ACTIVITY GUEST APPROVAL FORM/VISITOR
This form is required for an Erie High School student to bring a guest to an Erie High School dance or
special school sponsored activity.
This form needs to be completed in its entirety and turned in to the Principal’s or Assistant Principal’s
office at least two days before the scheduled event. Guests will not be permitted to enter the event
without prior approval.
**All guests must bring a picture ID to leave at the front gate throughout the duration of the
activity.**
***Guests must be in high school and/or under the age of 21.***

FOR NON-EHS GUEST
This form must be completed before you purchase tickets. Non-Erie High School guest must complete a
paper Prom Promise and submit with Guest Form by Thursday, April 23 (no exceptions!).

Initial next to each of the following to acknowledge that your have read, understand and agree to the
following rules and polices.
_____ I promise to behave responsibly before, during, and after Erie’s Prom. I will not have illegal
substances on or in my body and am aware that I will be dealt with by law officials, if deemed necessary.
I understand if this occurs, I will not be refunded my money. Be alcohol and drug free!
_____ I will follow the dress code guidelines. (1) No shorts, denim blue jeans or leggings. (2) No short
sleeve shirts (polos or t-shirts). (3) Gentlemen must have: a jacket, dress pants, and a tie or scarf/bow
tie. (4) Women in gowns or dress suits: no more than 4 inches above the knee, no lower than the level
of your belly button if it were on your back, not too revealing. (5) No undergarments may be visible. (6)
No ball caps or dew rags. Dress hats (i.e. cowboy hats, fedoras, top hats are allowed if part of outfit) .
Students will be denied admission and will not be refunded money if in violation of dress code.
_____ Any student who does NOT attend Erie High School must complete a guest form. Guest may not
be older than 20 years of age. Guest forms must be turned in TWO days prior to the event to Erie High
administration.
_____ I will abide by the dance and behavior requirements, including all school rules and SVVSD polices
(www.svvsd.org). This is a formal event of the year and a formal atmosphere will be maintained. Any
chaperone, sponsor or administrator may address any inappropriate behavior. Any behavior that is
deemed to be disruptive or dangerous in nature will be addressed. Students not following these
guidelines may be asked to leave the dance and may be referred for school level discipline.
_____ Since this is a school function smoking and tobacco use of any kind are NOT permitted for
anyone.
_____ Please bring all of your needed items with you when you enter the dance. An ID is required to
enter the dance. You will not be allowed to go to your car/limo during the dance. If there is an
emergency you will need a security or chaperone escort to and from your car/limo.
_____ You must bring your ticket and school ID. The ticket, number, and ID will be checked at the door.
Admission will begin at 8:00 pm.
_____ In order to provide adequate time for all prom goers to get to After-Prom safely, and also to avoid
any conflicts with the curfew laws of surrounding communities, the dance will conclude at 11:00 pm.
_____ To enter prom I must show current photo id.
