Maintenance Contract
Poulsen’s has issued this Contract on your behalf for your existing equipment. In consideration of the payment of the contract
premium to Poulsen Heating & Air Conditioning (1991) Inc. and in reliance upon the statements and selections made by the
contract holder and the issuing Dealer in this Contract and subject to the terms, conditions, exclusions and limitations of this
Contract set out below.
Terms of Extended Protection Plan
For purposes of this policy, the terms “we”, “us” and “our” as used herein shall refer to Poulsen Heating & Air Conditioning
(1991) Inc., acting directly on your behalf. This plan covers the functioning parts of the mechanical and electronic components
of the warranted product. Coverage is not provided under the terms of this plan unless the unit(s) has been serviced in
accordance with the manufacturer’s recommendations. This plan is a parts and labour plan for the identified product and term
only and does not provide for bodily injury liability, property damage, or any other type of liability coverage. This plan is void if
the unit(s) is worked on by any person or dealer not authorized by Poulsen’s. If parts are not available and the product cannot
be repaired, Poulsen’s will offer you a free estimate for replacement. Overtime calls, weekends and holidays are excluded but
subject to a prorated amount that will be covered.
All basic components of your equipment will be covered with the exclusion of the following:
Condensing Unit:

Fireplace:
Air conditioning leaks, refrigerant, compressor, coil and thermostat
Condensate pumps, filters, electronic air cleaners, humidifiers and thermostat
Heat exchanger, combustion chamber, glass and logs

You may choose either the 1 or 15 of the month following the date of the contract, payable by Visa or MasterCard.
The buyer’s right to cancel is available with 60 days notice after 1 year of renewal date.
