Benefits pen Enrollment for FY16 April 27 – May 15, 2015 Now is the time to review your current benefit elections,
learn about what's new for the upcoming year, and enroll
for benefits effective July 1, 2015
During the enrollment period you can:

Elect flexible spending accounts for health and dependent care for the
upcoming year; current elections do not continue
Make changes to your medical plan election
Add, or remove spouse or children to or from your medical plan
Enroll your dependents in dental coverage, if previously declined
Change your medical and dental premium deductions from pre-tax to posttax, or vice versa
Enroll in the ARAG Group Legal Plan; coverage continues for those
currently enrolled
No action is required if you are not enrolling in flexible spending accounts
and do not want to make any other changes in your benefit elections.
Please be aware that medical, dental, and flexible spending account
elections may not be changed during the plan year after the open enrollment
period ends, except in the event of a qualifying family status change, as outlined
below.
What's New?
Improved Medical Plan Benefits
•
The Preventive Care Services in each medical plan will be expanded to
include:
o Breast Cancer Risk Assessment and Genetic Testing for High Risk
Family History of Breast or Ovarian Cancer
o
Lung Cancer Screening for Insureds Age 55 or Over
o
•
Hepatitis C Virus Infection Screening
Outpatient Therapy Rehabilitation and Habilitation Therapy Services:
o PPO Plan - amount paid for In-Network benefits will increase to 85%
of Maximum Allowance, Out-of-Network will increase to 70% of
Maximum Allowance
o
Traditional and High Deductible Plans – the visit limit will increase to
20 per plan year
Summary of Benefits and Coverage
•
Employees are able to access the Summary of Benefits and Coverage
(SBC), as required by the Patient Protection and Affordable Care Act
(PPACA). The new SBC for the next year will be posted on the Office of
Group Insurance website. Those unable to access the SBC electronically
can obtain a hardcopy from the Office of Group Insurance (208-332-1860).
Medical and Dental Insurance Premiums
•
•
•
Employee insurance premiums will not increase for FY16. The
premium schedule for coverage as of July 1, 2015 may be
found here.
The University's contributions for employee medical and dental
insurance will increase for FY16, as indicated on the premium schedule
above.
Premiums for FY16 will start with the June 5, 2014 paychecks, paying for
the July health coverage.
Flexible Spending Accounts
•
Reminder – The Medical Flex Spending Accounts now include a
rollover provision. Employees who enrolled in health flexible spending
accounts for FY15 will be allowed to rollover up to $500 of unused funds in
their accounts to the next year. The rollover amounts will be added to next
year’s election. Remember, all employees participating in the flexible
spending accounts must enroll during the open enrollment period to
continue or start participation after June 30, 2015.

Dependent Eligibility Verification
•
The State Office of Group Insurance requires that all employees enrolling
their dependents (spouses and/or children) for the first time in medical
coverage through the state provide proof of eligibility. Employees who are
electing to cover a spouse or children under the Blue Cross plan during
open enrollment are encouraged to review the dependent eligibility
information found here in advance to confirm that their enrolled family
members meet these requirements.

Flexible Spending Accounts - Flexible Spending Accounts (FSA) allow you to
save money on eligible out-of-pocket health care and dependent daycare
expenses. Contributions you make come out of your paycheck before federal
income and Social Security taxes are deducted. The result – you pay less in
taxes and may have more take-home pay. Stanley, Hunt, DuPree & Rhine
administers both of the Flexible Spending Accounts.

Keep in mind:
You may enroll in a Flexible Spending Account during the open enrollment
period for each year, starting July 1st. New elections are required each year.
• You may not make any changes to your FSA during the year unless you
have a qualifying family status change (e.g. marriage, divorce, birth or
adoption of a child, etc.). There is a provision under the Health Flex
Spending Account that will allow you to rollover up to $500 of your unused
Health FSA balance to the next plan year. This is only for the Health FSA.
All the money in the Dependent Care Spending Accounts each year must
be exhausted. Any money remaining at the end of the year cannot be
carried over and it is forfeited. “Use it or lose it.”
• You and your dependents do not need to be enrolled in the medical plan
to enroll in the FSA.
Changes in Medical/Dental Plan Elections - Employees paying premiums on
a pre-tax basis may not make changes in their elections during the year after the
open enrollment period ends, unless they have a qualifying family status
change.
Qualifying family status changes include:
- Birth or adoption of a child
- Marriage
- Divorce
- Death
- Loss of eligibility for coverage under another health plan
- Significant increase in cost of coverage
- Employment-related events, such as a dependent’s job change, or annual
open enrollment
Employees should be sure to notify HRS of qualifying family status changes and
submit enrollment forms within 30 days of the event.

$7.38 per pay period. Existing employees may enroll or decline coverage during

information and forms will be available on Monday, April 27. Look for
forms for agencies that are not on the state controller's office payroll, and
select the forms for biweekly payrolls.

Original forms for medical, dental, and open enrollment choice cards must
be submitted to Human Resource Services - no copies or faxes accepted!
Flex Spending Account enrollment forms may be submitted using the
original form or by fax – to 426-3100. This is the only form accepted via
fax.
Deadline for all forms to be received in HRS is 5 pm, Friday, May 15,
2015.
