the Chattahoochee River National Recreation Area (Sandy Springs,
GA) for the provision of 800 MHz Radio System Access, retroactive

$9,900.00 for years 2015-2016, and $10,500.00 for years 2017-2018.

Health and Wellness

Request approval of a Memorandum of Understanding – Department
of Health and Wellness, with Emory University to collect information
from charts at the Tuberculosis clinic to perform a geospatial and risk
factor analysis on tuberculosis cases to inform intervention and
prevention activities. Effective upon BOC approval through October
31, 2015 with yearly renewal option.

Request approval for the Fulton County Department of Health and
Wellness and Georgia State University Research Foundation desire to
enter into an Intergovernmental Agreement (IGA) for GSURD to
provide e-SBI Pilot Project evaluation and training services that will
include managing data collection, entering results in an evaluation
database and submitting a detail report at the end of the contract
period for publication in a peer review journal.

Request approval of an amendment to the current County pool
ordinance - Department of Health and Wellness, to be effective upon
approval by the Board of Commissioners. The purpose of this
amendment is to update the existing pool ordinance to be in alignment
with recommended best practices for design and construction of pools
while ensuring the public health and safety of the citizens of Fulton
County. This action will incorporate recommendations based on the
Model Aquatic Health Code from the Centers for Disease Control and
Prevention (CDC).

Request approval to renew a Memorandum of Understanding between
the University of Georgia Cooperative Extension and Fulton County
Board of Commissioners to continue to provide research-based
education in the areas of Agriculture and Natural Resources, Family
and Consumer Sciences, 4-H and Youth Development to Fulton
County Citizens.

Public Works/General Services
15-0316
Request approval of a Radio Communications Tower Lease
Agreement between the Georgia Technology Authority (GTA), an
entity within the executive branch of the State Government of Georgia,
and Fulton County Emergency Communications. This Radio
Communications Tower Lease Agreement is needed for one of the
new radio tower sites for the County’s Digital Radio System Project.
The County’s Digital Radio System Project was approved via Agenda
Item # 13-0479 on 6/19/2013.

Request approval of a Revocable License Agreement between the
State Properties Commission, a public body within the executive
branch of the State Government of Georgia and Fulton County. The
Revocable License Agreement is needed in order to provide the
County with access to the Georgia Technology Authority (GTA), JC
Larimore Tower, for use in the County’s Digital Radio System Project.

Request approval of a Sewer Line Easement dedication between
Victor Sandoval and Maria Hernandez and Fulton County. The sewer
line dedication is a requirement under the County’s development
regulations and is to be granted to the County in connection with
construction of a residential development property and is known as the
965 Upper Hembree Project.

Request ratification of an emergency purchase order– Behavioral
Health and Developmental Disabilities Department - Emergency
Purchase Order #15SC96644CT, in the amount of $48,640.00 with
LOCUMTENENS.COM (Alpharetta, GA) for the provision of physician
services at the Center for Health and Rehabilitation for up to 16 hours
per week from February 11, 2015 through December 31, 2015.

15EPO96933DR-3 in the total estimated amount of $16,650.00 with
Spot Cooler, Inc. (Norcross, GA) for the rental of heat pump units at

Public Comment - Citizens are allowed to voice County-related
opinions, concerns, requests, etc. during the Public Comment portion
of the Commission meeting. Before speaking, each participant must fill
out a speaker card, located at the entry way, the podium, and the
media and court reporter tables. Speaker cards must be submitted to
the Clerk’s staff, who will accept them on a first-come, first-served
basis. Once Public Comment begins, speaker cards will no longer be
accepted. Speakers will be granted up to two minutes each, and
Public Comment will not exceed 30 minutes. Those who could not
speak during Public Comment will be allowed to speak first at a

Regional Commission (ARC), the City of Atlanta, the Atlanta-Fulton
County Water Resources Commission (AFCWRC), Cobb CountyMarietta Water Authority, DeKalb County and Gwinnett County each in
the amount of $800,000.00, effective upon Board of Commissioners
approval through December 31 2016. Fulton County and The City of
Atlanta each provide 50 percent of the AFCWRC share which is
included in AFCWRC’s approved 2015 budget.

Sheeting Materials in the amount of $100,000.00 with Vulcan Signs,
Inc. (Foley, AL) to provide sign and sheeting materials for the
fabrication and maintenance of road signs upon execution of contract
through December 31, 2015, with two renewal options.

Works/General Services, RFP #09RFP08155K-DJ, Operations &
Maintenance Services for Fulton County Wastewater Facilities and
Pump Stations in the amount of $12,093,822.00 with Veolia Water
North America – South LLC (Atlanta, GA) to provide operation and
maintenance services for the County’s wastewater facilities and pump
stations. This action exercises the first of a five year renewal option.
No renewal option remains. Effective dates: July 6, 2015 through July
5, 2020.
