6th Grade End of Year Activity Permission Slip

Dear CRMS Families,
Summer is just around the corner and we are in full planning mode for the end of year school events.
From the Earth Fair, Career Day, and Olympics, to the last day, there are a number of great activities and
learning opportunities for students to participate in. Additionally, there are a number of places we could
use your support. Please stay up-to-date by checking the website, liking us on Facebook, and/or
following us on Twitter and get involved by volunteering.
6th Grade Last Day Field Trip/Activities
On the last day of school, Friday, June 5, 2015, students will be participating in fun activities and field
trips to finish off a great school year. 6th graders will watch a movie in the gym, meet for lunch/snacks,
and then go to the pool to swim. Students are encouraged to bring a lunch from home that day,
although sack lunches from the cafeteria will be available to those students needing one.
The cost to attend this event is $5.00 to help offset the cost of swimming and snacks. Please make
checks payable to Cedar Ridge Middle School. Money is due no later than Friday, May 29th.
Students also need to be reminded that they must be eligible to attend this activity by demonstrating
positive behaviors and academic progress. Students that earn 2 or more discipline referrals or Storm
Warnings between May 22nd and June 5th will not be eligible to attend.
Please sign this permission slip and return it with your $5.00 to the front office by May 29, 2015. If you
