Instructions
1. Customer:
Complete the entire section. If a specific delivery date is required include this date.
2. Tape Cartridge:
Clearly identify the quantity ordered and select the correct cartridge type. The IBM seven-digit
part number must be indicated by marking the appropriate box.
3. Label Information:
Identify the label type that matches your requirements and indicate horizontal or vertical label
orientation. For RFID include the part number. For WORM circle the Media ID and part
number.
Alpha Characters: Alpha characters require a background color; indicate color of choice,
alpha character is black.
Numeric Characters: Numeric characters are black on vibrant colors as shown. For the number zero, note
as Ø.
Label Sequence: Indicate sequence from beginning to end. Sequence example for 100 labels;
From: 12300 To: 12399. Each label includes a media identifier that matches the cartridge
format; JD, JC, JB and so forth. Any mix of alpha and numeric characters is allowed.
4. Initialization:
If your order requires initialization complete this section, select the drive and model to match
your requirements. Select the appropriate initialization format. If you are unsure of this
information it can be verified by the data center manager or your IBM Client Executive.
5. Cleaning Cartridge:
Clearly identify the quantity of cleaning cartridges ordered. Identify the last three digits in the
label sequence. For the number zero, note as Ø.
6. Special Options:
Use this area to identify options such as pad print, special RFID formats, no media ID.
7. Special Instructions: Use this area to indicate split ships, partial orders, special delivery instructions, or any other
special request.
8. For Internal Use:
Leave blank.
9. Delivery Information: Labeled orders ship approximately 1 week from receipt of order. Labeled and initialized orders
ship approximately 1-2 weeks from receipt of order. Standard ground routing applies. For air
route delivery the customer pays the difference between Standard Ground Routing and Air
Freight Charges.
