Pacifica families and individuals by providing a safety net of food, housing assistance, and other
critical services along with coaching, advocacy, information, and referral. The PRC has
partnered with the County of San Mateo since 1974 and is the only agency providing economic
security services, including comprehensive safety net services, in Pacifica for Pacifica residents.
POSITION SUMMARY:
Under supervision of the Direct Service Manager, the Case Manager provides a comprehensive
assessment of client needs, explores options for change with clients, and develops an action plan
to move clients from financial crisis to stability. The Case Manager provides direct emergency
services and connects clients with appropriate agencies. In some cases, the Case Manager serves
as client advocate to help obtain services and assists clients with short and long term needs. The
Case Manager will maintain accurate and timely client records and work in collaboration with the
Direct Service Manager to provide case management and coaching. All job duties are completed
with the intention of supporting and stabilizing individuals and families in crisis in accordance with
PRC's policies and procedures.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Maintains strict confidentiality of individuals and families using PRC services
Case manages 50% of PRC caseload, approximately 90-110 case contacts per month.
Independently assesses individuals and families using PRC services and develops an
action plan to help them achieve short and long-term stability.
Analyzes the need for financial assistance of individuals and families using PRC services
and determines appropriate expenditures for utility, housing, food, and other critical
family needs.
Assesses eligibility for CalFresh, formerly known as food stamps, and other public
benefits; assists applicants in completing applications and coordinates submission and
follow up for benefits.
Provides information and referral to other helping agencies in San Mateo County
Documents client eligibility for programs with screening requirements
Records client service, budget, and case notes in case management database
Intervenes and advocates for individuals and families using PRC services with various
public and private agencies to gain access to services and helping them achieve their
goals.
OTHER DUTIES AND RESPONSIBILITIES:
Assists Direct Service Manager with onsite and off-site food distribution programs, such
as the free bread and produce days, Family Harvest, and the Produce Mobile
Serves as back up to Administrative Assistant, as needed.
Represents PRC in meetings with core agencies, San Mateo County Human Services
Agency, and other partners.
Completes additional tasks as requested or assigned.
EDUCATION AND EXPERIENCE:
BA in social work, psychology, counseling, or other social service field, or 10+ years
experience providing social services.
At least three years of experience working in the social services field.
Prior experience working with safety net services highly desirable.
KNOWLEDGE, SKILLS AND ABILITIES:
Ability to effectively assess and work with individuals and families in crisis, who often
have complex and multiple issues to address.
Knowledge of safety net services available in San Mateo County.
Engaging, flexible style with individuals and families, while still being able to set
appropriate limits
Ability to maintain appropriate, professional boundaries and confidentiality.
Empathetic listening and interviewing skills without judgment.
Knowledge of and sensitivity to issues of ethnicity, race, class, gender, and sexual
preference.
Bi-cultural and bilingual skills in Mandarin, Cantonese, or Tagalog preferred.
Excellent organizational and administrative skills with the ability to complete high quality
and timely documentation.
Intermediate skills in MS Office Programs, in particular Word, Excel, and Outlook.
Ability to work independently, as well as in partnership with other staff and/or other
helping agencies.
Excellent interpersonal, communication, and writing skills.
Flexibility, sense of humor, tact and diplomacy desirable.
Creative problem-solver and ability to take initiative
ORGANIZATIONAL RELATIONSHIPS:
The position reports to the PRC Direct Service Manager and works closely with core agencies,
San Mateo County Human Services Agency and other safety net or social service agencies.
PHYSICAL DEMANDS:
This position requires up to 5hours per day sitting, talking, and listening; standing and walking
for up to 3 hours per day; 3+ hours per day at computer keying and reaching with arms and
hands; and, lifting up to 35 pounds.
WORK ENVIRONMENT:
Most work is done in an office setting. Some travel within the city and county for meetings,
trainings, and programs is required on a regular, monthly basis. At least twice per month,
services are provided off-site.
