 Accommodation Booking Process A proud initiative of Rooftop Foundation Inc., For Family members requiring overnight accommodation or “Day-­‐Rest” in the ACT There are a number of accommodation options available for family members at subsidized rates. Visiting families and/or patients can make their accommodation booking by contacting the selected facility and request to make a booking under the IGA East Row House program room rates. Eligibility: Visiting families and/or patients are required to present upon check-­‐in at the accommodation a signed Visitor’s Card / verification by hospital ward clerk or medical personnel for their visit. Visitor’s card can be downloaded from www.igaeastrowhouse.org.au Overnight Accommodation And Long Stay 1 The following are a range of accommodation available for family members: 1. The Tradies Quality Hotel , Dickson 4-­‐star hotel Cnr Badham and Cape Street, Dickson ACT 2602 Tel: (02) 6247 4744 Room rate: $125 per night up to 2 people in a room. Additional $30 per person sharing the same room, maximum of 4-­‐5 people in the same room. 2. The application form is available at www.enable.health.nsw.gov.au. Selected accommodation facilities listed below has day rest facility those visiting family members not requiring overnight stay, but wishes to simply freshen up. Some “Day-­‐Rest” room includes lounge, television, tea and coffee making, shower and other amenities. This facility is provided free to family members. 4.
“Day-­‐Rest” facilities 2 Payment of stay 3 Eligibility: Family members are asked to print a “visiting card” from the web: www.igaeatrowhouse.org.au and get it signed/verified by the Hospital or Treatment centre staff BEFORE visiting the facility. The signed “visiting card” is provided to the accommodation staff on entering the “Day-­‐Rest” room as proof of eligibility. 1. The Tradies Quality Hotel, Dickson and Woden The Tradies Quality Hotel in both Dickson and Woden are located next to the Tradies Club. As an interstate visitor, you are invited to use the club facilities, including TV lounge, Café and Restaurants. 2. Dickson Central A Day-­‐Rest common lounge room has been established for those visiting family members who simply want a place to freshen-­‐up, shower, rest and watch television for a few hours. Tea/Coffee making facility is included free of charge. Clients using the accommodation facility is required to complete all necessary forms required by the accommodation provider on Check-­‐in and make full payment on departure. Note: Clients eligible for IPTAAS (Isolated Patients Travel and Accommodation Assistance Scheme) will be required to complete the application form and submit to the relevant Local Health District. Information about IPTAAS, application form and processes are available at www.enable.health.nsw.gov.au. A brief summary of the eligibility and how to apply is listed on the following pages as a guide. 2 IPTASS (Isolated Patients Travel and Accommodation Assistance Scheme) Eligibility To be eligible, you must: •

Be a resident of NSW or Lord Howe Island and also be eligible for Medicare; Live more than 100km from the nearest treating specialist; Require specialist treatment that is not available locally; Claim all benefits available through a private health fund prior to applying to IPTAAS (if applicable); Not be eligible for any other Government travel assistance schemes. How to apply •
Visit the EnableNSW website at www.enable.health.nsw.gov.au and download an IPTAAS Application Form or click on the link to ‘Apply online’; •
When you visit your specialist, take the form with you and ask that they complete Form C (and Form B if applicable to be completed by your referring Doctor); •

Collect any tickets or receipts for travel and accommodation and keep a record of your trips; Attached your travel information, receipts and tickets (copies or originals) to your completed application form and submit to EnableNSW or the IPTAAS office for your area. Note: Form B is only required if it is medically necessary for you to travel by air and/or have an escort/carer, or if advanced payment is needed. 17 years of age are automatically entitled to an escort/carer. Claims will be processed within 30 days of receipt provided all paperwork is completed. Incomplete forms will be returned to applicants for more information. Benefits under IPTAAS Assistance provided under IPTAAS will depend on your medical needs. IPTAAS will only subsidise travel and accommodation required for medical reasons and claims must be certified by a medical specialist. In some cases, payments can be made before you travel. To apply for advanced payment, contact EnableNSW or an IPTAAS office and request a declaration form. IPTAAS will reimburse receipts received within 12 months of the appointment or completion of a block of treatment.
