Pizza Punch Card Fundraiser Order Form

PIZZA “PUNCH CARD” FUNDRAISER
Harborside Academy along with Villa D’Carlo Restaurant – 5140 6th Avenue
is offering a punch card fundraiser to benefit student outward bound accounts.
What is it?? A punch card that can be used for dine-in or pick up only.
How much is it?? Each punch card is $15.00; students get $10.00 credit from each card purchased to
their individual Outward Bound account.
How does it work?? You must purchase a large pizza and you will receive a FREE medium cheese
pizza. Each punch card has ten punches…so that means you can get 10 FREE medium cheese
pizzas. Use the punches one or two at a time or all in one trip, great for family parties, sports
banquets or gifts!! Please note…Punch cards do not expire!
How to order?? Collect orders from neighbors, family and friends and have them PAY now for the
cards, checks are to be made out to Harborside Academy PTSO. Fill out order form found on back of
this sheet, then return to office by April 30th along with the money for all cards ordered. Punch cards
will then be printed and returned to students for distribution the following week.
