May 1, 2015 will receive a confirmation letter via email. The registration
fee includes the educational materials, which will be provided on a flash

is separate from the conference registration, but may be combined with the

Cancellation Policy
A refund, minus 30% for administrative costs, will be given for
cancellations received in writing before May 1, 2015. After this date, no
refunds will be available.
