment Slip, proceed to enrolment. (Please take note of
your schedule for enrolment).
COLLEGE INSTRUCTIONS:
I.
COLLEGE of REHABILITATION SCIENCES
1. The following documents are to be submitted during the confirmation
of enrolment:
1.1. Original form 138 (High School Card)
1.2. Original Certificate of Good Moral Character from the high
school principal, adviser and guidance counsellor
1.3. One (1) passport size colored picture
1.4. Accomplished Modified Physical Activity Readiness
Questionnaire (PAR-Q) which may be downloaded by

2. All applicants are advised to bring the following documents during
confirmation of enrolment for presentation purposes only:

II.
COLLEGE of NURSING
1. All applicants are required to bring one (1) of their parents or
guardians during the confirmation of enrolment as an
orientation will be given prior to the confirmation procedure.
Signing of the contract for all freshmen applicants will be done
after the said orientation.
2. All applicants are required to submit all their credentials on that
day. Enrolment will not proceed if the applicant has not yet
submitted their credentials to the Dean’s Office.
2.1. Original Form 138 (4th year Report Card)
2.2. Certificate of Good Moral Character (from School
Principal)
2.3. Proof of payment of reservation (metrobank
payment slip or official receipt coming from the
Office for Admissions).
3. For non-Filipino applicants/foreigners and Filipino citizens who
are graduates of foreign schools: Please submit your enrolment
credentials to the Dean’s Office. These will be forwarded to the
Office of the Registrar for evaluation and verification prior to the
confirmation and signing of the contract.
III.
COLLEGE of EDUCATION
1. The following documents are to be submitted during the
confirmation of enrolment:

XIII. COLLEGE OF TOURISM & HOSPITALITY MANAGEMENT
1. Successful applicants and their parents should proceed to the waiting
areas (rooms 9A & 9B) at the Buenaventura Garcia Paredes,
O.P.,(BGPOP) bldg. on April 07, 2015.
2. Applicants will proceed to either room 6A for TRM or room 6B for
HRM to submit their original high school card and certificate of good
moral character with 2X2 picture.
3. On the day of the Confirmation of Enrolment, Freshmen may
survey/purchase college uniforms available at room 704 of the
Buenaventura Garcia Paredes, O.P.,(BGPOP) bldg. Please be ready
with the down payment if ever.
4. Refer to the OFAD website (ofad.ust.edu.ph.) for additional
instructions.
