ADULT AMATUER/RECREATIONAL ATHLETIC WAIVER AND RELEASE OF LIABILITY FORM FOR REDZONE SPORTS
IN CONSIDERATION of being permitted in any way in Redzone Sports athletic sports programs, related events and activities,
the undersigned acknowledges, appreciates, and agrees that:
1.
The risk of injury from the activities involved in this program is significant, including the potential for permanent
paralysis and death, and while particular rules, equipment and personal discipline may reduce this risk, the risk of
serious injury and/or permanent paralysis and death does exist; and,
2.
I KNOWININGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM
THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my particicipation on and off the
playing field and/or event; and,
3.
I am 18 years old or older; and,
4.
I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe
and unusual significant hazard during my presence or participation, I will remove myself from participation and bring
such to the attention of the nearest official immediately; and,
5.
I, myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE,
DISCHARGE, AND COVENANT NOT TO SUE REDZONE SPORTS (RZS), their officers, independent contractors,
officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable,
owners and lessors of premises used to conduct the event (“REALEASEES”) WITH RESPECT TO ANY AND ALL
INJURY, DISABILITY, DEATH, or loss or damage to the person and/or property, TO THE FULLEST EXTENT
PERMITTED BY LAW, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE.
I further expressly agree that the foregoing ASSUMPTION OF RISK, RELEASE AND WAIVER OF LIABILITY AND INDEMNIY
AGREEMENT is intended to be as broad and inclusive as is permitted by the law of the State of California and if any portion
thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This agreement
applies to all past, present, and future visits and uses by me to any RZS league, RZS event, facility and property usage.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPITION OF RISK AGREEMENT, FULLY UNDERSTAND ITS
TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND
VOLENTARILY WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE
AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE
THAT IF ANY PORTION OF THIS AGREEMEWNT IS HELD TO BE INCALID THE BALANCEM NOTWITHSTANDING,
SHALL CONTINUR IN FULL FORCE AND EFFECT.
